We provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Our team includes health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care, and uses wholistic health and community development concepts.
The Community Health Nurse works within an interprofessional team comprised of registered and non-registered healthcare professionals, both directly and under the facilitation of medical directives, to facilitate the coordination of client care and supports optional functioning of the team to provide wholistic healthcare services at the community level. The CHN implements mandatory and discretionary community health programs in specified communities within the professional scope of practice established by the College of Nurses of Ontario.
NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.
QUALIFICATIONS:
current registration as a Registered Nurse with the College of Nurses of Ontario.
willingness to work to a full scope including performing delegated acts through medical directives;
experience in community health nursing a definite asset;
training and experience in basic and/or advanced wound care an asset;

Position Description:
The Community Health Nurse is a member of the CTS team and works closely with team members to provide the highest possible quality of health care services to clients.
The role of the Community Health Nurse CTS is to work with clients to help them achieve their desired health goals by establishing trusting relationships, conducting nursing assessments, engaging in health teaching and harm reduction counselling related to drug use, monitoring for signs of drug overdose or anaphylaxis, and intervening in emergency situations based on the Centre’s policies and procedures and according to the College of Nurses of Ontario standards of nursing practice. As a member of the larger interprofessional care team, the Community Health Nurse may also provide primary care to people who experience barriers to accessing care due to their use of street drugs and concurrent disorders.
This may include providing immunizations, wound care, offering screening, counseling and testing for sexually transmitted infections and blood-borne infections such as HIV and Hepatitis C and connecting clients to substance use treatment services (including opioid agonist treatment) and other health and social services when appropriate (i.e. housing, income support, food support, mental health counseling). The Community Health Nurse will be the lead in overdose interventions including assessment, naloxone and oxygen administration.
Reporting Relationship:
The Community Health Nurse, CTS is directly accountable to the Director of CTS for the carrying out of their duties and through the Director of CTS to the Executive Director.
As a Community Health Nurse, the incumbent is responsible to meeting the licensing and regulatory requirements of the College of Nurses as the professional governing body.

Professional Experience
- Three to five years nursing experience in a community setting or combination of community and hospital or public health settings: experience in a primary care setting an asset
- Experience working with a broad range of populations, including: people with HIV/AIDS, hepatitis C, street involved people, transgender people, men who have sex with men, sex trade workers, people with mental illness, people with substance use disorder, and other marginalized groups
- Experience working within a harm reduction approach

Hours of Work

Benefits

Operating within their full scope of practice, the NP conducts medical assessments, provides diagnoses, initiates treatment, prescribes medications when appropriate, and contributes to the collaborative treatment planning. Role-Specific Accountabilities
- Conduct comprehensive medical assessments for individuals participating in long-term addiction recovery, identifying and addressing physical and mental health needs within a trauma-informed care framework.
- Provide medical consultations, establish diagnoses, and prescribe medications when clinically appropriate, in accordance with CARNA Nurse Practitioner standards and full scope of practice.
- Collaborate with the Medical Director and interdisciplinary team to support individualized treatment planning that promotes stabilization, recovery, and improved health outcomes.
- Contribute to withdrawal management planning and monitoring by identifying symptoms of substance withdrawal or post-acute withdrawal syndrome and recommending appropriate interventions.
- Ensure timely and accurate clinical documentation, including assessments, treatment plans, prescriptions, and communication relevant to patient care.
- Participate in care conferences or case discussions as requested, providing medical insight to support the clients recovery journey.
- Communicate relevant medical information with appropriate team members to support coordinated, person-centered care.
- Practice in accordance with all regulatory requirements, ethical standards, and internal organizational policies.
Qualifications
- A masters degree in nursing with a focus on Advanced Clinical Practice including recognized certifications from advanced training for NPs in Substance Use Disorder treatment(s).
- Must have current non restricted registration with the College and Association of Registered Nurses of Alberta (CARNA) in the NP adult or family stream.
- Hold active registration with TPP Alberta for prescribing controlled substances.
- A minimum of 5 years of related experience as a Registered Nurse in clinical practice.
- Current experience as an NP in an acute care setting, preferably Critical Care and Addictions and Mental Health Sectors.

PROJECT SUMMARY
The Lillooet Tribal Council is recruiting a Full-Time Home and Community Nurse for the Sttimc Outreach Health Services Office (SOHS). Responsibilities include developing wellness plans, enhancing health literacy, and addressing any barriers to ensure ongoing culturally sensitive care.

Assist clients with referrals to external resources and coordination of services as needed.
Support discharge planning with appropriate service providers.
Provide education to clients, families, etc. to ensure informed wrap around care.
Deliver workshops and presentations on health and prevention.
Prepare reports on program status and community health trends.
QUALIFICATIONS
A minimum of 5 years relevant experience.
Bachelor of Science in Nursing (BSN), Registered Nurse, (Licensed Practical Nurses/ LPNs will be considered based on experience)
Current registration and in good standing with the British Columbia College of Nurses and Midwives (BCCNM)-
Knowledge of available community resources and how to access them.
Cultural competency and safety when working with the Sttimc.
Successful completion of a Criminal Record Check.
Valid drivers license.
SALARY: The wage range for the Home and Community Care Nurse position is based on the BCNU Wage Grid and is structured to align with our specific budget constraints. This range aims to provide competitive compensation while remaining adaptable to variations in experience and qualification.
Registered Nurse (RN): 41.42/hour to 43.00/hour
Licenced Practical Nurse (LPN) qualified applicants: 32.84/hour
SCHEDULE:
Standard 7-hour shift, Monday to Friday, with flexible scheduling options available (Evenings and weekends as needed)

Job Summary:
S R.T. MedStaff is currently seeking Registered Nurses (RNs) and Registered Practical Nurses (RPNs) for our Community Visiting Nursing Program. The Visiting Registered Nurse or Registered Practical Nurse will provide safe and holistic nursing care to patients in a variety of service delivery locations primarily in Mississauga. We are currently hiring for various shifts including days and evenings.
Responsibilities:
- In collaboration with the patient/family/caregiver and other health care professionals, help formulate a plan of care based on patient assessment
- Provide nursing care to patients according to the written plan of care
- Evaluate effectiveness of treatment plan(s) and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals
- Act as a patient advocate by protecting and promoting patient rights
- Promote patient safety and assist them in living comfortably in their home environment
- Share nursing knowledge and expertise with other health care providers
Qualifications:
- Registered Nurse or Registered Practical Nurse in good standing with College of Nurses of Ontario
- One (1) year recent experience in the community
- Excellent problem solving, time management, and crisis resolution skills
- Excellent interpersonal and communication skills, both verbal and written
- Experience with Information Technology
- VSS within the past year of hire confirming absence of a criminal history/record
- Valid driver’s license and reliable vehicle is a requirement
- Second language is considered an asset
