The Biidaaban Healing Lodge is a 12-bed residential treatment program located on Biigtigong Nishnaabeg formerly known as Pic River First Nation, which is approximately 25 kilometers from Marathon, ON. We are mandated to provide quality treatment to all Aboriginal and Metis people residing in the Robinson Superior area. We provide treatment for unresolved trauma stemming from sexual abuse, family violence, adult children of trauma, grief and abandonment and anger solutions.

Primary Responsibilities:

Working as a team member within the Healing Lodge and in conjunction with the First Nation communities in the area and under the direction of the Executive Director, the community outreach workers are responsible for:

  • To design and deliver educational programs that exemplify prevention, promotion and aftercare services and deliver them at community level
  • To develop an effective evaluation tool that assesses the needs of our First Nation communities and to define strategies that will meet the needs identified through community needs assessments
  • To deliver mobile treatment and training units to meet the needs of individuals, families and communities within the Robinson Superior area in the areas of Mental Health First Aid First Nations, lateral violence, grief and loss and trauma recovery
  • To identify promotional strategies aimed at publicizing the Biidaaban Healing Lodge programs at community level
  • To deliver programs in conjunction with Biidaaban Healing Lodge staff as needed and required

Qualifications:

  • A college degree in Social Work; Psychology or a related field of study with three to five years counselling experience, preferably in a treatment setting with the ability to do individual and group therapy

(A combination of education and work experience will be considered.) We are willing to provide training to the right candidate

  • Experience working with First Nations individuals, families and communities, and a working knowledge of healing and recovery from a Native perspective.
  • Ability to work independently and as part of a team with strong communication skills, a working knowledge of computer programs as a word, excel, publisher, power point and outlook to develop pamphlets, posters and other promotional items
  • An understanding of data entry systems would be an asset
  • An understanding of the issues that impact First Nation communities and the Anishinaabek way of life

Position Overview

Qualifications

The ideal candidate for this position will have post-secondary education in Social Services or a related field, along with experience supporting individuals with complex needs such as mental health challenges, substance use, and homelessness. They must demonstrate strong interpersonal skills, empathy, cultural sensitivity, and the ability to engage marginalized populations effectively.

Key qualifications include First Aid and Naloxone training, crisis response experience, knowledge of community resources, and a valid Class 5 BC Driver's License with access to a reliable vehicle. The role requires flexibility, risk awareness, ethical practice, and a commitment to ongoing professional development. Additional assets include Food Safe and Violence Prevention Training.

About the Role:

The Settlement Outreach Worker plays a critical role in assisting immigrants and refugees with their integration into the local community. This position involves outreach, engagement, and providing essential information and support to newcomers. Through an integrated services approach, the Settlement Outreach Worker provides one-on-one detailed needs assessment and referrals, one-on-one information and orientation, group information and orientation, community connections, client outreach, service bridging, resource development, and delivery assistance to eligible clients.

Responsibilities:

  • Actively engage with immigrant and refugee communities to raise awareness about available settlement services.
  • Conduct outreach activities at local events, community centers, schools, and places of worship to connect with newcomers.
  • Build trust and rapport with immigrants and refugees to encourage participation in settlement programs and services.
  • Assess client needs, including strengths and barriers, in their first language where possible, to assist in identifying immediate and long-term needs.
  • Develop and monitor individualized settlement plans based on identified needs, available services, and community supports.
  • Offer guidance on accessing government services, including applying for permanent residency, health benefits, and other public services.
  • Provide individualized support to clients, tracking their progress and assisting them in meeting their settlement goals.
  • Provide translation and interpretation support to clients.
  • Follow up with clients to ensure they are receiving the support they need and address any barriers to accessing services.
  • Plan and deliver group/individual orientation and information sessions on topics related to settlement.
  • Work within defined program budgets.
  • Maintain accurate participant files in accordance with funder and YMCA guidelines.

