The purposes of this position are:
 

  • To assist in the provision of health education programs and services and to assist in ensuring the availability of, and access to medical/health programs and services by the people of the Curve Lake First Nation within the administrative policies and procedures established by the General Manager, and directed by the Manager of Health & Family Services
  • To assist in the development and maintenance of community based medical/health documentation.

Duties and Responsibilities:

Duties:
The duties and responsibilities of this position are categorized into three main functions:
 
  1. Provision of Direct Health Care 
  • Assessing home situations with the assistance of the CHN
  • Assisting with health related problems through counseling and/or referral to appropriate agencies
  • Teaching community members about dental care, communicable diseases, mental health, child development, drug abuse, etc.
  • Assisting families to cope with chronically ill, sick and aged family members
  • Working with other health professionals in obtaining health histories by serving as in interpreter
  • Providing emergency first aid when necessary
  • Assisting with school health programs such as yearly health assessments, class inspections, vision and hearing screening, measuring heights and weights, planning and implementing health education programs for the students
  • Coordinating Non-Insured Health Benefits and ensuring registered status Indians receive all benefits they are entitled to
  • Responsible for the coordination of medical transportation
  • Assisting in the development or maintenance of medical/health documentation and files
 
  1. Liaison Activities
  • Advising other health care personnel about local Native culture and traditions and their implications on specific health problems
  • Assisting in planning, implementing and evaluating programs for the community
  • Cross cultural sensitivity workshops for non-native service providers   
  • Participating in Health clinics as necessary
  • Making hospital visits to community members when there is little or no family support
  • Interpreting health programs and services to the community
  • Attending meetings as requested
  • Acting as a member of various Committees   
  • Assisting in the development of grant applications and the annual operational budget
 
  1. Community Development Activities
  • Working with existing community groups, and assists in development of others, to improve the health of the community
  • Conducting workshops, short courses and health education programs about topics of interest to the community

  • Performs such other related duties as may reasonably be required

TERMS OF EMPLOYMENT:

This is a Permanent Full Time position beginning immediately.  Annual salary range for this position will be $52,058 to $56,685 based on a 39.5 hour work week.

We provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Our team includes health professionals, educators and support workers representing both traditional Indigenous and contemporary models of care, and uses wholistic health and community development concepts.

The Community Health Nurse works within an interprofessional team comprised of registered and non-registered healthcare professionals, both directly and under the facilitation of medical directives, to facilitate the coordination of client care and supports optional functioning of the team to provide wholistic healthcare services at the community level. The CHN implements mandatory and discretionary community health programs in specified communities within the professional scope of practice established by the College of Nurses of Ontario.

The Community Health Representative (CHR) works as a Community Health Team Member in the day-to-day operation of the Community Health Program and provides community health information, instruction and guidance to Thessalon First Nation to educate the community members in safe health practices and to develop healthy lifestyles.


Duties and Responsibilities :

  1. Meets with community leaders, conducting and participating in community meetings and visiting homes to describe, demonstrate and encourage the adoption of safe health and hygiene practices.
  2. Assisting the nurse in maternal, well-baby and child care preventive health clinics and home visits.
  3. Preparing and/or distributing printed material, showing films, slides and presentations, and giving prepared talks on health and hygiene to school classes and other selected groups.
  4. Responsible for initiating programs which promote healthy eating to all ages such as community kitchens, baby food making, organic gardening, canning wild meat, etc.
  5. Carrying out a preventative dental program consisting of instruction in fluoride rinses, dental flossing, tooth brushing.
  6. To conduct a number of community health workshops/information sessions within the Thessalon First Nation community which supports a healthy lifestyle for all ages.
  7. Responsible for developing programs which focus on health promotion and injury/illness prevention and include such programs as maternal and infant health, child health, school health, adolescent health, adult health and elder health.
  8. Responsible for initiating programs which promote physical activity for all ages such as an Elders walking program, childrens recreational programs, etc.
  9. To act as interpreter, when necessary, between members of the community and the health care delivery team and assist visiting professionals in the conduct of their business within Thessalon
  10. Responsible for networking with the Environmental Health Officer in regards to community safety such as dog bites, house mold inspections, West Nile, etc.
  11. Responsible, in conjunction with the Community Health Nurse, to ensure the monitoring and reduce the spread of communicable diseases by applying universal precautions at public places.
  12. Responsible for initiating community programs which supports social cohesion for community members such as Christmas, Easter, Halloween, Aboriginal Day and Family Day celebrations.


