Description


The Hospital to Home (H2H) program is a short-term care initiative designed to help patients transition from hospital to home, particularly when they no longer require acute care but need extra support. The goal is to support timely discharge from hospital to reduce awaiting long-term care (ALC) and reduce 30-day readmission rates.


As a member of the care team in the transition program, this role participates as part of the multidisciplinary team with the provision of patient care in hospital and community. The RPN provides patient care under the direction of the care team and/or physicians in accordance with hospital standards, policies and procedures. This position contributes to the shared responsibility of ensuring health and safety policies are followed to provide for a healthy environment for staff, physicians, volunteers, learners, patients and families


The Registered Practical Nurse supports the delivery of quality care to individuals transitioning from hospital to home. This includes creating individualized care plans and ensuring best practice approaches to assessment, treatment, navigation, and support for individuals and families. This role provides comprehensive patient and family teaching, administers medications, obtains medical histories, and provides guidance for other professionals within the program to ensure high-quality, standardized care.

Qualifications


Education :



  • Current certification or the willingness to obtain Naloxone, BLS and CPI certification



Experience: 



  • One (1) year of experience working in hospital, community and/or other healthcare facilities, applicants with other combinations of applicable experience may be considered

  • Previous experience working as a member of a multi-disciplinary team



Registration/Licensure: 



  • Current registration in good standing with the College of Nurses of Ontario

  • Valid Ontario Driver’s license, with a clean driver’s abstract and regular access to a vehicle



Knowledge and Skills



  • Strong knowledge and understanding of concurrent disorders and associated treatment modalities

  • Knowledge, skills and experience in assessment, treatment and rehabilitation, crisis management and advocacy

  • Ability to work as a team and collaborate with others including members from other community organizations

  • Advanced skills in reasoning, problem solving, decision-making and workload organization

  • Advanced communication skills

  • Ability to take initiative and work with minimal supervision

  • Proven initiative, judgement, decision making and problem-solving capabilities

· Ensure safe and legal usage and storage and documentation of all medications available in the first aid room. Have Collected data and obtained records about workplace injuries through comprehensive health assessments
· Assessed employees and performed medical examinations when injured confidentially and professionally.
· Provided first aid, over-the-counter medications, and immunizations (tetanus boosters, flu clinic, etc.) as needed
· Educated employers and employees on how to prevent accidents and injuries and how to provide basic first aid
· Provided case management of injuries via telehealth for workers not on site and scheduled follow-ups, as needed, on-site or through telehealth
· Worked with safety personnel to safeguard workers through participating in safety walks and through the development, implementation, and evaluation of safety policy and procedures to ensure compliance of legislative requirements
· Provided employee and employer with return-to-work assessments and modified work offers to ensure appropriate accommodations are provided for safe and gradual return to work
· Have a clear understanding of the OHSA Regulations as presented in the 'Rainbow Chart'
· Liaised between management, occupational health physician, safety team, claims team, human resources, and other health specialties (internal and external resources) for the best plan of care for the employee and their well-being
· Conducted follow-up appointments and adjusted treatment plan according to employee's health status
· Complete point-of-care testing for post-incident and reasonable suspicion A&D testing
**Qualifications**
+ Must currently be an occupational nurse, registered nurse, or nurse practitioner
+ Minimum 5 years of nursing experience, critical care experience preferred
+ Ability to work and be onsite approximately 8-10 hours/day
+ Valid CPR, ACLS, BTLS/PHTLS, audit ATLS
+ Case management experience preferred
+ Strong computer skills

**Veterinary Receptionist**
**Location: Toronto, ON**
**Employment Type: Full-time**
**Role Type: Existing**
**Compensation:** **$17.85 to $25.00 per hour, depending on experience.**
**Beaches Animal Hospital** is currently welcoming a **Veterinary Receptionist** / **Animal Care Attendant** to join our team. This is an exceptional **full-time** opportunity for an organized, motivated, and empathetic individual to join us in a very important support role for the entire hospital. In this role, you will help our clients and their pets feel welcomed and appreciated!
+ Create a warm and welcoming environment for our patients and pet parents
+ Ability to answer a multiline phone system and track and book appointments effectively
+ Placing outbound calls to pet parents for follow-up and future appointment bookings
+ Respond and manage email correspondence; clients, referring veterinarians & internal office messages
+ Admitting and discharging pet patients
+ Invoicing/billing; creating estimates, obtaining pet owner confirmation, and processing payments
+ Scheduling appointments
+ Excellent customer service skills; a real aptitude for putting people at ease and creating a welcoming environment
+ General housekeeping duties; cleanliness of reception area, pet holding areas, and common areas
+ Willingness to learn more regarding; veterinary services, updated medical office procedures, and assisting the Veterinary staff with various tasks and treatments
+ Previous customer service or front desk experience in hospitality is required
