The position involves:

Education & Knowledge:

+ Provide Occupational Health practices in accordance with company standards and all applicable regulations in the Province of Ontario.
+ Provide emergency care/technical support to site emergency response planning including but not limited to, medical emergency response, crisis management.
+ Work with third party vendors to facilitate health services programs and resources based on business needs.
+ Attain/maintain current knowledge of the site risks/hazards including ergonomics.
+ Manage and maintain confidential P&G electronic medical records and off-site paper records as required by law.
+ Maintains medical directives related to your nursing practice and in compliance with the College of Nurses of Ontario Standards of Practice for RNs and under the direction of the site contract physician.

Job Qualifications
Qualifications:
+ Candidates must be a Registered Nurse in good standing with the College of Nurses of Ontario

**Job purpose**
The Field Nurse Case Manager (FNCM) will play an essential role in supporting patients living with a dermatological disease to improve both their health outcomes and quality of life. They will report to the Medical Director within the Medical Affairs department. FNCM will be accountable for day-to-day nursing and administrative operations of the patient support program (PSP), general oversight within their assigned territory. The successful candidate will represent Sun Pharma and work closely with the designated PSP.
**Duties and responsibilities**
FNCM offers enrolment and reimbursement support to physicians and medical case management services to patients. Assist the physician with the completion of program-specific paperwork and follow-up on submissions pertaining to obtaining drug reimbursement/coverage through private and provincial drug formularies to ensure patients have access to their prescribed treatments in a timely manner. Act as a key resource to physicians who are prescribing this medication and continuous support for renewals, and coverage changes.
+ Educate physicians and/or their staff on various PSP services
+ Liaise with physicians offices, pharmacies, service providers and other parties to provide comprehensive and coordinated service and resolve issues as they arise.
+ Provide therapy guidelines to manage physician expectations with respect to reimbursement
+ Assist MDs in completing Special Authorization (SA) forms, enrolment forms, prescription renewals, and other documentation
+ Provide ongoing support and updates to physicians on their patients
+ Customize current program elements to meet the needs of assigned physicians: adjust based on in-clinic and remote MD needs.
+ Provide product support
+ Participate in scheduled physician meetings with Sun Pharma or PSP personnel
+ Regular travel to various physician offices
+ Responsible for defined key performance indicators with in the given territory
+ Acts as a liaison between PSP and external clinical staff
+ Participate in quality assurance activities and continuous improvement initiatives
+ Provide in-person and virtual support to internal and external stakeholders.
+ Work directly with PSP and clinic to secure funding for patients therapies
+ Adhere to Performance Standards and Procedures required by SUN and the PSP
**Additional responsibility (Only applicable to customer facing roles)**
Report any adverse event received from customers for company products to the pharmacovigilance department / Drug Safety on the same day or within 1 business day
**Qualifications**
+ Graduate of Registered Nursing Program and Registered/Licensed Practical Nursing program holding current registration for Ontario province is required.
+ Minimum 2 years experience in Patient Support Programs or equivalent is required
+ Ability to engage with a physician and their staff as a trusted and well-informed medical counterpart
+ Ability to resolve conflict in a professional manner
+ Strong sense of organization and attention to detail, accurate data collection
+ Self-regulation of time management and the ability to multi-task and adhere to deadlines
+ Familiarity with PIPEDA and how it applies in a confidential patient environment
+ Ability to work in a quiet working environment with proven ability to work autonomously within a remote/virtual team environment
+ Adhere to Performance Standards and Procedures required by the PSP (Patient Support Program)
+ Previous Case Manager experience or Drug Reimbursement Navigation experience would be an asset.
+ Understanding of provincial and private coverage criteria, Special Authorization processes and how to navigate the Canadian drug coverage landscape is an asset
+ Strong critical thinking and problem-solving skills
+ Strong computer and technical skills: Excel, Word, Outlook, Tablets
+ Must be fluent in verbal and written communication in English. French is an asset but not mandatory.
**Working conditions**
+ Field based out of Greater Toronto Area, Ontario

The presently-anticipated base compensation pay range for this position is $101,000 to $123,500. Actual base compensation may vary based on a number of factors, including but not limited to geographical location and experience.
