Job Summary

Reporting to the Director of Integrated Health Services, the Nurse Practitioner (NP) is a key member of the interdisciplinary team, delivering comprehensive primary health care services in both the Primary Health Care (PHC) clinic and the Homelessness and Addiction Recovery Treatment (HART) Hub. Practicing to full scope, the NP provides culturally responsive care, working both independently and collaboratively within an interprofessional team.

The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes health promotion, counseling, disease prevention, diagnosis, treatment, and referrals to specialists or diagnostic testing. Both the PHC clinic and the HART Hub provide clinical services, requiring the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.

The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring accessible and high-quality care for individuals with diverse health, social, cultural, and economic needs. Functioning both autonomously and within a collaborative team environment, the NP contributes to the advancement of program goals, enhancing health outcomes within the community.

Requirements

Education

What Youll Do

The Somerset West Community Health Centre (SWCHC) is excited to announce the launch of the 24/7 Homelessness and Addiction Recovery Treatment (HART) HUB, a transformative initiative designed to provide integrated support to people experiencing complex health and social issues related to substance use and addictions: and aims to connect people who are experiencing homelessness or are at risk of losing their housing and/or people who have mental health, to treatment and preventative services, while addressing basic needs such as food, clothing and showers.

What Youll Do

Reporting to the Manager, Supportive Housing and Substance Use Health, the Nurse Practitioner (NP) works as part of an interdisciplinary team to provide health care & treatment to clients who reside in supportive housing units and support with medical stabilization beds. The NP will collaborate closely with members of the Supportive Housing and Substance Use Health team such as Basic Needs Coordinators, RNs, Peer Coaches, System Navigators, Housing Case Managers, and Counselling services.

Your Day-to-Day at a Glance

The Nurse Practitioner (NP) provides a comprehensive range of services, including triage, physical assessment, diagnosis, treatment, counselling, health promotion, and preventative care, tailored to clients with diverse health, social, cultural, and economic needs.

Working to the full scope of practice within the Walk-in Clinic, the NP will be expected to function both independently and as part of an interprofessional team, delivering care in a culturally sensitive manner. The successful candidate may also be required to provide services during evening hours (until 8:00 p.m.) and/or on weekends.

In addition, the NP will:

  • Maintain a progressive roster of clients for whom they serve as the most responsible provider.
  • Collaborate closely with the team, particularly the Nursing Care Coordinator, to plan and coordinate care for clients in Supportive Housing.
  • Participate in community outreach opportunities with partner organizations, which may include on-site visits, delegation of responsibilities, or implementing medical directives in collaboration with other NPs and the nursing team.

What we are looking for:
The Nurse Practitioner (NP), Community Health Clinic, will provide comprehensive primary care services for unattached patients over the age of 6 months within our geographic area of the Ontario Health Team. The NP will also provide support as needed for other patient types requesting primary care in the community such as palliative patients, paediatrics, and those searching for Cancer screening. The NP will need advanced knowledge and decision-making skills to provide a comprehensive health focused approach in meeting the needs of this population.
The NP will practice within the interprofessional team model and be responsible for promoting best practices in complex medical, geriatric and palliative care , paediatrics and promote excellence in the nursing care of complex and frail adults through direct patient care and consultation. The NP will work collaboratively with OHT medical lead, primary care physicians, other NPs and community partners caring for patients in the community. The NP will work closely with the Medical Lead in establishing priorities for this new model and all other programs available and offered by the Ontario Health Team partners.

What you bring to the role:
Master’s Degree in a related field or working towards
Current registration from the College of Nurses of Ontario as an RN (EC) - Nurse Practitioner-Primary Health Care (PHCNP) or Nurse Practitioner-Primary Care
Demonstrated current expertise in primary care and management an asset
Membership in the Registered Nurses’ Association of Ontario & Nurse Practitioner Association of Ontario, preferred
National certification in other specialities including Gerontology Nursing from the Canadian Nurses Association and paediatrics, preferred
Advanced clinical knowledge and implementation of evidenced based practice
Superior interpersonal, coaching, and leadership skills
Excellent decision making, problem recognition and problem solving skills
Demonstrated commitment to continuing professional development

Mission:

Reporting to the Director of Health and Wellness (DHW) / Director of Care (DOC), the Registered Nurse (RN) is responsible to provide direct resident care and medication administration and document care in compliance with regulatory and Extendicare Resident Health and Wellness standards and policies and procedures.

  • To complete and ensure residents' care assessments, service plans and documentation are up to date and changes communicated to DHW/DOC for billings;
  • To assist with scheduling of residents' care conferences and participate in conferences;
  • To work closely with DHW/DOC to facilitate residents' move in and transfers;
  • To order and safe keep drugs and narcotics and administer in compliance with all regulatory requirements and Extendicare policies and procedures;

  • To supervise resident self medication assessment and audits;

Qualifications:
