Job Summary:
We are seeking a compassionate and dedicated Disability Support Worker to provide care and support to individuals living with physical, intellectual, or developmental disabilities. The role involves assisting clients with daily living tasks, promoting independence, and enhancing their quality of life in a safe and respectful environment.

Key Responsibilities:
  • Assist clients with personal care, hygiene, meal preparation, and mobility
  • Support clients in achieving personal goals and participating in community activities
  • Implement individualized support and behavior management plans
  • Administer medications and assist with health-related appointments, as required
  • Maintain accurate records of care, incidents, and progress
  • Ensure a safe and supportive environment, respecting client dignity and privacy
  • Communicate effectively with families, healthcare providers, and support teams
  • Encourage independence and skill development
Required Skills:
  • Strong interpersonal and communication skills
  • Patience, empathy, and a genuine desire to help others
  • Ability to work independently and as part of a care team
  • Problem-solving and decision-making abilities
  • Understanding of disability support principles and person-centered care
  • Basic computer skills for record-keeping and reporting
Work Experience:
  • Previous experience in disability support, personal care, or related fields is preferred
  • Experience working with individuals with intellectual or developmental disabilities is an asset
Education Requirements:
  • High school diploma or equivalent (required)
  • Certificate III or IV in Disability, Community Services, or a related field (preferred)
  • CPR and First Aid certification (may be required)
  • Valid driver's license and background check (may be required depending on the role)

**Job Summary**
The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care.
**What you will do:**
+ The Clinical Social Worker completes appropriate intake documentation, provides ongoing assessment, develops treatment plan in collaboration with the multi-disciplinary treatment team. The Clinical Social Worker also manages multiple complicated cases, maintains current knowledge of ever-changing community resources, monitors patient needs and progress, develops aftercare plans. Maintains documentation on patient throughout hospitalization.
+ Provides brief therapeutic interventions with patients and/or families, as well as community staffings, in coordination with the physician and treatment team. Works as in integral member of the treatment team in the provision of treatment and aftercare planning.
+ Ensures compliance with privacy and confidentiality laws. Interfaces regularly with legal entities to address treatment needs.
+ Assists with monitoring and evaluation of services provided for department quality improvement plan.
+ Maintains current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, religion and utilizes appropriate communication skills.
+ Provides information, education and direction in response to community requests.
+ Regularly accesses pharmaceutical and community resources to provide patients with needed psychiatric medications.
+ Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA's. Must maintain current licensure through continuing education. Must uphold all ethics, rules and regulations required by the Clinical Board.
+ Diagnoses, assesses and addresses both mental health and chemical dependency conditions. Performs risk assessments for suicidality, homicidality and grave disability.
+ Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA's. Must maintain current licensure through continuing education. Must uphold all ethics, rules and regulations required by the Clinical Board.
+ Screen and identify patients who need care management per high-risk criteria.
+ Assess, develop, implement and monitor a comprehensive discharge plan of care through an interdisciplinary team process in conjunction with the patient and family. Collaborate with the multi-disciplinary team to identify problems or needs that require special planning, intervention, teaching or follow-up
**What you bring:**
+ Master's Degree Required: Social Work from an accredited graduate school of social work
+ 2 years Required: Clinical experience with designated population and
+ Preferred: Hospital, emergency room, chemical dependency, or community agency experience and
+ Preferred: Knowledge of community resources, knowledge of Medicare, Medicaid and insurance requirements as these relate to continuing care.
+ Required: Clinical Social Worker Required Credentials Must have LCSW, LICSW, LMSW, CSWA, SWAICO, SWAAL, SWAL status.

PeaceHealth is seeking a Care Management/Clinical Social Worker for a Part Time, 0.50 FTE, Day position. Hourly compensation is $45.14 - $67.70; hiring rate dependent on qualifications and professional experience.
**Job Summary**
Responsible for identifying and interacting with medically and psychosocially complex patients and families who are likely to benefit from care management and meet high risk criteria and for coordination of discharge planning services for these patients in collaboration with RN Care Management and other members of the care team.
**Details of the position**
1. Screen and identify patients who need care management per high risk criteria.
2. Assess, develop, implement and monitor a comprehensive discharge plan of care through an interdisciplinary team process in conjunction with the patient and family. Collaborate with the multi-disciplinary team to identify problems or needs that require special planning, intervention, teaching or follow-up.
3. Identify key problems, strengths, and resources to be addressed in the discharge plan of care. Coordinate and facilitate improved ability to comply with plan of treatment; counseling or support needed to cope with situation; improved ability to access appropriate level of care due to lack of financial resources or lack of available service.
4. Actively support measures that promote effective use of resources.
5. Identify, plan and arrange for appropriate services applying a knowledge of services available in the community, state, and federal health regulations and admission, discharge and appropriate level of care. Coordinate effective planning and arranging for needed services upon discharge.
6. Intervene by arranging services, education and providing psychosocial support to prepare the patient and their family to manage their healthcare needs within the acute care setting and post discharge.
7. Performs other duties as assigned.
**What you bring**
+ Master's Degree in Social Work from an accredited graduate school of social work.
+ Must have LICSW, or must have CSWA (Clinical Social Work Associate) status.
