360°ree;kids has been a leader in the delivery of innovative support services for over 30 years as we work to improve the lives of homeless and at-risk youth and families in York Region. We are a vitally important community agency serving all of York Region. At 360°ree;kids, we provide a unique continuum of care approach with a range of programs and services that are essential to the health and well-being of youth and that are proven strategies for helping to keep youth off the streets by providing safe housing options.

Provide youth with supervision and basic needs (food, bedding, and hygiene) while they emerge from crisis. The Child and Youth Worker will work with the youth to assess their immediate needs, provide support to the residents, maintain order and to assist the residents achieve success in transitioning to securing housing.

Key Responsibilities:

  • Follow up with youth regarding their case plan, goals, and activities, and make referrals or do warm transfers as required. Youth worker will be a prime worker to up to 4 youth at a time that are residing in the connections program.
  • Work with youth to assess their housing needs and help them look for housing. (Youth with higher needs will be referred to the Housing Support Worker).
  • Make appropriate referrals to Housing Workers, ATS, Individual and Family Counsellors, Employment, Education, Specialized Support Workers such as Mental Health or Addictions, and others as required.
  • Provide youth with encouragement and help connect them to supports that will help them address challenges in their lives and, once re-housed, will provide supports that help them to maintain their housing.
  • Provide ongoing daily support, assistance and encouragement to break the cycles of poverty and homelessness.
  • Facilitate weekly resident meetings to communicate programs available to residents and receive feedback and address program/client concerns. Workers receive and record concerns from residents and follow up until resolution is achieved. A meeting is held each week on a designated day and time.
  • Provide support, crisis intervention and conflict resolution as required.
  • Conduct Exit Interview with client and enter discharge information into HIFIS. Clean out room of discharged client.
  • In the event of a crisis or critical incident, immediately reports to appropriate level.
  • Provide youth with a safe place to stay and basic needs (food, bedding and hygiene) while they emerge from crisis.
  • Within 72 hours of admission, conduct the full Intake process with clients, using the VISPDAT needs assessment tool, the Rapid Re-Housing, Triage, and Intervention program, and the HIFIS case management goal-setting tool.
  • Responsible for nightly room checks; to ensure safety of the residents and facility. Medication administration, monitoring, storage, and disposal (as required).
  • Enforces house rules, policies, and procedures. Make regular rounds throughout program areas and communicate regularly with staff from other program areas
  • Follows appropriate policies and procedures of 360°ree;kids. Ensures that the program operates in line with 360°ree;kids’ mission, vision and values and activities comply with applicable standards and laws.
  • Completes all mandatory recording and reporting including daily documentation, statistical data collection, recording case notes in HIFIS and input required data fields into HIFIS and Client Records.
  • Attends training workshops and relevant conferences that pertain to the role. Other duties as required.


Education and Skills Required:

  • Post-Secondary degree/diploma in a Child/youth related field (e.g. Child and Youth, Social Service Worker, Social Worker, Psychology, Sociology) or the equivalent combination of education and experience working with youth.
  • Minimum of three years’ experience working with “at-risk” or street involved youth.
  • Experience using a case management system or HIFIS and VI-SPDAT an asset.
  • Therapeutic Crisis Intervention (TCI), Suicide Prevention training an asset.
  • Current First Aid/CPR certification an asset.
  • Current Broad Record Check (BRC).
  • Demonstrated extensive knowledge of working with youth, including understanding of the regulatory environment (e.g. CFSA).
  • Demonstrated client focused counselling skills and role modelling.
  • Must possess excellent communication skills and have the ability to be adaptable with a flexible approach taking into consideration the unique qualities of each client.
  • Exceptional interpersonal skills that promote positive team building, negotiation, and conflict resolution.
  • Will possess a broad knowledge of community resources and the ability to strongly advocate for clients and professional referrals.
  • Shift timings Mon-Fri rotation 7am-3pm, 3pm-11pm, 4pm-12am.