Qualifications:

  • Post secondary degree/diploma in Social Work, Human Services, or related field.
  • Intermediate (Standard) First Aid, CPR-C.
  • Non-Violent Crisis Intervention and Suicide Intervention Certification preferred.
  • Able to obtain a Police Information Check with Vulnerable sector.
  • Minimum 2 years of experience working with immigrant, refugee, or marginalized populations in a settlement or social service setting.
  • Strong working knowledge of issues/practices related to immigration and refugee issues, social determents of health, harm reduction, individual assessment, and case management.
  • Strong working knowledge in culturally sensitive and trauma-informed practice, and strength-based approach.
  • Fluency in multiple languages is highly desirable, especially languages spoken by local immigrant or refugee populations.
  • Ability to work independently and collaboratively as part of a team in a fast-paced, dynamic environment.
  • Flexibility to work occasional evenings or weekends as needed for community events or meetings.
  • Proficiency in Microsoft Office, databases and other technology.
  • Annual registration in a professional organization as required by law.
  • Valid driver’s license and has a reliable vehicle.

What Youll Be Doing

  • Conduct outreach efforts to identify and connect with homeless individuals that require assistance. Once identified, add individuals to the BNPL through the communities coordinated access initiative.
  • Meet with potential clients, assess, and determine the appropriate housing response to an individuals or a familys request for support services and accommodations to resolve their housing needs.
  • Provide resources and referrals to individuals or families that assist with problem solving and address immediate needs.
  • Conduct housing searches and build inventory lists of available housing units for candidates.
  • Liaise with property managers, landlords, realtors, and other sources to identify and secure vacant properties for client placement.
  • Assertively engage with clients to secure housing including unit rental, damage deposits, securing income assistance if required, setting up of utilities, signing of leases, and arranging for furnishings and household items to establish their homes.
  • Ensure appropriate warm transfers to Housing First or other agency support teams/programs.
  • Document all activities.
  • Provide support as needed to the Outreach Coordinator.

What You Bring

  • Bachelors degree in Social Services, or related field
  • 2 years experience in case management
  • Knowledge of community programs and services in Windsor Essex County is Essential
  • Excellent conflict resolution, communication, and advocacy skills
  • Must have a valid drivers license, automobile insurance, and use of your own vehicle
  • 2 years experience in the field of housing and homelessness, mental illness, or drug use preferred

What Youll Do

Reporting to the Manager, Outreach Programs, the Peer Outreach Team works as part of an interdisciplinary team to organize and support individuals dealing with substance use, mental health challenges, and homelessness.

The Peer Outreach Team will work in conjunction with all outreach programs at Somerset West CHC and Supportive Housing & Substance Use Health programs to provide a safe and accepting environment where clients can discuss their challenges without fear of judgment. This helps to reduce stigma and encourages open communication. The Peer Outreach Team will help clients access information about harm reduction strategies, such as drug poisoning prevention, and mental health resources. They will also connect clients to other services, such as: basic needs, primary care, treatment options, housing and counselling services. They may also offer support in developing coping strategies, life skills, and decision-making abilities. This empowerment will help clients take control of their lives and make informed choices. Additionally, the Peer Outreach Team will advocate for their clients within healthcare and social service systems, helping them navigate complex barriers and access the services they need.

In times of crisis, the Peer Outreach Team can provide immediate support, helping clients manage situations related to their substance use or mental health. Peer Outreach Team will provide ongoing encouragement and motivation, helping clients set realistic goals and celebrate their achievements, no matter how small.

Life and Work Experience

    • Living and lived experience of intravenous drug use, homelessness, street involvement or sex work is an asset.
    • Experience advocating for people who use drugs is an asset
    • Experience with overdose response is an asset
    • High School or postsecondary education is an asset
    • 1-2 years experience volunteering or working in a harm reduction, drop-in or shelter program is an asset.
    • A police record check is required for this position; however, candidates with certain criminal charges may be considered on a case-by-case basis, depending on the nature of the offense.

Physical/Mental/Emotional Requirements:

This position will require the ability to:

  • lift and carry items, ~approx. 25-30lbs per shift, through extended foot outreach, and ~approx. 50lbs in standing position.
  • Walk approximately 5km over the course of a shift, including on uneven ground and with limited visibility; also the ability to mobile outreach support
  • Make rapid safety and risk decisions using strong clinical judgment.
  • Assess the safety to enter unknown locations (usually in public).
  • Collect and safely dispose of used inhalation and injection equipment
  • Work within a diverse team and collaborate with partner agencies.
  • Engage with people in crisis and use conflict de-escalation skills when needed
  • Manage multiple urgent, competing tasks daily
  • Expect exposure to psychological and emotional stressors, vicarious trauma, and potential risks of violence.

