The Biidaaban Healing Lodge is a 12-bed residential treatment program located on Biigtigong Nishnaabeg formerly known as Pic River First Nation, which is approximately 25 kilometers from Marathon, ON. We are mandated to provide quality treatment to all Aboriginal and Metis people residing in the Robinson Superior area. We provide treatment for unresolved trauma stemming from sexual abuse, family violence, adult children of trauma, grief and abandonment and anger solutions.

Primary Responsibilities:

Working as a team member within the Healing Lodge and in conjunction with the First Nation communities in the area and under the direction of the Executive Director, the community outreach workers are responsible for:

  • To design and deliver educational programs that exemplify prevention, promotion and aftercare services and deliver them at community level
  • To develop an effective evaluation tool that assesses the needs of our First Nation communities and to define strategies that will meet the needs identified through community needs assessments
  • To deliver mobile treatment and training units to meet the needs of individuals, families and communities within the Robinson Superior area in the areas of Mental Health First Aid First Nations, lateral violence, grief and loss and trauma recovery
  • To identify promotional strategies aimed at publicizing the Biidaaban Healing Lodge programs at community level
  • To deliver programs in conjunction with Biidaaban Healing Lodge staff as needed and required

Qualifications:

  • A college degree in Social Work; Psychology or a related field of study with three to five years counselling experience, preferably in a treatment setting with the ability to do individual and group therapy

(A combination of education and work experience will be considered.) We are willing to provide training to the right candidate

  • Experience working with First Nations individuals, families and communities, and a working knowledge of healing and recovery from a Native perspective.
  • Ability to work independently and as part of a team with strong communication skills, a working knowledge of computer programs as a word, excel, publisher, power point and outlook to develop pamphlets, posters and other promotional items
  • An understanding of data entry systems would be an asset
  • An understanding of the issues that impact First Nation communities and the Anishinaabek way of life

About the Intensive Health Outreach Team (IHOT):

Intensive Health Outreach Team (IHOT) provides low-barrier interim health care, psychosocial support, and help accessing longer-term health services to people in the Vancouver inner city who are unhoused or newly housed. IHOT is a short-term service that supports people aged 17 and up who live outdoors, in shelters, or are newly housed in supportive housing, including in the Supported Rent Subsidy Program.

As a Community Liaison Worker with Vancouver Coastal Health, you will:

  • Work as a member of the interdisciplinary team to provide a variety of advocacy/liaison services to clients within a designated program living with health issues including those who are taking HIV medications.
  • Screen clients for eligibility for services, link clients to appropriate internal and external resources and advocate for clients with various community and/or government agencies.

Qualifications:

Education & Experience

  • Completion of Grade 12, plus completion of Welfare Aid or Social Services Certificate from a recognized college.
  • One (1) to two (2) years' experience working with marginalized client populations with complex health and/or psychosocial problems, or an equivalent combination of training and experience.
  • Valid BC Driver's license and access to a personal vehicle for business-related purposes, as required.

Knowledge, Skills & Abilities

  • Knowledge of community resources and services.
  • Knowledge of related issues facing clients starting or receiving HIV anti-viral therapy.
  • Working knowledge of the social, emotional and health-related problems of the client population.

Detailed Overview

Provides social work services in accordance with the BC College of Social Workers standards of practice, Fraser Health mission, values and in collaboration and partnership with private and public community based organizations providing services to older adults who are frail, isolated/marginalized, at risk and may be hard to reach. Provides comprehensive services including psychosocial assessments and therapeutic support to clients and their families; serves as liaison with community agencies and participates in educational services, planning, program development and consultative support to the interprofessional team in the development and enhancement of services for this community population.
Responsibilities

