Nurse Practitioner – Virtual Care

Work from home as a Nurse Practitioner with flexible scheduling models available

Fonemed is expanding its services and is looking for passionate and dedicated Nurse Practitioners to add to its exceptional and growing team in a work from home telehealth position. At Fonemed, you will have the opportunity to work with a team of over 50 Nurse Practitioners and to create a schedule that works for you. You will assess patients using telephone and video technology. Through virtual care, we meet and provide care for individuals where they are. Position Overview

The Nurse Practitioner is responsible to provide appropriate assessment and care to patients in a virtual clinic environment. The successful applicant works independently in diagnosing conditions, prescribing medications, performing, or ordering diagnostic tests and treatments, evaluating treatments, and providing follow-up care within the continuum of care. The Nurse Practitioner provides comprehensive and holistic nursing care to clients including health promotion, injury and illness treatment and prevention.

As an advanced practitioner, the Nurse Practitioner functions in the roles of practitioner, educator, and leader. As governed by provincial regulations, he/she practices independently, collaborating with other healthcare professionals including primary care physicians for issues outside the Nurse Practitioner scope of practice.

Essential Duties:

  • Assessing client presentation, evaluating urgency of care needed, selecting appropriate level of care, facilitating referrals to other providers where appropriate, and determining follow up required
  • Independently and collaboratively assesses, diagnoses, and prescribes in accordance with professional standards, legislation, policies, and clinical practice guidelines
  • Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
  • Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client's permanent health record
  • Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals, or other services in the interest of the continuum of care
  • Follow best practice guidelines

Qualifications:
  • Graduation from a Nurse Practitioner program
  • Current or willingness to obtain licensure as a Nurse Practitioner with the College of Registered Nurses of Newfoundland and Labrador 
  • Graduation from an approved School of Nursing
  • Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
  • Experience working in emergency or urgent care considered an asset
  • Demonstrated knowledge of home and community health services
  • Family All Ages (FAA) is required
  • Experience practicing in a rural setting considered an asset
  • Exceptional clinical assessment and decision-making abilities
  • Demonstrated leadership qualities and problem-solving skills
  • Strong oral and written communication skills
  • Demonstrated ability to work well independently and in a collaborative team environment
  • Must have very strong computer and technology skills to be able to deliver virtual care through the use of technology
  • Experience and aptitude with e-consult is an asset
  • A commitment to professional development

  • A satisfactory background screening result 
  • You must have your own computer system with 2 monitors, USB headset, webcam, and high-speed internet connection to connect through VPN

  • Must be able to work staggered shifts, possibly extended shifts.
  • Excellent assessment, organization, communication, and team interaction skills.
  • Current immunization for hepatitis B, influenza, measles, mumps, rubella, tetanus, and diphtheria, COVID-19.
  • Current tuberculin skin testing (within 6 months after date of hire).
  • Certification and recent experience with IV initiation. PREFERRED
  • Experience with care of an intubated patient. PREFERRED
  • Experience with ECG analysis. PREFERRED
  • Experience in an acute care post-anesthetic care unit environment, with PACU experience PREFERRED
  • Advanced knowledge of regional anesthesia, neuroaxial anesthesia, multimodal analgesia and chronic pain. PREFERRED

Physical Requirements

  • The work environment is physically demanding and may require different levels of physical exertion, such as lifting and transferring patients.
  • At times, RNs may work in challenging and stressful circumstances, such as in an emergency situation.

Our locally owned Saskatoon branch is growing, and were looking for a compassionate, well-connected, and driven Community Care & Engagement Lead to help expand our reach and impact. This part-time, flexible role is ideal for someone with a nursing background (active or retired) who enjoys both connecting with people and making a difference in their lives.

Role Summary
Youll play a dual role in both client acquisition and care delivery setup. You'll help generate leads through outreach efforts, conduct in-home consultations with prospective clients and families, and guide them through the onboarding process including assessments and care plans. Your clinical credibility, community connections, and compassionate approach will be key to helping families feel confident choosing ComForCare. Your input will directly shape how we serve our clients and grow in the Saskatoon area. Youll work with flexibility and independence, while being supported by a committed local owner.

Key Responsibilities
Generate and convert new client leads through outreach, community networking, and in-home consultations
Build and maintain relationships with hospitals, clinics, retirement residences, physiotherapists, social workers, and other senior-serving professionals to generate referrals and increase brand awareness
Represent ComForCare at community events, health fairs, and stakeholder meetings
Conduct care consultations in client homes to understand needs and help close the care agreement
Complete admission documentation, perform non-medical assessments and develop personalized care plans
Provide guidance to caregivers based on client needs and oversee care delivery
Follow up with families to ensure satisfaction and address concerns
Work closely with the owners to support overall business growth
Additional duties may include providing support with client intake, caregiver scheduling, and caregiver recruitment initiatives as needed

What Were Looking For
A retired or active Registered Nurse, Director of Care, or experienced senior care professional
5+ years of experience in nursing, care coordination, or health services (active or retired)
Strong knowledge of client care and home support needs (especially for seniors and persons with disabilities)
Experience with community outreach, relationship-building, or marketing is considered an asset
Well-connected in the health care, senior care, or community services network
Deep understanding of local resources and referral pathways is a strong asset
Excellent relationship-building and communication skills
Organized, reliable, and able to work independently from home and in the field
Valid drivers license and access to a reliable vehicle
Preference will be given to local candidates with established relationships in the community

Hours
  • Approximately 10 hours per week (flexible hours)