Durham Community Health Centre (DCHC) is a registered, charitable organization that provides integrated, accessible, and equitable community based primary care, wellness services, and health education to Durham community members who face multiple barriers to their health and well-being.

DCHC also provides equity-based programs and services that focus on priority populations such as Indigenous, Black, the 2SLGBTQI Community, Newcomers to Canada, Seniors, and Unattached patients (i.e., those without a family doctor), to name a few. Position Overview

The Indigenous Outreach position and initiative is the result of an innovative partnership between Miwizii Biik Housing Development Corporation and Durham Community Health Centre. The (MBDC) was established in 2004 by Miwizii Biik Aboriginal Employment and Training, with a focus on training and affordable housing.

The Indigenous Outreach Worker position will provide culturally competent services across Durham Region with vulnerable and street-involved First Nation, Métis and Inuit individuals who may be at risk or have contracted Covid-19. The primary focus of this position is to respond to the housing/health/safety needs of individuals who may be negatively impacted by Covid-19.

The Outreach Worker assists clients using a wholistic approach focused on wellness and safety, decreasing risks and challenges facing the Indigenous Community as well as rebuilding and attending to practical needs (ie. shelter, food insecurity, transportation, housing subsidy and isolation issues). This position coordinates and organizes services for individuals, offers support to people that are vulnerable and living on the street or are in compromised housing arrangements.

The Indigenous Outreach Worker will collaborate within an interdisciplinary team of health professionals across the community network that influence the health environment targeted to Indigenous clients, as well as within the Agency’s team structure, in support of goals that align with the response to supporting Indigenous Peoples.

Key Responsibilities

Outreach Services

  • Provide a range of Outreach support services that promote wholistic wellbeing for those with experiencing homelessness, at risk of homeless or in a crisis situation.
  • Provide financial aid to those needing short term financial support for personal services such as prescription medication costs, physiotherapy services, transportation, food vouchers, etc.
  • Provide a range of opportunities for clients to learn daily life and living skills, while increasing their sense of community and belonging and connection to culture and traditions as necessary.
  • Provide comprehensive culturally appropriate resources and culturally relevant service plans.
  • Educate clients about their tenant rights and responsibilities.
  • Contribute to the coordination, planning and enhancement of local services, including those from Indigenous agencies and partners.
  • Support the implementation of realistic and culturally relevant goals and objectives through regular community and home visits.
  • Act as an advocate on clients’ behalf
  • Access and utilize community resources for clients.
  • Keep detailed client records to provide quality care.
  • Coordinate and submit expense claims to financial services.
  • Participate in events, workshops/orientations for clients and community
  • Coordinate, support and facilitate Indigenous based programming.

Administration, Quality Improvement and Reporting

  • Document within DCHC’s EMR, information regarding outreach contacts.
  • Complete Incident Reports as required.
  • Follow policies and standards regarding confidentiality and documentation.
  • Ensure all necessary reports are completed accurately and in a timely manner.
  • Attend relevant team meetings and planning activities.
  • Participate in quality improvement and evaluation activities.
  • Work with the health and safety guidelines provided by the agency, which includes annual mandatory training requirements

Key Qualifications

  • Completion of a formal 2-year post-Secondary program at a Community College (Health/Social Services).
  • One year experience working with the Indigenous community
  • Broad knowledge of Indigenous issues, history, culture and tradition
  • Experience providing support services
  • Current knowledge of community programs, services and resources
  • Highly organized and compassionate
  • Understanding and willingness to practice Covid-19 safety protocols
  • Available to work a flexible schedule
  • Valid driver’s license is required, and regular access to a vehicle

The Indigenous Outreach Worker is expected to make face to face contact with individuals from a 2- metre distance. Durham CHC will provide PPE in the form of face masks, gloves, and disinfectant.

  • Must be able to work in a variety of social and physical settings including walking on uneven surfaces and unexpected conditions (ex. tent city).