Lay Diabetic Educator Duties and Responsibilities:

  1. Conduct screening clinics for pre-diabetes and those at risk for developing diabetes at community events.
  2. Hosting diabetic information sessions for diabetics and those at risk of developing diabetes.

    2. Eye care
    3. Co-hosting with the Northshore Tribal council Chronic Care Nurse bi-monthly diabetic sessions

Qualifications :

Education:

  1. Post-Secondary diploma or degree or its equivalent within the Health field.
  • Native Community Care Counselling and Development (NCCCD) is a must, or willingness and ability to take training.

Position Description:

The Community Health Nurse is a member of the CTS team and works closely with team members to provide the highest possible quality of health care services to clients.

The role of the Community Health Nurse CTS is to work with clients to help them achieve their desired health goals by establishing trusting relationships, conducting nursing assessments, engaging in health teaching and harm reduction counselling related to drug use, monitoring for signs of drug overdose or anaphylaxis, and intervening in emergency situations based on the Centre’s policies and procedures and according to the College of Nurses of Ontario standards of nursing practice. As a member of the larger interprofessional care team, the Community Health Nurse may also provide primary care to people who experience barriers to accessing care due to their use of street drugs and concurrent disorders.

This may include providing immunizations, wound care, offering screening, counseling and testing for sexually transmitted infections and blood-borne infections such as HIV and Hepatitis C and connecting clients to substance use treatment services (including opioid agonist treatment) and other health and social services when appropriate (i.e. housing, income support, food support, mental health counseling). The Community Health Nurse will be the lead in overdose interventions including assessment, naloxone and oxygen administration.

Reporting Relationship:

The Community Health Nurse, CTS is directly accountable to the Director of CTS for the carrying out of their duties and through the Director of CTS to the Executive Director.

As a Community Health Nurse, the incumbent is responsible to meeting the licensing and regulatory requirements of the College of Nurses as the professional governing body.

Community Health Care

South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that offers a wide range of health and social services to residents within the South-East Ottawa catchment area. Our multidisciplinary team provides primary care, health promotion, social services, community development, home support, and advocacy to address the social determinants of health.


JOB SUMMARY


We are seeking a bilingual (French/English) Community Health Worker to join our Primary Care Outreach to Seniors (PCO) Team on a full-time, term contract. The role focuses on supporting seniors who face barriers to care by providing health education, advocacy, practical assistance, and access to services. Working as part of an inter-disciplinary team, the incumbent will also help clients and caregivers strengthen their support networks within the South-East Ottawa community.


Position responsibilities


Work collaboratively with members of the PCO Team and with colleagues from various agencies to most appropriately meet client needs

Implement the Regional Integrated Care Approach with clients and completes and updates coordinated care plans for all PCO clients

Collaborate with the PCO Registered Nurse to support clients

Maintain an active caseload and performs case management.

Build relationships with stakeholders in order to support clients in accessing services, and act as a client advocate by placing telephone calls to landlords, physicians, hospital staff, welfare workers, shelter drop-in centres, etc.

Provide individual support with social determinants of health

Refer clients to physicians and other organizations. Assists clients in obtaining resources for healthier living (i.e., personal care items, housing, income, etc.)

Maintain accurate and complete records of client needs and ensures record keeping is up to date

Arrange and facilitate follow-up services as per the orders of the family physician and other health care providers

Participate on internal and external committees dealing with supports to seniors, as approved

Participate in program evaluation and pilot projects as implemented

Assist in the revision of program content in accordance with results of program evaluations

Maintain client confidentiality and acts in an ethical and professional manner, in accordance with professional standards and in accordance with the policies of the Centre


Requirements


Bachelors degree in a relevant discipline or equivalent combination of training and experience.

