Primary Responsibilities:

  • Perform Intake, ongoing family services and children services functions under a generic model
  • Gather information and conduct investigations and assessments of the immediate safety of the children.
  • Intervene in crisis situations, which may include movement of children at risk to a place of safety as mandated by the Child, Youth and Family Services Act.
  • Develop a relationship with the child and family to facilitate an assessment of the strengths and needs.
  • Participate in consultations, Talking Circle/OTC, and case conferences with service providers and family’s FNMI Community, to discuss plans of care and service needs of the child and family.
  • Continually evaluate child’s needs while in care and revise the plan of care as required.
  • Coordinate internal/external service requirements for child and family, and advocate on behalf of the child to ensure the receipt of culturally appropriate and timely services. Develop partnerships with these resources in service planning.
  • Determine the need for admission of children to care as required. Liaise with Placement Worker regarding placement decisions in order to ensure effective consideration of the child’s specific needs
  • Coordinate and develop Customary Care Agreements with the family, extended family and community members, and Band representatives.
  • Facilitate ongoing contact between the child and family/extended family/FNMI community. Provide counselling and support to the child and the family during access visits.
  • Intervene on child’s behalf in crisis situations, facilitate the resolution of the crisis, and develop plans in consultation with the child and service providers to reduce the likelihood of the crisis being repeated.
  • Coordinate requirements for court hearings including preparing evidentiary materials, reviewing case information with lawyers, and appearing as a witness if required.
  • Complete case recordings, reports, correspondence, and legal forms.
  • Participate in public communication and education activities.


Qualifications, Knowledge and Skills:

  • B.S.W. from an accredited university, with a minimum of 2 years relevant working experience within a Child and Family Wellbeing context.
  • Alternatively, an equivalent combination of education and experience in Indigenous family and children issues in a child and family wellbeing context may be considered.
  • A valid Driver’s License and access to a reliable vehicle are required for this position.
  • Extensive knowledge of the history and culture of Indigenous Peoples including First Nations, Métis, and Inuit.
  • Ability to provide an acceptable Child Welfare Check with Vulnerable Sector Check (VSC).
  • Knowledge of Child, Youth and Family Services Act, Abuse Protocols, Risk Assessment Model and OnLAC.
  • Knowledge of child development and demonstrated skills in the areas of assessment, diagnosis and treatment planning.
  • Knowledge of the Toronto Indigenous community and history of child welfare in the community.
  • Excellent written communication; strong verbal communication; organizational skills; ability to multitask and prioritize; initiative and respect for confidentiality.
  • Completed Authorization Training would be an asset.
  • Demonstrated understanding, and commitment to, integrating the Native Child and Family Services of Toronto Mission and values in-to practice, service and relationships.
  • Demonstrated understanding of workplace Health and Safety practices and understanding of an employee's responsibility under current legislation.

Job Summary:
This position will cover duties related to all Child Protection departments while office is closed over the weekends between the hours of 2:00PM on Friday’s and 10:00AM on Monday’s. Employees can work from home but must have a reliable internet connection. May be required to attend to matters in the community or may be required to attend the office at any time during their set shifts.

The hours of work set out as per the agreement are an essential duty of the position. It is understood and agreed that it is a mandatory requirement for the worker in this position to be available to work these hours of work, and that the ability to work After Hours (i.e. days, nights, weekend work, rotational shift schedule and up to thirteen hours per shift) constitutes an essential duty of this position.

This position will be a Monday to Friday (8:30AM to 4:30PM) role from the time of hire to February 2026, at which time the role will move to the designated weekend shifts. This will allow for the successful applicant to familiarize themselves with the role and the organization.

Key Tasks and Responsibilities:
Telephone Intake:

  • Respond to all calls received by the answering service and determine eligibility for service.
  • Complete all required documentation for any new or subsequent referrals and/or matters related to open files.
  • Complete disclosures for those requesting access to their records.
  • Complete Community Links as required.


