Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Patient Care Coordinator has responsibility for the day-to-day operations of the assigned unit(s) including daily patient flow. Supports/participates in research, policy, quality improvement and risk management activities. The Patient Care Coordinator performs their duties with a unit or hospital focus and supports program and/or regional activities.

Responsibilities

Applies established criteria and prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes and by reviewing the admission/intake and discharge planning of patients and individual care plans to ensure optimal progression to discharge, in collaboration with the other clinical team members.
Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis; observes work and corrects where necessary, monitors the quality of care and the safety of patients, assigns and adjusts work assignments, schedules and coordinates staff, monitors and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education regarding illness prevention and health maintenance, acting as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
Initiates/leads/supports plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
Identifies learning requirements of staff and, in conjunction with the Manager, Educator and other members of the clinical team, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
Provides input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
Acts as a point person for patients and families who have concerns or questions regarding their care; follows ups as required. Participates in or leads the investigation of complaints by patients/families; follows up on complaints and implements corrective action as appropriate. Collaborates with Quality Improvement according to established protocols.
Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
Assists the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
Promotes staff awareness and involvement in research/evaluation activities and special projects and participates in research and special projects as appropriate.
Collaborates with internal and external stakeholders, participates on a variety of designated professional or operational committees at the unit, hospital, program and/or regional level.
Participates in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
Performs other related duties as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience. Completion of post graduate course work and/or certification applicable to the designated clinical area.

Reporting to the Regional Director, Patient Care Quality Office (PCQO), and working closely with the Regional Manager, PCQO, the Senior PCQO Liaison identifies, manages, advises and resolves issues and concerns from patients, families, clients and residents, working to build and maintain trust, and minimize risk exposure/liability to the organization. This position synthesizes information from internal and external stakeholders, including all levels of the organization (operational and clinical leadership), to provide analysis and resolution, which have direct impact on patient care services and quality improvement. Acts as a senior resource to the PCQO team members and provides senior expertise with respect to effective complaint management and processes. Other responsibilities include maintaining a system for tracking, analyzing and reporting client feedback activities and identifying trends and opportunities for improvement. Liaises with staff in Quality Improvement & Patient Safety, Patient Experience, Risk Management, Legal Services and Information Access and Privacy.

As a Senior Patient Care Quality Liaison with Vancouver Coastal Health you will:
Provide leadership in all aspects of complaints management in support of patients/families/clients, and all levels of the organization, to ensure timely and effective resolution of issues/concerns and identify areas for improvement.
Ensure obligations and responsibilities are met with respect to Ministry of Health legislation and directives in the context of the Patient Care Quality Review Board Act.
Coach and direct PCQO team members with building and maintaining trust amongst internal and external stakeholders during the complaint process, determe appropriate follow up actions in response to patient/family complaints and related situations.
Recommend departmental standards for effective tracking of issues, actions and follow up activities, using sound judgement regarding the management of client feedback issues, including both verbal and written communication and courses of action when initial steps do not resolve issues to a complainant's satisfaction.
Refer issues with potential risk or liability to Risk Management, Information Access & Privacy and Communications.
Serve as a liaison between patients/families, the organization, and the community, assist patients/families to know and understand their rights and responsibilities with how to navigate the organization's processes.
Coordinate conferences between patients/families and their representatives and administrative staff as appropriate.
Maintain data quality of client feedback information in the BC Patient Safety & Learning System (PSLS), ensure patient/family feedback is documented, status of resolution is monitored, and activities and outcomes are analyzed and reported for accountability and improvement purposes.
Recommend and maintain policies and procedures related to patient/family/client/resident feedback.
Participate in departmental meetings, communicate needs and concerns and providing input from a client relations and quality of care perspective.
Keep abreast on relevant legislation, regulations, standards, and laws, collaborating with other departments as appropriate.
Carry out responsibilities in accordance with health and safety requirements. Immediately report unsafe situations by notifying supervisor or other appropriate personnel.
Conduct face-to-face interviews and gather detailed information.
Provide empathetic support to clients while establishing clear boundaries in respect to the role the PCQO has in reviewing care concerns.
Qualifications Education & Experience
Master’s Degree in a relevant human services discipline.
Five to seven (5 to 7) years' recent, related experience including conflict resolution/crisis intervention experience, preferably in a health care setting, as well as experience in providing clinical patient care and directing junior staff, or an equivalent combination of education, training and experience.
Knowledge & Abilities
Advanced knowledge of complaint management processes in health care systems.
Advanced organizational skills and ability to prioritize workload under time pressures to meet deadlines.
Advanced knowledge of pertinent laws and legislation, including and not limited to, the Patient Care Quality Review Board Act, Freedom of
Information and Protection of Privacy Act, and the Residents' Bill of Rights.
Ability to exercise sound judgment, critical thinking and effective decision-making.
Ability to communicate effectively, both verbally and in writing, with all levels of the organization, including advanced complaints management best practices, presentation and writing skills.
Ability to analyze and problem-solve complex issues towards effective solutions.
Ability to lead, participate in, and facilitate inter-disciplinary groups in a collaborative environment with multiple interests.
Ability to work effectively with others, including working effectively in a multicultural environment, independently and as part of a team, and understanding the frameworks of other disciplines.
Effective facilitation and negotiation skills.
Ability to perform the duties of the position and operate related equipment, including computer software applications.