Provides comprehensive therapeutic services to adults who are frail, isolated/marginalized, at risk and may be hard to reach; explores psychosocial response to identify problems, personal concerns, treatment objectives and facilitates change in copying style, behaviour, attitude and feelings; participates with the client, families, community supports and the interprofessional team to coordinate services, manage cases and facilitates meetings.
Conducts comprehensive social work and psychosocial assessments by interviewing the client and families, obtaining relevant information, gathering social data and interpreting the data using a variety of tools, in accordance with professional standards, clinical practices.
Collaborates with the interprofessional team and private and public community-based service providers in client care planning; follows up and provides ongoing monitoring of complex psychosocial situations; evaluates client response to interventions.
Partners with community services, health and social agencies including provincial Ministries, physicians and other primary care providers in the assessment and management of individuals at risk, making referrals to appropriate resources when longer term treatment or follow-up is required.
Arranges and participates in joint home/outreach visits to clients and/or families with other health care professionals, as required, provides advice to clients and/or caregivers about available community resources.
Acts as a clinical resource for clients and their families by providing information regarding community resources and access to these resources, facilitating referrals where appropriate; facilitates the development of resources by identifying services needed.
Determines if client is capable of making decisions around client care plans, in collaboration with the interprofessional team; promotes client autonomy by ensuring their participation in the planning and decision making and the development of problem solving abilities where possible and respecting the client's right to privacy and confidentiality.
Participates in quality improvement activities by providing input into the development, implementation and evaluation of program goals, objectives, policies and procedures.
Develops, coordinates and delivers relevant educational programs, psycho-educational and support groups and/or training for clients, families and community agencies on psychosocial issues related to health, illness and disability, particularly within this population.
Maintains client records by documenting assessments, charting information and preparing progress notes and reports in accordance with established standards, policies, procedures, and current regulatory requirements.
Maintains a variety of records such as workload statistics by recording information into approved formats, collating data, analyzing trends, accounting for variances, preparing reports as required and providing recommendations to the Manager, Clinical Operations, as required.
Participates on local committees and planning bodies to identify opportunities for change, assess available resources, analyze outcomes and identify areas to enhance client care services and reduce service gaps. Engages in related research activities and program planning.
Performs other related duties, as assigned.

Qualifications

Education and Experience

Current full registration with the BC College of Social Workers.
Two (2) years of recent related experience in a complex health care environment working with vulnerable seniors, or an equivalent combination of social work education, training and experience.
Valid BC Driver's License and access to personal vehicle for business-related purposes.

Skills and Abilities

Comprehensive knowledge and understanding of clinical practice models related to vulnerable frail seniors, families and groups.
Ability to conduct comprehensive social work and psychosocial assessments.
Comprehensive therapeutic skills which could include crisis counseling and brief therapeutic interventions applicable to clients, families and their support networks.
Comprehensive knowledge of pertinent/relevant health care and privacy legislation.
Ability to educate and provide consultation to clients, families and community agency health professionals.
Ability to work independently and with others including working effectively in a multicultural environment, as part of an interprofessional team.
Ability to community effectively both verbally and in writing.

**Job Summary**

Details of the position
+ The Clinical Social Worker completes appropriate intake documentation, provides ongoing assessment, develops treatment plan in collaboration with the multi-disciplinary treatment team. The Clinical Social Worker also manages multiple complicated cases, maintains current knowledge of ever-changing community resources, monitors patient needs and progress, develops aftercare plans. Maintains documentation on patient throughout hospitalization.
+ Provides brief therapeutic interventions with patients and/or families, as well as community staffings, in coordination with the physician and treatment team. Works as in integral member of the treatment team in the provision of treatment and aftercare planning.
+ Ensures compliance with privacy and confidentiality laws. Interfaces regularly with legal entities to address treatment needs.
+ Assists with monitoring and evaluation of services provided for department quality improvement plan.
+ Maintains current understanding of issues related to culture, ethnicity, gender identity, age, sexual orientation, religion and utilizes appropriate communication skills.
+ Provides information, education and direction in response to community requests.
+ Regularly accesses pharmaceutical and community resources to provide patients with needed psychiatric medications.
+ Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA's. Must maintain current licensure through continuing education. Must uphold all ethics, rules and regulations required by the Clinical Board.
+ Diagnoses, assesses and addresses both mental health and chemical dependency conditions. Performs risk assessments for suicidality, homicidality and grave disability.
+ Provides instruction, training and supervision for graduate level social work students. Also provides clinical supervision toward licensure for CSWA's. Must maintain current licensure through continuing education. Must uphold all ethics, rules and regulations required by the Clinical Board.
+ Screen and identify patients who need care management per high-risk criteria.
+ Assess, develop, implement and monitor a comprehensive discharge plan of care through an interdisciplinary team process in conjunction with the patient and family. Collaborate with the multi-disciplinary team to identify problems or needs that require special planning, intervention, teaching or follow-up.
+ Performs other duties as assigned.
