Job Summary:

We provide primary health care through a range of options including prevention, assessment, diagnosis, and treatment. Our interprofessional teams include health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care and using wholistic health and community development concepts.
The Clinic Nurse/Care Coordinator is part of a client-centered professional team structure and reports to an Interprofessional Team Manager. The clinic nurse works with a variety of registered and non-registered health care providers, both directly and under the facilitation of medical directives, to facilitate coordination of client care and support optimal functioning of the team to provide health care services. Primary Care Nurse Role:
The Primary Care Nurse will use their clinical knowledge and skills to support both Patients and Clinicians through the various stages involved in the primary care process. Expectations:
Patient Care
- Pre-visit Staging of the Patient:
1)Triaging and assisting/supporting Nurse Practitioners and Doctors with visits.
2) Research all reports, lab work, letters and medications and enter the chart to support patient visits.
- Clinical Visit Supports:
1) PAPs, Well Babies and Well Womens checkups as well as vaccines and other assessments as requested by the Providers.
2) Take patient vitals (height, weight, BMI, blood pressure) and conduct any pre-work as appropriate to patients symptoms, attributes or reasons for visit (e.g. glucose test, urinalysis, temperature, vision, lung capacity, etc.).
3) Provide charting supports to team members during visit as needed.
4) Provide patient education and documentation to support patient wellness, self-care, and information about tests/procedures.
- Follow Up Care:
1) Follow up on tasks assigned by clinical team members (labs results, referrals, calls to health care providers etc.)
2) Review charts to identify patient screening requirements as per protocol (e.g. mammogram, PSA).
Mentorship, Education, and Process Improvement
- Develop positive working relationships/partnerships with other disciplines, agencies and health care professionals in order to promote coordinated service, quality care and access to appropriate resources for patients.

Job DescriptionWith a particular focus on enhancing the patient experience, the Registered Nurse will provide advocacy and education to patients and families experiencing a cancer diagnosis, through facilitating access to services, coordinating care, providing support, as well as symptom management throughout treatment and follow up.
The Registered Nurse possesses excellent decision-making and clinical judgement skills to appropriately manage, assess and triage all the referrals and assign appropriate resources. As part of this function, the Registered Nurse is required to collect and report volumes, diagnostic wait times, and symptom management data.
- The Registered Nurse will provide psychosocial, physical, and spiritual support, which may be complex and dynamic, utilizing the nursing process. The Registered Nurse will act as an expert resource to healthcare providers within the community to facilitate access to care, services and resources, and potential entry into the cancer care system.
- Provides nursing assessment and coordination of care utilizing appropriate protocols, tools and documentation processes.
- Provides ongoing assessment of symptoms and alters appointments/consults in collaboration with the clerical team as necessary.
- Provides education and support regarding required diagnostic tests.
- Integrates substantial knowledge, skill, decision making and communication techniques in assessing health needs and in planning, implementing and evaluating nursing care for the patient with a potential cancer diagnosis.
- Assesses the need for additional patient/family support and facilitates access to Institution/community supportive care services, as required.
- Individualizes the inter-professional patient care path to reflect interventions to meet identified needs while promoting safety, privacy and confidentiality, in consultation with the patient/family/significant others and the health care team.
- Participates in the development and delivery of educational material to meet the learning needs of patients, family members, peers and other health professionals.
- Completes health teaching for clients/families regarding each component of the diagnostic pathway.
- Engages in continuous learning opportunities to maintain clinical competence and to advance own knowledge.
- Engages in creative, collaborative problem solving and planning strategies to provide information on and assist in the resolution of identified pathway delays and service gaps.
- Demonstrates a commitment to the philosophy and goals of the organization.
- Baccalaureate in Nursing
- Registered Nurse in good standing with the College of Nurses of Ontario
- Oncology Certification –CON(C)
- Oncology Specialty Certificate an asset
- Must agree to participate in an online Foundations in Cancer (De Sousa Institute) immediately after hire
- Demonstrated counselling skills
- Minimum of 3 years Oncology nursing experience
- Superior interpersonal and communication skills.
- Demonstrated leadership and strong organizational skills.
- Embraces principles of evidence based practice
- Experience in patient education and program development
- Knowledge of current Mississauga Halton Central West Regional Cancer Program
- Current certifications of competence from the College of Nurses of Ontario
- Current membership in other professional organizations (i.e. RNAO) and the Canadian Association of Nurses in Oncology (CANO) an asset.
Additional Information
Hours: Monday to Friday, Days; 8 hour shifts, 0800-1600, 0830-1630 (Hours are subject to change based on operational needs).
Salary: $39.07 to $56.00 per hour

Responsibilities:
Working closely with the assigned Cardiologist, the principal responsibilities of the NP may include but are not limited to the following:
- Conduct comprehensive patient assessments, take medical histories, perform physical examinations, and interpret findings
- Order diagnostic tests and review incoming results
- Maintain accurate and up-to-date patient records using electronic medical records (EMR) systems
- Provide education and preventative care to patient
- Formulate clinical treatment plans
- Promptly follow up on incoming results
- Communicate effectively with patients, families, and interdisciplinary team members
- Administrative duties, including form completion, prescriptions renewals, and other administrative duties as deemed necessary for efficient clinic operation
- Perform other duties as assigned.
Education and Qualifications:
- Current and valid Nurse Practitioner license in Ontario
- Certification from an accredited Nurse Practitioner program
- Minimum three (3) or more years of experience as a Nurse Practitioner in a cardiovascular setting
- Demonstrative expertise in cardiovascular assessment, diagnosis and treatment
- Strong clinical, organizational, interpersonal and communication skills
- Ability to provide patient focused provision of service
- Ability to multi task within a fast paced environment
- Excellent time management skills, ability to work independently, professional and courteous
- Ability to use good judgement with excellent decision-making and problem-solving skills
- Ability to work collaboratively with all One Heart Care team members
- Ability to use the information technology provided
- Proficiency in Microsoft Office, EPIC and electronic medical records (EMR) systems. Working knowledge of EPIC and/or Cerebrum an asset.
