Position Overview
The Nurse Practitioner is responsible to provide appropriate assessment and care to patients in a virtual clinic environment. - Independently and collaboratively assesses, diagnoses, and prescribes in accordance with professional standards, legislation, policies, and clinical practice guidelines
- Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
- Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client's permanent health record
- Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals, or other services in the interest of the continuum of care
- Follow best practice guidelines
Qualifications:
- Graduation from a Nurse Practitioner program
- Current or willingness to obtain licensure as a Nurse Practitioner with the College of Registered Nurses of Newfoundland and Labrador
- Graduation from an approved School of Nursing
- Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
- Experience working in emergency or urgent care considered an asset
- Demonstrated knowledge of home and community health services
- Family All Ages (FAA) is required

Purpose
Reporting to the Clinical Director, the Nurse Practitioner (NP) provides care for unattached babies and children in the context of a broader pediatric team. NP provides and manages primary care for marginalized clients with varying degrees of social and medical complexities in accordance with current Canadian Community Health Nursing Standards of Practice. The NP's approach to client care is holistic and utilizes current client education and therapeutic techniques. The NP collaborates, consults, and refers to a multidisciplinary team (internally and externally) including RNs, Dietitian/Lactation Consultant and Family Support Worker. This individual is a natural leader who practices to the full scope of practice, remains informed on evidence-informed practices, has the willingness to participate in quality improvement and research initiatives, and has a constant desire to learn both as an individual and as a collective team. A passion for pediatrics, newcomer health and reducing other healthcare barriers.
Accountabilities and Responsibilities
Duties and responsibilities include, but are not limited to:
- Provides client diagnosis, assessment, planning, implementation, and evaluation in person and virtually in accordance with the College of Nurses of Ontario standards of practice and under specific medical directives while maintaining and managing care for a full team roster.
- Delivers advanced care to specialized populations
- Responsible for collaborating with all team members in a respectful and professional manner, which includes consultation with family physicians when patient care requires assessment or treatment that is beyond the NP scope of practice or current level of expertise.
- Maintain professional competencies and a professional license and liability insurance to practice (where applicable).
- Act as a member of an interdisciplinary team that assists with the development, analysis, and implementation of patient programming and services.
- Provide point of care in other community settings as identified by leadership.
- Responsible for maintaining high-quality standards and timely documentation in the Electronic Medical Records (EMR).

The Nurse Practitioner (NP) reports to the Executive Director and collaborates with the resident, family/caregiver, and health care team in the development, implementation and evaluation of the residents’ plan of care, as well as provides leadership and mentorship to team members that enhances their knowledge, assessment skills, and ability to care for residents.
Responsibilities:
- Diagnose residents, order and interpret diagnostic tests, and prescribe medication and other treatment,
- Provide direction and leadership to RN's, RPN’s, PSW’s, and other direct care team members in all aspects of resident care, within established legislative standards,
- Complete nursing documentation, and assist residents with move-in,
- Is inspirational and passionate about quality dementia care and capturing positive moments
- Work collaboratively with attending physicians,
- Contribute positively to a team approach in being person centered with one another,
- Ensure medication reconciliation occurs as needed,
- Implement resident-focused programs, policies, and procedures with the Director of Nursing and the Quality Team,
- Provide education to clinical team and offer in-services,
- Liaise with Residents, families and outside agencies as required Respects the Residents' Bill of Rights
- Any and all other tasks as assigned
Qualifications:
The qualifications needed to join our family are as follows:
- Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
- Post-graduate education and experience in gerontology (recommended)
- Specialty certification in gerontological nursing (recommended)
- Experience working as a Nurse Practitioner,
- Experience in long-term care, with providing empathetic care for the elderly,
- Experience providing leadership and supervision in any healthcare environment.
- Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
- Must have a clear vulnerable sector screen, no older than 6 months, TB test results less than 6 months old, must be able to provide three supervisory reference