Assessment:

  • Complete Investigative Plan and Safety Assessment for any referrals with a response time of less than seven (7) days.
  • Initiate a joint police investigation when required.
  • Develop safety plans with families to mitigate child protection concerns.


Ongoing Services:

  • Provide regular check-ins with safety plans on high-risk matters.
  • Follow up on any urgent matters that occur over the weekend and complete the required documentation for assigned workers.


Connections:

  • Provide support to children and youth placed in care or with kinship and their caregivers as required.
  • Provide regular check-ins with safety plans on high-risk matters.
  • For any new kinship placements, assess family plans and complete the initial 5-point safety assessment.


Legal:

  • Attend to all necessary legal procedures and documentation, including all court documentation and client contracts.

Functional Duties:

  • Have a clear understanding of the Children’s Aid Society’s Child Welfare Mandate, the CYFSA legislation and regulations, the agencies strategic framework, the Eligibility Spectrum (2021), and Child Protection Standards (2016).
  • Assist families to build and connect with their network support and collaterals to obtain their first-hand impressions about overall family functioning, and utilize their support to best aid the family in their own planning
  • Handle emergency admissions/taking children to a place of safety to gather relevant information about children and their families and their psychological and social functioning and to make recommendations based on psychological assessments
  • Intervene with families to prevent situations of child neglect and abuse
  • Arrange and carry out foster placements, including emergency admissions
  • Maintain documentation in accordance with Ministry standards and Agency policies and procedures
  • Be aware of roles and responsibilities under the Ontario Occupational Health and Safety Act, Regulations, and agency health and safety policies and perform duties in a manner that always ensures safety.
  • Such other duties as may be deemed appropriate.


Team Building/Interpersonal Duties:

  • Develop skills and knowledge by attending training, workshops, and individual and group supervisions.
  • Work autonomously or in a multi-disciplinary team to provide ongoing risk assessment, analysis, judgement and support to children/youth and families.
  • Refer, collaborate, and attend case conferences with other child/youth protection agencies and community partners/professionals as required.

The candidate is also responsible for the planning, directing, coordinating, organizing and controlling of activities required to deliver both mandatory and non-mandatory services and programs to all Thessalon First Nation community members within policies and applicable legislation. The Interim Director of Health and Social Services will also ensure that business practices and internal policies support sound fiscal management and that high standards for service delivery are developed, implemented and monitored.

Objectives :

  1. Ensure health and social service delivery requirements and general health care principles are being met in the daily operation of the Health Centre programs and services in compliance with regulatory bodies.
  2. Monitor, follow-up, and ensure an optimal level and quality of service delivery by developing and implementing a short- and long-range strategic plan for the centre in consultation with regulatory bodies and Chief and Council.
  3. At all times, maintain and support compliance of guidelines contained with approved Personnel Policy to coach and lead employees for continuous improvement.
  4. Coach and supervise all health centre staff to meet service delivery needs by recruiting, hiring, retaining accredited team members.
  5. Advocate for Thessalon First Nations community members in health and social service matters.
  6. Monitor and adhere to strong fiscal responsibility and financial management of service delivery as set out in the annual budget for the Health Department. To work in partnership with Thessalon First Nation Finance department to support generally accepted accounting principles and submit reports for partner deadlines.
  7. Actively participate and lead with positive engagement in the spirit of community minded growth with leadership, management, supervision and communications with all Thessalon First Nation community members and partners.

