We’re looking for a permanent, full-time Director of Clinical Care to lead our Village Nursing Care team at Kootenay Street Village in Cranbrook, BC .

If you're a qualified Registered Nurse (RN)  that brings previous leadership experience and a passion for providing high quality care and support for seniors, then we’d like to hear from you!  We also welcome Licensed Practical Nurses (LPNs) who bring relevant experience, as you will be considered for this role based on your expertise and background.
 
The successful candidate:

• Oversees nursing team based on clearly defined lines of authority and communication.
• Is accountable for all aspects of resident care and quality improvement activities.
• Has a passion for helping seniors live their best life.
• Supports the team in preparing and implementing comprehensive care plans, tailored to the residents’ short and long term requirements.
• Ensures care is aligned with legislative requirements, governing bodies (CCALA, BCCNP) and GLM mission and philosophy.
• Ensure RAI MDS program implementation, including outcome evaluation and monitoring of performance metrics.
• Oversees tracking as required by IH and GLM.
• Liaises with educational institutions & placement of practicum students.
• Establishes working relationships with all stakeholders.
• Manages family and client concerns as per Positive Solutions process.
• Works with the Community Manager in hiring, performance management and evaluation.
• Co-facilitates departmental meetings.
• Coordinates and monitors operational activities within the department.
• Manages and monitors all nursing envelope expenditures, explains monthly variances, and works with the Community Manager to identity corrective actions.
• Participates in strategic planning and accreditation activities.
• Ensures that all required committees are functioning within their terms of reference and areas of responsibility.
• Works with the Quality & Clinical Practice Consultant in identifying and addressing learning needs, including P&P review.
• Works on call as needed (telephone consultation).
• Holds a Bachelor's of Science degree in Nursing (BSN) or Bachelor's of Science degree in Psychiatric Nursing (BSPN) or related health discipline (Masters degree preferred).
• Current registration with the BCCNM.
• Has a minimum of five (5) years management experience involving the supervision of a multidisciplinary team of staff, budgetary responsibilities and quality management process.
• Brings previous experience with long-term care.
• Can effectively multi-task, manage time effectively and prioritize work.
• Has a strong ability to plan, direct, evaluate and control the delivery of multi-faceted care services within a social model framework.
• Possesses excellent interpersonal and conflict resolution skills; enjoys teamwork. 
• Possesses a Clear Vulnerable Sector Criminal Record Check.
• Can provide a negative TB Screening test, and complete Immunization Records as outlined in the BC Centre for Disease Control for Health Care workers

Qualifications

PRIORTIES

• Provide clinical supervision, consultation and training to therapists, group facilitators, and contracted staff to ensure exceptional clinical support services.
• Contribute to the delivery of support services through team building, excellent customer service, continuous quality improvement and risk management.
• Provide expertise on high-risk cases, including risk assessment and crisis intervention, as requested by staff.
• Monitor client intakes into the program to ensure every client has been provided with a high level of clinical service from the team.
• Determine the training and professional development needs of the team and develop/update program curriculum and tools for programs with team consultation.
• Support the implementation of the program’s strategic plans, direct and monitor clinical activities so that strategic goals and approved budgets are met.
• Ensure the clinical teams understanding of all clinical and administrative procedures and determine where the policies, procedures and protocols require updates or adjustments to ensure best practices are being adhered to.
• Respect the agency to maintain and strengthen collaborative relationships with community partners on various projects and initiatives as assigned.
• Facilitate and support scheduling, audits, performance management and quality monitoring of services.
• Maintain a high level of clinical competence, participate in professional/program development, clinical training opportunities and staff training.
• Stay informed about CRPO, OCSWSSW and other regulations and industry best practices through ongoing professional development.

Requirements

TALENTS & EXPERTISE

• A Bachelor's degree in Social Work or a relevant field. A Master's degree is considered an asset.
• Minimum 2 years of experience in a supervisory or management role, with a strong ability to lead and motivate a team.
• Minimum of 3 years of hands-on clinical treatment experience in a relevant healthcare setting.
• Current Standard First Aid, CPR and CPI certificate.
• Registration with the OCSWSSW or the CRPO.
• Supervisory experience in a Not-for-profit, unionized environment is a strong asset.
• Proven ability to lead by example, engage and mentor staff, and guide teams toward successful project completion and goal achievement with humility and efficiency.
• Effective communication and interpersonal skills.

**Job Summary**
Responsible for identifying and interacting with medically and psychosocially complex patients and families who are likely to benefit from care management and meet high risk criteria and for coordination of discharge planning services for these patients in collaboration with RN Care Management and other members of the care team.
**Details of the position**
1. Screen and identify patients who need care management per high risk criteria.
2. Assess, develop, implement and monitor a comprehensive discharge plan of care through an interdisciplinary team process in conjunction with the patient and family. Collaborate with the multi-disciplinary team to identify problems or needs that require special planning, intervention, teaching or follow-up.
3. Identify key problems, strengths, and resources to be addressed in the discharge plan of care. Coordinate and facilitate improved ability to comply with plan of treatment; counseling or support needed to cope with situation; improved ability to access appropriate level of care due to lack of financial resources or lack of available service.
4. Actively support measures that promote effective use of resources.
5. Identify, plan and arrange for appropriate services applying a knowledge of services available in the community, state, and federal health regulations and admission, discharge and appropriate level of care. Coordinate effective planning and arranging for needed services upon discharge.
6. Intervene by arranging services, education and providing psychosocial support to prepare the patient and their family to manage their healthcare needs within the acute care setting and post discharge.
7. Performs other duties as assigned.