Wasauksing First Nation is accepting applications for On-Call Social Navigator & Intake worker (Mon-Fri, Evenings, 4:00pm-12:00am). We are looking for compassionate, caring and self-driven individuals to join shelter team. Under the direct supervision of the Housing Manager, the Social Navigator provides non-judgmental support, attends the 24 hour intake line, advocacy and resource information to potential on and off reserve Indigenous residents and shelter guests overnight. They act as the first point of contact for men and women and children, welcoming new and returning users of the emergency domestic violence/intimate partner violence shelter, providing a calm process of shelter intake procedures, immediate crisis counselling and referrals to other supportive social services as necessary.

Job Duties & Responsibilities:

  • Be a leader in maintaining a warm, welcoming, respectful, and non-judgmental environment, while ensuring the support and security of staff, volunteers and guests.
  • Interview clients individually, in families, or with referral source, to assess their situation and needs to determine eligibility and types of services required.
  • Provide informal counselling in crisis and assist clients in organizing a plan of safety.
  • The Shelter values and protects confidentiality of client information. All staff are to ensure clients personal information is safeguarded appropriately.
  • Provides referrals to appropriate resources.
  • Keep up to date with Wasauksing Health and Social service program opportunities for WFN guests.
  • Completes and maintains related records including intake registration, service requests and referrals if not admitted.
  • Duties will also include completing bag, purse and pocket checks, a tour of the facility, review of general housekeeping policies and procedures upon admittance.
  • Maintains the smooth operation of the emergency shelter.
  • Maintain daily documentation and information sharing with staff
  • May occasionally be required to participate in incident investigation, including providing written statements or other documentation as necessary.

Education & Experience:

  • A degree in social work/counselling psychology, social service worker diploma, an asset, or an equivalent combination of education, training, and experience.
  • Minimum 2-3 years experience working in front line in social service delivery is required.
  • Current full registration withthe Ontario College of Social Workers and Social Service Workers an asset.
  • Demonstrated knowledge of Indigenous worldviews and perspectives of health, wellness and traditions of caring.
  • Knowledge of surrounding community supports and services.
  • Demonstrated ability to work independently while maintaining inclusive and collaborative relationships with the rest of the shelter team.
  • Excellent interpersonal skills (de-briefing, active listening, conflict resolution, de- escalation, problem solving, supportive) is essential.

Program Overview – Youth Justice Program
Turning Point Youth Services (TPYS) is an accredited, non-profit agency serving youth (12 to 24 years of age) and their families in the east end of Toronto. Turning Point’s mission is to empower youth to make positive change, an to accomplish this, it offers a continuum of services for youth and their families with a focus on mental health, housing/sheltering, and youth justice. Turning Point Youth Services and WoodGreen Community Services announced on July 2, 2024 the integration of the two organizations. The combination of these two agencies will enable a more effective and integrated approach to the provision of comprehensive support services for youth in the GTA.

What You Will Do

  • Promote a safe and therapeutic environment and establish therapeutic relationships with clients through role modeling consistent with the Agency Mission, Vision and Values.

  • Provide direct, client care and supervision to clients in the live-in program.

  • Develop, implement, evaluate and fully participate in life skills and recreational programming, where necessary.

  • Represent the Agency in a professional manner when dealing with families, external professionals, general public and community partners.

  • Participate in ongoing training and supervision sessions on a regular basis.
  • Participate in all mandatory training provided by the Agency.
  • Work in accordance with all policies and procedures of the Agency.

About the Position:

Working full time in Sheshatshiu and reporting to the Clinical Lead, this position requires an individual with a Social Work background. The incumbent will be part of the Social Worker Prevention Services Team, partnering directly with Indigenous Representatives and Community Workers who are Sheshatshiu Innu First Nation (SIFN) members. Working together, the team will develop and deliver prevention services for Innu children, youth and families who are members of SIFN. The Social Worker has direct responsibility to help oversee the ongoing implementation of the Innu-CSSD Protocol. The Protocol outlines the commitment by both Prevention Services and CSSD to share information and work collaboratively on the continual improvement of culturally meaningful services for Innu. The Social Worker is an ally to all Innu coworkers, directly partnering in the delivery of advocacy services and case management processes. The work will be undertaken consistent with the Innu Care Approach, reflecting Innu culture and values.

Duties and Responsibilities:

  • Continuously build knowledge about Innu culture by engaging in ongoing learning opportunities related to Innu history and the Innu vision for collective health, including but not limited to the Innu Healing Strategy, the Innu Care Approach and the Sheshatshiu Innu timeline
  • Collaborate with other community health workers across the First Nation to strengthen connections between Innu health services and programs
  • Foster productive working relationship with the provincial child protection staff (CSSD) in Sheshatshiu and other offices as needed
  • Develop prevention activities and strategies reflecting Innu culture and the Innu Care Approach to meet community needs
  • Support prevention staff in the development and coordination of intervention plans and services

Job Summary :

Under the direction of the Program Manager, the Social Worker works within a multi-disciplinary team to provide evidence-based, trauma- informed counselling to individuals affected by mental illness and substance use issues.

Staff assist clients in the areas of interpersonal relationships, mental health and addiction issues, housing and support services, self-management of medication, social recreation, activities of daily living, life skills and community preparedness as well as others recovery oriented areas identified by clients. Coast staff work in partnership with family members, aboriginal communities, and community agencies and services who are committed to positive outcomes for clients.

Typical Duties:

  1. Provide individual counselling to address unique to R&R clients’ needs and challenges, including unresolved trauma, emotional dysregulation, problem solving, relapse prevention, etc.
  2. Plan and implement therapeutic group sessions utilizing a variety of frameworks, including Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, Cognitive Remediation, and Mindfulness.
  3. Contribute to program and policy development by advising of the program development requirements to meet the needs of the clients.
  4. May be required to: plan, coordinate, and deliver educational sessions to clients and their family members on topics such as detoxification, relapse prevention, stress management, wellness, self-care, problem solving, coping skills, etc.
  5. Monitor, assess and evaluate client’s progress in collaboration with other members of the health care team to ensure program meets client/family needs and the effectiveness of group therapy. Identifies problems and implements established protocol for high risk clients in crisis and makes adjustments to treatment needs in consultation with the health care team.
  6. Provide formal and informal staff education and competency building activities by facilitating / co-facilitating in-services, preparing educational materials and compiling resources related to evidence-based therapeutic approaches.
  7. Contribute to multidisciplinary input and team building by participating in regular client progress reviews and team meetings.
  8. Initiate and participate in individual professional development; maintain current knowledge and skills in relation to addiction, mental health, and evidence-based therapeutic approaches.
  9. Maintain a safe environment for clients, visitors and employees by adhering to established safety and emergency practices and procedures and reporting any risks and anomalies to the Coordinator.
  10. Participate in research and quality improvement activities by identifying client care issues, collecting data, and identifying needs and recommendations for corrective action and changes to practices, procedures or protocols.
  11. Perform other related duties as required.

Requirements

  • Bachelor's degree of Social Work from an accredited University.
  • A minimum of three (3) years’ experience in a similar setting with a mental health or addictions related agency;
  • Registration with the BC College of Social Workers
  • Ability to provide/obtain the following upon hire:
    • Valid First Aid Certificate ("Emergency First Aid - Community Care" or equivalent)
    • WHMIS certificate
    • A Ministry of Justice criminal records check

Knowledge/Abilities:

  • Knowledge of issues facing persons with a psychiatric disability or substance use issue, particularly as related to meeting client needs of finances, legal/criminal justice involvement, family supports, housing, and advocacy.
  • Thorough understanding of the Mental Health Act.
  • Experience in tracking and reporting outcome measures.
  • Experience in collaborating with community case managers, housing providers, and other agencies to determine the effective utilization of available resources
  • Experience in therapeutic group development and facilitation skills
  • Case management experience.
  • Understanding of recovery principles, including strengths-based case management
  • Excellent communication skills and the ability to work effectively as part of the team.
  • Knowledge of CBT, DBT, motivational interviewing and trauma recovery
  • Knowledge of mental illness, substance use issues, psychosocial rehabilitation principles, and therapeutic modalities.

