Overview:
This member of the Canadian Mental Health Association works alongside a team of professionals whose focus is safety, stability and support for individuals in a mental health crisis who are in immediate contact with police but not suitable for incarcerations or admission to the hospital. The team would consist of a multidisciplinary team either within the Canadian Mental Health Association, Kenora Branch or another community agency. This member is responsible for assessing clients physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the a practitioner, and providing a range of treatment, rehabilitation, and support services both for a variety of programs offered by the Canadian Mental Health Association, Kenora Branch. The Registered
Nurse/Registered Practical Nurse will coordinate shift-management with other registered nurse/registered practical nurses within the Canadian Mental Health Association, Kenora Branch. This position reports directly to the Safe Bed Program Team Lead and Chief Executive Officer.


Qualifications:
Registration or eligible for registration with the College of Nurses of Ontario
Knowledge of mental health/illness and mental health difficulties and treatment modalities Assessment/ Evaluation skills
Working knowledge of community resources and agencies
Ability to work independently and in a team environment
Ability to manage conflict and crisis situations
Computer literacy and recording skills
Cultural Competence for diverse populations
Valid Drivers Licence
Criminal Records Check
Certification in CPR and First Aid or willingness to obtain same


Duties:
Adherence to all policies of the CMHA Kenora Branch
Assurance of confidentiality of all client, personnel and organizational records and information
Maintenance of client files and data files including updating of client information consistent with policies and
procedures of the CMHA Kenora Branch
Maintenance of professional qualifications and registration
Participation in ongoing training and education and application of new skills to practice
Coordinate shift management as per established policies and procedures
On call from Monday to Friday from 4:30 p.m-9:00 p.m. and on weekends and statutory holidays. When on
vacation coverage, sick etc. coverage is coordinated through internal programs
Provide crisis intervention, Advocacy
Take the lead role or participate in providing medication administration and medical services
Develop, revise, and maintain client psychopharmacological and medical treatment including transcribing,
administering, evaluating and recording psychotropic medications prescribed by the treating practitioner
Under the direction of the Safe Bed Program Team Lead & Chief Executive Officer and treating practitioner
manage pharmaceuticals and medical supplies
Provide case management for an assigned group of clients and their families
Function as a fully integrated member of the team for the client residing within the residences or other
identified agency programs
Assessment of client needs and mental status and the development of appropriate recovery plans
Facilitate and monitor the implementation of client recovery plans
Participate in staff meetings and recovery plan meetings
Participate on specific Committees or task or work groups as assigned by the Safe Bed Program Team
Lead and Chief Executive Officer
Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and
Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures; including but not limited to Fire,
Disaster and Evacuation Plans
Ensure client safety and client safety activities are emphasized throughout orientation of new staff
members
Support an environment which avoids, prevents and corrects all activities or actions which may result in an
adverse outcome in the delivery of mental health services
Participation in administrative and office functions
Responsible for self care

The position involves working within a nursing framework collaboratively with Nurse Practitioners, community mental health and/or addiction teams, primary care physicians, psychiatrists, family health teams and other community health service providers to support the health needs of individuals living with mental illness and/or addiction, including those accessing Methadone, Suboxone and Sublocade at local clinics.

The location for the provision of these services will vary throughout Perth County and surrounding areas and services are delivered in-person, by phone, and virtually based on client need.

KEY POSITION RESPONSIBILITIES AND DUTIES:

Registered Nurse

  • Responsible for the delivery of high-quality nursing care.
  • Collaborate with the individual and those in his/her circle of care to develop and implement a plan and provide follow through/evaluation to promote quality of life.
  • Engage in community awareness, education and outreach as designated and appropriate, representing the agency in a professional manner.
  • Participate in and support the use of OTN and EMR technology within the agency and community.
  • Demonstrate knowledge about primary health needs in persons living with a mental illness and/or addiction.
  • Excellent health assessment skills, sound clinical judgment and decision-making skills.
  • Ability to provide education to staff, clients and community members about health and wellness.
  • Demonstrated ability to take initiative and develop new programs to meet identified gaps in service.
  • Assist with program development, implementation and evaluation.
  • Support the ongoing development and evaluation of a Community Withdrawal Services Program for Huron Perth.

Administrative

  • Maintain a complete, accurate and comprehensive clinical record of each client contact.
  • Report statistics about clinical activities using designated agency format as required.
  • Complete monthly, annual and other reported documentation as required in a punctual and comprehensive manner.
  • Maintain administrative and clinical records, including client files and data entry, as per Policy and Procedure Manual.

