Position Overview

Reporting directly to the Director of Care, the Registered Nurse (RN) is responsible for providing our residents with quality care. In providing a range of nursing care to residents with a variety of physical and / or behavioural concerns, the RN will collaborate with health team members to facilitate positive outcomes for the resident.

Key Tasks and Responsibilities:  
 

  • Provide nursing care in accordance with administrative policies, physician orders, and the Standards of Nursing Practice as described by the College of Nurses of Ontario.
  • Determine, collect and organize the information required for care planning by utilizing all available resources, reporting pertinent information to other members of the multidisciplinary health care team.
  • Collaborate and participate with other members of the health care team in care planning and to act as a resource person to other staff members.
  • Assess a resident's condition by recognizing and interpreting signs and symptoms and, where appropriate, initiate remedial health measures.
  • Assess, plan and implement appropriate interventions, and evaluate nursing care for individual residents.
  • Assume responsibility for the administration of medications and treatments as prescribed and for the safekeeping of drug and narcotics.
  • Identify and understand the needs of the residents, setting realistic short and long-goals.
  • Maintain records for each resident in compliance with policies of Sherwood Park Manor.
  • Assume maximum safety for residents and staff, ensuring incident or accident reports are completed.
  • Develop and modify the resident care plan as determined by the biopsychosocial assessment of the resident's individual needs, evaluating regularly for effectiveness.
  • Assist with health teaching for residents, including collaborating with the Assistant Director of Care and/or Director of Care in performance appraisals and reviews.
  • Perform additional related duties as required.

Skills and Knowledge:
 
  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Experience working in Long Term Care desirable.
  • Current CPR and First Aid certification.
  • Skill in planning, co-ordinating and supervising the work of other nursing personnel.
  • Training in the Gentle Persuasive Approach and U-First is considered an asset.
  • Experience using PointClickCare an asset.

The RPN’s duties are to provide optimum individualized resident care under the direction of the Registered Nurse in accordance with Standards of Nursing Practice, College of Nurses of Ontario, Long Term Care Homes Act and all other applicable legislation.   • Monitors and evaluates resident needs and care goals regularly according to LTC standards and policies and procedures by analyzing and interpreting overall resident outcomes and modifying each plan of care as needed using professional judgement.
  • Reports to RAI Coordinator regarding resident assessment and flow sheet documentation observations.
  • Develops staff to achieve performance expectations through orientation, supervision, teaching, peer review, coaching and mentoring.
  • Assists in development and evaluation of unit goals and objectives, improving nursing standards, and development of policies and procedures.
  • Ensures compliance with routine practices and effective implementation of appropriate infection control protocols based on surveillance of risks and outbreak management in collaboration with the RN and Director of Care.
  • Attends all relevant in-house services regarding Occupational Health and Safety and follow all guidelines for employees and employers as legislated under the Ontario Occupational Health and Safety Act.

Position Summary :

Under the supervision of the Department Manager, coordination of the Team Lead, and working as a member of the unit team, the Registered Nurse is responsible for the provision of quality nursing care to a patient or group of patients. Position Summary:

The Team Leader supports the Medical-Surgical program (specifically the transitional care unit on 1E) by acting as a resource for frontline staff and works in partnership with the inpatient medicine physicians. This position demands an exceptional level of nursing practice expertise as well as the personal ability to effectively coach and nurture the staff being supported to provide quality care. Reporting to the Medical-Surgical Manager, the Team Leader is responsible for ensuring the overall coordination and facilitation of effective, efficient, and safe clinical activities, promoting interprofessional collaboration, supporting continuous quality improvement, and liaising with other patient care areas as required. The Team Leader is ideally positioned to provide problem-solving, consultations, and clinical support to nursing colleagues, interprofessional team members and students.

The Team Leader has the overall picture of the Medical-Surgical program and creates the staff assignment to match patient needs with staff skill set; this includes Team Leaders taking patient care assignments to maintain competence and skills. The Team Leader works to balance as many variables as possible, ensuring that the daily operations and organization of the department support the delivery of quality care within existing resources. The Team Leader supports patient flow across the entire organization and works in partnership with the other departments to facilitate flow.
