Summary of Duties
We are seeking Advanced Practice Nurses to facilitate processes to support excellence in patient-centered practice based on relevant best practice, conceptual frameworks, research literature and professional standards of practice, legislative requirements, and other scholarly resources.
Through a holistic and integrated approach, the APN works with members of the inter-professional team, patients and their families to provide comprehensive care.
We are also seeking Nurse Practitioners demonstrating a high level of autonomy and expert skill to formulate clinical decisions. In collaboration with the interprofessional team, the NP is responsible for managing, in a comprehensive manner the health needs of patients receiving systemic therapy. The NP is responsible for leading the provision of person-centred care by completing comprehensive assessments, ordering diagnostic tests and implementing therapeutic plans of care. The NP is an advanced practice role designed to contribute to the quality of patient care through practice, education, research and leadership practices according to the philosophy and standards of Sunnybrook. The NP collaborates with the Patient Care Manager, Supervisor and Advanced Practice Nurse to enhance nursing practice through the development, implementation and evaluation of best practices.
Qualifications - Advanced Practice Nurse:
Knowledge, skills, and experience in adult education, program design, teaching, group facilitation, team work and conflict management.
Proficient computer skills.
Current College of Nurses of Ontario Certificate of Competence.
Master’s degree in Nursing or health-related discipline (completed or in progress).
A strong desire to promote professional practice and excellence in evidence-based, patient centered care.
Superior interpersonal, coaching, and leadership skills to facilitate identification and development of staff learning needs.
Committed to excellence in all aspects of the discipline including professional practice, research and education.
Membership in the Registered Nurses’ Association of Ontario, preferred.
Minimum of five (5) years of relevant nursing experience.
Self-motivated, able to cope with several projects at one time.
Flexible with change and able to set priorities and problem-solve.
Qualifications - Nurse Practitioner:
Current College of Nurses of Ontario registration in the extended class (EC) as an NP (adult) required
Master in Nursing/Nurse Practitioner (MN-NP) or Masters of Science in Nursing with successful completion of a post Master’s Acute Care Nurse Practitioner Certificate Program required
Current certification in Basic Cardiac Lfe Support required•
Expert knowledge and clinical skills in managing symptoms and complex patient care needs for patients receiving systemic therapy
Demonstrated experience through roles of advanced practitioner, educator, researcher and leader
Ability to function collaboratively within an interdisciplinary team and practice in a self-directed manner
