Position:
Nurse Practitioner (NP)- Full Time Permanent.

Reports to:
Executive Director, Owner/Operator

Position Summary:

  • The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.
  • The NP lead clinical services would include ongoing assessments that will be documented in the plan of care as well as a review and an update of the plan of care for each resident at least twice per year.
  • The NP will complete a comprehensive geriatric assessment (CGA) for the management of care for frail seniors.
  • The Nurse Practitioner will coordinate and work in collaboration with inter-professionals such as the other members of the care team, a social worker, an occupational therapist, a pharmacist, and a BSO Clinician to offer geriatric clinical integrated care to support frail older adults and seniors who live at SRR.
  • The NP will provide direct patient care focusing on promoting health, maximizing patient safety, and function that will support frail seniors living in the retirement home community.
  • The Nurse Practitioner functions within their expanded scope in a highly autonomous manner to use their expert skill to formulate clinical decisions and appropriate diagnosis and manage acute/chronic illness and wellness promotion for all residents. Care is conducted in the patient’s suite and SRRs in-clinic.


Education:

  • Master's Degree in Nursing, and successful completion of the Ontario Primary HealthCare or Adult Nurse Practitioner Certificate Program.
  • Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario).
  • Thorough understanding of the RN(EC) scope of practice and core competency framework.
  • Current CPR and First Aid Certificates.
  • Canadian Certificate in Gerontology an asset, with three-five (3- 5) experience working with older adults and frail seniors.
  • Desired: Training in P.I.E.C.E.S.TM, Gentle Persuasive Approach, U-First!, Montessori, an asset


Experience and Skills:

  • Preferred 1-3 years of experience working with older adults and frail seniors.
  • Demonstrated extensive knowledge and skill working with individuals with various types of Major Neurocognitive Diseases, Behavior, and Psychological Symptoms of Dementia (BPSD), delirium, mental health, addictions, neurological conditions, and chronic conditions commonly affecting seniors.

Position Overview

The Nurse Practitioner is responsible to provide appropriate assessment and care to patients in a virtual clinic environment. Essential Duties:

  • Assessing client presentation, evaluating urgency of care needed, selecting appropriate level of care, facilitating referrals to other providers where appropriate, and determining follow up required
  • Independently and collaboratively assesses, diagnoses, and prescribes in accordance with professional standards, legislation, policies, and clinical practice guidelines
  • Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
  • Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client's permanent health record
  • Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals, or other services in the interest of the continuum of care
  • Follow best practice guidelines

Qualifications:
  • Graduation from a Nurse Practitioner program
  • Current or willingness to obtain licensure as a Nurse Practitioner with the College of Registered Nurses of Newfoundland and Labrador 
  • Graduation from an approved School of Nursing
  • Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
  • Experience working in emergency or urgent care considered an asset
  • Demonstrated knowledge of home and community health services
  • Family All Ages (FAA) is required
  • Experience practicing in a rural setting considered an asset
  • Exceptional clinical assessment and decision-making abilities
  • Demonstrated leadership qualities and problem-solving skills
  • Strong oral and written communication skills
  • Demonstrated ability to work well independently and in a collaborative team environment
  • Must have very strong computer and technology skills to be able to deliver virtual care through the use of technology
  • Experience and aptitude with e-consult is an asset

  • In consultation/collaboration with the Oncology Program Medical Director and patient care team, the Nurse Practitioner engages in direct clinical practice by:

Job Description

The Nurse Practitioner will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing the care of eating disorder clinic patients who require assessment and ongoing monitoring of physical status. The Nurse Practitioner abides by the practice standards as set out by the College of Nurses of Ontario.

Accountabilities:

  • Assessing patient medical status as it relates to Eating Disorders diagnoses
  • Collecting and interpreting data about the health of the patient

Qualifications

  • Masters degree in nursing/ACNP/current extended class registration RN (EC) required
  • CNA certification in mental health would be an asset
  • Nurse Practitioner designation to treat across the lifespan is required
  • Nursing experience in working with patients who meet DSM5 diagnostic criteria for an eating disorder is required
  • Nursing experience in working with other mental health diagnoses
  • Exemplary clinical nursing skills, knowledge and expertise, especially in patient assessments and consultation with family physicians

With a multidisciplinary team of healthcare professionals, NFCHC is committed to empowering individuals and fostering a healthier, more connected community.

Purpose

Reporting to the Clinical Director, the Nurse Practitioner (NP) provides care for unattached babies and children in the context of a broader pediatric team. NP provides and manages primary care for marginalized clients with varying degrees of social and medical complexities in accordance with current Canadian Community Health Nursing Standards of Practice. The NP's approach to client care is holistic and utilizes current client education and therapeutic techniques. The NP collaborates, consults, and refers to a multidisciplinary team (internally and externally) including RNs, Dietitian/Lactation Consultant and Family Support Worker. This individual is a natural leader who practices to the full scope of practice, remains informed on evidence-informed practices, has the willingness to participate in quality improvement and research initiatives, and has a constant desire to learn both as an individual and as a collective team. A passion for pediatrics, newcomer health and reducing other healthcare barriers.

Accountabilities and Responsibilities

Duties and responsibilities include, but are not limited to:

  • Provides client diagnosis, assessment, planning, implementation, and evaluation in person and virtually in accordance with the College of Nurses of Ontario standards of practice and under specific medical directives while maintaining and managing care for a full team roster.
  • Delivers advanced care to specialized populations
  • Responsible for collaborating with all team members in a respectful and professional manner, which includes consultation with family physicians when patient care requires assessment or treatment that is beyond the NP scope of practice or current level of expertise.

Position Summary:
The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high-quality, client-centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.

Main Responsibilities/Duties:

  • Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.
  • Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.
  • Prescribe medications and therapeutic interventions within the NP scope of practice.
  • Manage and coordinate care for patients with chronic and complex health conditions.
  • Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals.
  • Provide patient education, counseling, and advocacy to promote health and well-being.
  • Support the development and implementation of health programs tailored to the needs of refugee and pediatric patients.
  • Maintain accurate and timely electronic medical records (EMR) and documentation.
  • Participate in quality improvement initiatives and professional development activities.
  • Support and contribute to our company’s commitment to workplace equity, diversity, and inclusion.
Education and Employment Requirements:

  • Successful completion of a post-graduate Primary Care Nurse Practitioner program.
  • Current Extended Class Registration with the College of Nurses of Ontario.
  • Proof of current membership or alternative professional liability coverage.
  • Current CPR certification (Level C or HCP).
  • Demonstrated experience in community and/or primary care.
  • Experience in chronic disease management and prevention.
  • Strong ability to work both independently and collaboratively within a healthcare team.
  • Excellent oral and written communication skills in a clinical setting.
  • Proficiency in the use of electronic medical records (EMR); Accuro experience is an asset.
  • Passion and experience in Refugee Health and Pediatric Care are considered strong assets.
Additional Information:

  • Employment Type: Permanent, Full-Time (37.5 hours per week) Part-time arrangements will be considered.
  • Salary: $52.10 - $62.76 per hour + on-call stipend

To be successful as our new NURSE PRACTITIONER, you will:

  • Hold a current registration as a Nurse Practitioner with the College of Nurses of Ontario

The Nurse Practitioner will provide support and guidance in completing assessments, ordering, interpreting, and communication diagnostic testing and procedures, provide support for and lead resident centred palliative care services, and participate in the clinical review of Village quality initiatives.