General

  • Participate on internal agency committees and/or outside community committees as appropriate.
  • Adhere to policies and protocols as outlined in the Human Resource and Procedure Manual.
  • Attend and actively participate in any team and staff meetings, the Annual General Meeting, and periodically scheduled strategic planning, team building and agency events
  • Assist with the orientation and training of new agency staff, volunteers and interns/students as designated and need.
  • Provide coverage of CMHAHP AMHS services for other team members as needed.
  • Help ensure the organization and program objectives are met.
  • Work harmoniously with colleagues, including not behaving in ways that are likely to be offensive in any manner to others and confronting such behaviours by others.

QUALIFICATIONS:

EDUCATION: Registered Nurse (BScN Degree) in good standing with the College of Nurses of Ontario and liability protection under Registered Nurses Association of Ontario or independently is required.

EXPERIENCE: 3-5 years of nursing experience and an understanding of issues individuals face with substance abuse and mental health concerns is required. Knowledge about primary health care needs in persons living with a mental illness or addiction is required. Knowledge of working with individuals experiencing withdrawal from substances is an asset.



KNOWLEDGE AND SKILLS: Broad knowledge of addiction and mental health treatments and explanatory theories, homelessness, and community resources. Demonstrated knowledge of the principles and techniques of case management of mental illness and addiction and their application. In addition, the incumbent will demonstrate integrity and ethics, good coping skills, flexibility and commitment to the work.

Abilities: Communication-written and oral, planning, leadership, interpersonal, negotiating, directing/motivating, organizing, and decision making.

Regent Park Medical Clinic is seeking a Temporary Full-Time Registered Nurse. Reporting to the Lead Physician, the Registered Nurse will be responsible for guiding patients through the healthcare system. The Registered Nurse will coordinate care, provide expert support and education to ensure timely access to services and continuity of care.

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support, that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31, 2027, with the possibility of an extension.

Responsibilities:

  • Support clients with chronic mental health conditions in navigating care systems.
  • Provides nursing services during clinic visits including but not limited to ambulatory/treatments, foot care, wound care, palliative care, vascular clinics and post operative care.
  • Educate clients and caregivers on disease management, medication and safety.
  • Coordinate care across homecare, community health and mental health services.
  • Participate in community outreach, health promotion and wellness initiatives.
  • Support clients in maintaining independence at home.
  • Advocate for patients and families to ensure access to appropriate services.
  • Ensure that the continuum of care including referrals are carried out and followed up with in a timely manner.
  • Manage patients /family needs to ensure that they are seeking services with the right provider at the right time for the right service.
  • Develops, implements, and evaluates care plans based on assessments whilst ensuring plans are followed and updated.
  • Coordinate comprehensive care between clients, families, primary health care team members, and specialists to meet the identified need(s). This may include screenings, education, continuing care, and appropriate referrals.
  • Help educate clients and families on how to effectively access health care services.
  • Develops effective partnerships with patients and their families fostering the patients independence, growth and overall wellbeing.
  • Provide and coordinate proactive holistic care from birth to death with the client/family. Holistic care may include physical, spiritual, social, emotional, and environmental aspects.
  • Optimize relationships with stakeholders including clients, families, and primary health care team members. Able to actively listen and effectively facilitate communication to foster client/family ownership of their care.

Qualifications:

  • Current registration and good standing with the College of Registered Nurses of Saskatchewan (CRNS) or with Registered Psychiatric Nurses Association of Saskatchewan (RPNAS)
  • Completion of an accredited Bachelor of Science in Nursing (BSN) or equivalent combination of education and experience.
  • Demonstrated knowledge of primary care and chronic disease management (e.g., INR management, STI treatment, PAP smears) as per CRNS guidelines and employer-approved protocols.
  • Certified or eligible for RN Specialty Practices (e.g., INR Management, STI treatment, PAP Smears) as per CRNS guidelines and employer-approved protocols.
  • Proficient in EMR systems (e.g., Accuro or equivalent), including documentation, lab review, and medication reconciliation.
  • Strong clinical judgement, interpersonal communication, and ability to work collaboratively in an interdisciplinary team.
  • Proven ability to prioritize, manage caseloads, and maintain confidentiality and professionalism.
  • CPR certification (Basic Life Support BLS).
  • Experience in community health, case management, or a Primary Health Care setting or as a Nurse Navigator is considered an asset.
  • Commitment to ongoing professional development and adherence to evidence-based practices.
  • Local to the area is considered as asset.
  • Demonstrated cultural competency skills.