Maintain the department’s system for tracking, analyzing and reporting client feedback activities in the BC Patient Safety & Learning System (PSLS).
Provide analytics support to the Regional Manager, Regional Director and PCQO Liaisons in the tracking, management, resolution and education of patient/client/resident issues.
Works closely with all members of the PCQO team, VCH Executive Office, Quality & Patient Safety, Risk Management, and Communications and Public Affairs, referring to them all client feedback issues with the potential for liability or other risks to VCH.
Qualifications Education & Experience
Bachelor's degree in a relevant human services discipline.
Three to five (3 to 5) years of recent, related public and client relations experience preferably in a health care setting, or an equivalent combination of education, training and experience.
Knowledge & Abilities
Demonstrated ability to deal effectively with the public and staff in a courteous, tactful and diplomatic manner.
A high degree of discretion and maturity in handling confidential information in sensitive and potentially emotionally charged situations.

As an Intake Coordinator, Patient Care Quality Office (PCQO) with Vancouver Coastal Health you will:
Support Vancouver Coastal Health (VCH) in its focus on establishing and maintaining effective relationships with our patients/clients/residents.
Receive all incoming complaints, concerns, inquiries and compliments both internal and external stakeholders to VCH and determines the type and urgency of complaint/inquiry received.
Review and perform initial resolve, or may re-direct more complex complaints/concerns to a PCQO Liaison for follow up.
Maintain the department’s system for tracking, analyzing and reporting client feedback activities in the BC Patient Safety & Learning System (PSLS).
Provide analytics support to the Regional Manager, Regional Director and PCQO Liaisons in the tracking, management, resolution and education of patient/client/resident issues.
Works closely with all members of the PCQO team, VCH Executive Office, Quality & Patient Safety, Risk Management, and Communications and Public Affairs, referring to them all client feedback issues with the potential for liability or other risks to VCH.
Qualifications Education & Experience
Bachelor's degree in a relevant human services discipline.
Three to five (3 to 5) years of recent, related public and client relations experience preferably in a health care setting, or an equivalent combination of education, training and experience.
Knowledge & Abilities
Demonstrated ability to deal effectively with the public and staff in a courteous, tactful and diplomatic manner.
A high degree of discretion and maturity in handling confidential information in sensitive and potentially emotionally charged situations.
Decision-making ability to appropriately triage all incoming complaints and inquiries from patients, families and staff.
Demonstrated understanding of complaints management, the BC Patient Safety & Learning System (PSLS), quality improvement and clinical/medical terminology.
Good working knowledge of software applications, including Word, PowerPoint and Excel.
Demonstrated ability to work under pressure and meet deadlines.

Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Patient Care Coordinator has responsibility for the day-to-day operations of the assigned unit(s) including daily patient flow. Supports/participates in research, policy, quality improvement and risk management activities. The Patient Care Coordinator performs their duties with a unit or hospital focus and supports program and/or regional activities.

Responsibilities

Applies established criteria and prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes and by reviewing the admission/intake and discharge planning of patients and individual care plans to ensure optimal progression to discharge, in collaboration with the other clinical team members.
Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis; observes work and corrects where necessary, monitors the quality of care and the safety of patients, assigns and adjusts work assignments, schedules and coordinates staff, monitors and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education regarding illness prevention and health maintenance, acting as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
Initiates/leads/supports plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
Identifies learning requirements of staff and, in conjunction with the Manager, Educator and other members of the clinical team, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
Provides input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
Acts as a point person for patients and families who have concerns or questions regarding their care; follows ups as required. Participates in or leads the investigation of complaints by patients/families; follows up on complaints and implements corrective action as appropriate. Collaborates with Quality Improvement according to established protocols.
Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
Assists the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
Promotes staff awareness and involvement in research/evaluation activities and special projects and participates in research and special projects as appropriate.
Collaborates with internal and external stakeholders, participates on a variety of designated professional or operational committees at the unit, hospital, program and/or regional level.
Participates in the review, development, implementation and evaluation of evidence-informed clinical decision support tools/policies.
Performs other related duties as assigned.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience. Completion of post graduate course work and/or certification applicable to the designated clinical area.

