Niigaaniin is looking for a Life Long Care Support Worker to support our community members and team at our Indigenous Friendship Center in Elliot Lake! PURPOSE
Under the direction of the Enji Maawinjidiyaang Start Up Coordinator the Life Long Care Support Worker will work collaboratively to promote high quality of life and personal independence through support for Urban Indigenous Seniors and Members of all ages.
The Life Long Care Support Worker will provide culturally appropriate community support services to urban Indigenous individuals that are elderly, frail, physically disabled, and chronically ill, which promote an integrated response, community participation, independent living and an improved quality of life. The program provides security checks, transportation, adult day programming and congregate dining, and Indigenous support services.
The Life Long Care Support Worker will facilitating activities and programs to promote physical, mental, emotional, and spiritual wellness. This position supports in the initial development and set up of Enji maawinjidiyaang as well as promotion, coordination and delivery of workshops and community engagement.
The Life Long Care Support Worker will assist Urban Indigenous families and community members in accessing Enji Maawinjidiyaang via membership enrolment and participation in group and individual services, workshops, transportation, advocacy and programs to support independent living and good quality of life.
QUALIFICATIONS
Requirements:
Specialized training or experience working with Indigenous seniors and vulnerable populations in a community setting long term care,- It is preferred the applicant identify as an indigenous person,
- First Aid/CPR certified or open to training for certification,
- Ability to work flexible hours,
- Superior oral and written communication skills, excellent time management skills with the ability to multi-task in a fast-paced environment, and
- Must have a clear Criminal Reference Check and Vulnerable Peoples Sector Check applicable to the responsibilities of this position, and
- Class G Drivers license and access to a vehicle.
Knowledge, Skills and Abilities:
Knowledge and understanding of Anishnaabemowin, culture and traditions of Indigenous people of the Robinson Huron Treaty,- Knowledge of chronic health conditions and gerontology-related issues is an asset,
- Knowledge of health-related legislations and/or any other policies relevant to program delivery,
- Ability to deliver programming to improve health conditions, i.e elder abuse prevention, diabetes prevention and / or management, chronic health condition management, etc,
- Effective crisis intervention skills / prevention skills,
- Excellent case management skills,
- Ability to coordinate programs and services,
- Ability to facilitate workshops and programming while meeting target outcomes,
- Familiar with community services networks and knowledge the Niigaaniin Services would be an asset,
- Ability to work in a performance-based system with a proven track record of meeting targets and outcomes is essential,
- Requires minimum supervision, able to work independently and maintains a supportive and cooperative environment with colleagues and the communities,
- Ability to use Microsoft 365 Platform; Word, Excel, PowerPoint, OneDrive, SharePoint and Outlook,
- Strong organizational and interpersonal skills,
- Excellent communication (verbal/written),
- Excellent stress management skills,
- Efficient time management skills with adherence to timelines and deadline, and multitask in a fast-paced environment,
- Ability to learn new skills and be flexible to adapt to changing workload, and
- Able to work independently and maintains a supportive and cooperative environment with colleagues and the communities.

The Family First Family Health Team (FHT) is looking for a committed and experienced 0.8 FTE mental health social worker with a background in primary care to join our dedicated team of allied health providers and administrative personnel. Collectively and proudly we aim to improve the quality of and access to comprehensive primary care in the communities of Orleans, Navan, and Blackburn Hamlet.
Permanent Part time (30 hours per week)
Role and Responsibilities
To provide patient-centered care, as part of a multi-disciplinary team, to patients rostered to Family First FHT’s physicians. The role consists of a combination of clinical and program work, including but not limited to:
- Counselling child, youth, adults, and families;
- Facilitating education sessions for groups of 10-14 people;
- Delivering mental health group programs (e.g. Working with Emotions DBT group, Mindfulness group, Coping with Anxiety, Optimizing Wellness CBT group)
- Completing psycho-social assessments;
- Improving existing and developing new health programs.
Essential Requirements
- Minimum 3 years counselling the youth population;
- 3-5 years of recent continuous experience in the assessment, treatment, and management of common mental health problems;
- Has a strong commitment to a collaborative care model and can work in a team effectively;
- Treats all people with professionalism, courtesy, discretion, and respect;
- Has a leadership attitude that is positive, encouraging, and that contributes to a positive work environment for all;
- Acts with diligence, reliability, and commitment to their work.
Ideal/Preferred Requirements
- Experience/certification in Dialectical Behaviour Therapy (DBT), Cognitive Behaviour Therapy (CBT), and Acceptance and Commitment Therapy (ACT);
- French (or other language abilities);
- Experience with EMR
Education and Training Requirements
- Master of Social Work (MSW) from a recognized school of social work
- Registered Social Worker (RSW) with the Ontario College of Social Workers and Social Service Workers.