Job Summary:

Join our team of dedicated health professionals to support youth and young adults ages 17 to 24 identifying as female. Set in a peaceful quiet neighbourhood, 3 blocks from the beach, in sunny White Rock, this setting is ideal for those who love the outdoors! You will have the opportunity to utilize your knowledge and skills to support individuals who may have experienced mental health, physical health, trauma, family/cultural disconnection and substance use. The program utilizes a two-eyed seeing approach, weaving Indigenous cultural traditions of healing with western medicine to build a program that is holistic, healing and flexible for the unique individual needs of participants. The program operates under a harm-reduction lens to support individuals in learning what healing, wellness and treatment mean for each participant.

Our focus will be community-driven, both within the program itself by establishing a sense of community between participants and employees (eating meals, participating in activities and healing practices such as yoga/meditation, going on walks together). We will also be connecting individuals to resources within the community – both in Surrey, and individuals home communities. There will be a focus on land-based healing practices, adventure therapy and skill building.

Typical Duties:

  • Community outreach within the Maple Ridge area, connecting with youth that are not accessing services and are in need of supports obtaining community services (mental health, physical health, addictions, criminal justice issues, employment, finances, housing, education and/or other needs defined by the youth)
  • Ability to work later hours and weekends to connect with youth.
  • Navigating, liaising and effectively communicating with services for youth in Maple Ridge and advocating for supports on their behalf.
  • Supporting youth in obtaining and attending appointments to connect with GP/NPs and other health professionals.
  • Supporting youth with dealing with Criminal Justice System (probation, court attendance, etc.)
  • Working with other on developing a restorative justice approach for youth.
  • Ensures the implementation of the programs including, leisure activities, cooking, housekeeping and maintenance tasks by engaging the clients daily life skills activities.
  • Provides community based Psycho social rehabilitation services by, identifying and supporting clients in accessing and developing links into community programs and services.
  • Teaches and models life skills and strategies necessary for self care, independent living and interaction with outside social agencies.
  • Applies a range of effective communications strategies such as role modelling to establish a collaborative rapport with the clients.
  • Provides the support and information necessary to help clients build self-esteem and develop decision- making skills through role modelling and other techniques.
  • Assists clients to identify and gain access to formal and informal community supports such as leisure, vocational, educational services through active community integration; providing community services resources of community services.
  • Observes and records the physical and mental health status of clients and take necessary action.
  • Advocates for and with the clients to encourage them to maintain their rights and dignity.
  • Works with clients, peers, supervisors, other agencies and relatives in a cooperative and professional manner.
  • Develops individualized life skills programs with clients involving, where appropriate, family, friends and community resources.
  • Ensures all requirements of the QWIK process and administrative tasks are completed within the allocated timeframes.

  • Distributes medications safely in accordance with medication policies and procedures.

Required Qualifications

  • Diploma in Child & Youth Care, Community Social Service Work, or an equivalent combination of education, training and experience or other qualifications determined to be reasonable and relevant to the level of work
  • Recent, related experience of two years

    • Certificates in First Aid + CPR, WHMIS and FoodSafe

We're looking for an Addictions Support Worker to join our Black Health and Social Services Hub - ACTT/EPI Team!


The Addictions Worker will conduct comprehensive assessments, provide client centered stagewise brief interventions and case management using a harm reduction model. This position will work closely with other professionals through the provision of addictions expertise and linkages between mental health and primary health care.