Skills and Abilities
Clinical proficiency/expertise in the designated clinical area.
Ability to supervise and provide leadership and work direction.
Ability to coordinate patient assignments, in collaboration with staff.

Qualifications

Education and Experience

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Three (3) years of recent related clinical experience in the designated clinical area, including one (1) year of supervisory experience, or an equivalent combination of education, training and experience. Completion of post graduate course work and/or certification applicable to the designated clinical area.

Clinical proficiency/expertise in the designated clinical area.
Ability to supervise and provide leadership and work direction.
Ability to coordinate patient assignments, in collaboration with staff.
Ability to monitor staff workload and patient factors and adjust patient assignments accordingly.
Ability to establish priorities for patient care, patient flow/discharge planning and overall daily operations of the designated unit(s).
Ability to monitor the quality of patient care and ensure maintenance of standards for nursing practice and nursing support staff.
Ability to monitor, assess and address performance issues based on professional standards of practice and to provide input to the Manager on formal performance reviews.
Ability to role model, coach and mentor best practices in patient care.
Ability to identify individual or group learning priorities to meet identified program, organizational and learner needs, in collaboration with other clinical staff.
Ability to support implementation of education based on changing practices e.g. legislation and technology.
Ability to provide input into budgets and minor and capital equipment.

Detailed Overview

In accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of practice and the Mission and Values of Fraser Health, the Patient Care Coordinator provides clinical supervision, guidance and support to nursing staff, other clinical team members and support staff within the assigned unit(s). The Patient Care Coordinator has responsibility for the day-to-day operations of the assigned unit(s) including daily patient flow. Supports/participates in research, policy, quality improvement and risk management activities. The Patient Care Coordinator performs their duties with a unit or hospital focus and supports program and/or regional activities.

Responsibilities

Applies established criteria and prioritizes admissions/intake, leads and facilitates actions related to daily patient flow, plans for patient discharge and appropriate patient placement/referral by ensuring the care team follows through on specific care paths and processes and by reviewing the admission/intake and discharge planning of patients and individual care plans to ensure optimal progression to discharge, in collaboration with the other clinical team members.
Supervises nursing and nursing support staff and coordinates the work of the clinical team overall on a day-to-day basis; observes work and corrects where necessary, monitors the quality of care and the safety of patients, assigns and adjusts work assignments, schedules and coordinates staff, monitors and may authorize staff hours of work, including overtime, vacations, leave requests, ensuring service levels and staffing requirements are met.
Supports quality patient care and excellence in practice by promoting interprofessional collaboration and team building, supporting staff to plan and carry out patient/family education regarding illness prevention and health maintenance, acting as a mentor and resource to staff, role modeling best clinical practices, assessing clinical skills and by coaching staff and/or liaising with other health professional leaders to help them achieve established clinical performance goals.
Initiates/leads/supports plan of care conferences and rounds (e.g. iCare) for the purpose of optimal patient care management according to best practice; disseminates clinical information to nursing/nursing support staff and other members of the clinical team.
Identifies learning requirements of staff and, in conjunction with the Manager, Educator and other members of the clinical team, provides input into the development of learning objectives; provides clinical support and guidance and evaluates the effectiveness of learning opportunities.
Provides input into the development of a comprehensive orientation program, in collaboration with the Educator and other clinical staff.
Acts as a point person for patients and families who have concerns or questions regarding their care; follows ups as required. Participates in or leads the investigation of complaints by patients/families; follows up on complaints and implements corrective action as appropriate. Collaborates with Quality Improvement according to established protocols.
Participates in quality improvement and risk management activities by methods such as evaluating effect of changes in practice, generating recommendations for alternative approaches to practices, conducting safety audits, and supporting implementation of teaching/learning when required.
Assists the Manager/Director by attending panel interviews, providing recommendations on the selection of new staff, providing feedback to the Manager on staff performance appraisals, and providing feedback on optimal use of budgetary resources and regarding minor and capital equipment needs as requested.