Duties and Responsibilities :

  1. Facilitate health and social service committee(s) decision making through the provision of administrative support and appropriate and accurate research, statistics and advice.
  2. Provide advisory, consultative and evaluate services as needed to any level of membership within Thessalon First Nation.
  3. Ensure appropriate reporting requirements are fulfilled including reporting obligations to funding sources and Chief and Council from other health and social service personnel are completed by reporting due date deadlines.
  4. Obtain information surrounding unresolved health and social issues for appropriate meetings and committees as requested.
  5. Observe and identify potential health and social concerns within the community and provide strategic direction with the local health and social service committee(s) or appropriate individuals.
  6. Develops, in conjunction with staff, workplans and monitoring systems to ensure the effective management of programs and service.
  7. Develops working arrangements with external health services such as CCACs, AHACs and other Indigenous and non-Indigenous organizations.
  8. Responsible for the day-to-day operations of the Thessalon Health Centre.
  9. Facilitates the support of a collaborative and safe work environment.
  10. Represent Thessalon First Nation on boards and/or committees as directed by Council.
  11. Implements the efficient and effective resolution of service complaints.
  12. Monitors health legislations and their potential impact on the health services of the area and create solutions for service delivery to address projections.
  13. Responsible for the financial monitoring and regulatory reporting submission of all Social Services & Health programs and services with absolute adherence to regulatory requirements. This is a non-negotiable requirement.

Qualifications :

Education:

  1. Post Secondary degree in health and social services, public or health administration.

Experience:

  1. Three years experience with progressive management responsibility in community health or social services.
  2. Demonstrated ability to work effectively in a multidisciplinary team environment.
  3. Proven ability to establish and maintain productive relationships with Chief and Council, as well as other community-based organizations, committees and government agencies.
  4. Excellent communication, motivation, leadership and interpersonal skills.
  5. Ability to prepared detailed reports, follow-up reports and evaluations.
  6. Knowledge and understanding of Anishinaabe culture and traditional health.
  7. Thorough knowledge and proficiency in program development, delivery and quality assurance.
  8. Sound knowledge and application of human resource management and accounting policies and principles.
  9. Able to manage within a fast-paced environment along with multiple priorities.
  10. Valid Drivers License and reliable transportation.

Accountability

The Health and Social Services Director is directly accountable to the Thessalon First Nation Band Manager and to Thessalon First Nation Chief and Council.

Salary Pay Band Range

$80 000.00 $100 000.00

● Develop, implement and evaluate Social Service Programs
● Provide support to department managers in team member intervention when needed.
● Facilitate support groups for families and/or residents.
● Bereavement counseling for residents/families/team members as needed.
● Display an awareness, and evidence on a daily basis, of why feelings matter most to
individuals.
● Coordinate Family Council meetings
● Act as the Home Resident Placement Coordinator in absence of the Admissions
Coordinator
● Participate in tours of the Home with potential applicants and/or their representative.
● Maintain a wait list in conjunction with CCAC.
● Maintain communication with placement agencies, applicants/representatives, to ensure
exchange of information to ensure occupancy remains above LHIN specific occupancy
target.
● Outreach services by maintaining contact and relationship with hospital social workers
and discharge planners, community agencies, physician offices, social workers in
nursing homes and retirement homes in an area or region and any other agency that
would be a source of referral for new residents.
● Required to understand the nature and meaning of quality indicators
● Participates in the improvement of the indicators and achieves satisfactory results.
● Monthly review and quarterly trending and analysis of resident satisfaction
questionnaires with developed and implemented action plans. Semi-annual analysis with
action plan to Executive Director
● Any and all other tasks as assigned

Qualifications:
The qualifications needed to join our family are as follows:
● Diploma in social services, gerontology, or a degree in Social Work.
● Registered under the Social Work and Social Services Work Act and holds a current
general certificate of registration.
● Or must have held a Social Services Coordinator or Social Services Worker position at a
different LTCH without meeting the updated qualifications set out in the Regulation, on
the condition that the person has had (3) years full-time experience in the position during
the (5) years immediately after being hired at the different LTCH or the equivalent parttime experience in the position during the (7) years immediately after being hired at the
different LTCH.
● Previous experience in a geriatric social related field is an asset.
● Well developed interpersonal skills and demonstrated ability to interact efficiently and
courteously with residents, public, employees, and volunteers.
● Excellent written and verbal communication skills.
● Ability to make the most of the time you have to provide positive social interactions.
● Good organizational skills.
● Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one
(1) year of being hired.
● Second language is an asset.