Minimum two years experience in a community-based setting.

Minimum two years experience working with seniors.

Knowledge of related community resources.

Ability to assess situations and to respond appropriately.

Interest in a variety of health issues and willingness to learn about them.

Ability to work flexible hours based on client need.

Ability to work collaboratively in a multi-disciplinary setting, with clients and other agencies who are involved with the care of the clients.

Sound judgement and strong problem-solving skills.

Valid drivers license, insurance, and use of a personally owned vehicle.

Proficient use of computers and various software applications.

Fluency in spoken and written English and French; other languages an asset

Salary range: 53,435 to 62,881/annum

The purposes of this position are:
 

  • The Manager of Health Promotion is responsible for leading and managing staff who coordinate health promotion programs and initiatives within the First Nation community. They will collaborate to develop, implement, and evaluate strategies that promote health, wellness, and community well-being.

Duties:
The duties and responsibilities of this position are categorized into three main functions:
 
  1. Administration:
  • Responsible to the Director of Health and Family Services for the effective and efficient administration of health promotion programs.
  • Develops with staff, applications for grant and funding programs.
  • Monitors and reports to Director of Health on approved budgets for Health Promotion
  • Approves accounts payable, prepares purchase orders, prepares payroll documents for Health Promotion staff and develops and maintains documentation for respective funding agencies and submits same to the Finance Manager and Administrative Services for processing.
  • Maintains, with staff, inventories of supplies, equipment and materials at an adequate level; replaces when required within the First Nation’s purchasing policy.
  • Manages and directs the employees of the Health Promotion programs consistent with the policies and administrative processes of the Curve Lake First Nation.
  • Evaluates employee performance; recommends the employment, promotion, disciplining and termination of related staff.
  • Monitors government policies and legislation and recommends policy positions on issues of import to the Curve Lake First Nation.
  • Researches and prepares statistical, financial, policy, and other reports as required by the Director of Health and Family Services
  • Ensures adherence to Occupational Health and Safety procedures.
 
  1. Program Development and Implementation:
  • Oversee development and implement comprehensive health promotion programs and initiatives that align with the First Nation's goals and priorities.
  • Conduct needs assessments, research, and data analysis to identify community health needs and develop evidence-based strategies.
  • Lead staff in their collaboration with community members, leaders, and organization to design culturally appropriate health promotion interventions.
  • Ensure the integration of traditional knowledge and practices into health promotion activities.
 
  1. Community Engagement and Collaboration:
  • Establish and maintain collaborative relationships with community members, elders, health professionals, and external organizations with staff.
  • Facilitate community consultations and engagement processes to gather input and ensure programs meet community needs.
  • Collaborate with internal and external partners to leverage resources, share best practices, and enhance program effectiveness.
  • Represent the First Nation at relevant meetings, conferences, and events related to health promotion.
 
  1. Program Management and Evaluation:
  • Develop program budgets, timelines, and work plans to ensure effective implementation and resource allocation.
  • Monitor program activities. Ensure staff evaluate outcomes and prepare reports to assess program effectiveness and impact.
  • Apply evaluation findings to refine programs, identify areas for improvement, and enhance program sustainability.
  • Maintain accurate records, documentation, and data related to program implementation and evaluation.
  • Report evaluative findings to Health Director for communication to community.
 
  1. Supervision and Team Leadership:
  • Provide leadership, guidance, and supervision to a team of health promotion coordinators and support staff.
  • Foster a positive work environment that encourages professional growth, teamwork, and cultural sensitivity.
  • Conduct performance evaluations, identify training needs, and provide professional development opportunities for staff.
  • Promote a culture of inclusivity, respect, and collaboration within the health promotion team.

QUALIFICATIONS AND EDUCATIONAL REQUIREMENTS:

The incumbent to this position will meet the minimum requirements that include:
  • Degree or diploma in health promotion, public health, community health, or a related field.  Management experience 2-3 years.
  • In-depth knowledge of health promotion theories, models, and evidence-based practices.
  • Strong understanding of social determinants of health and their impact on First Nation communities.
  • Knowledge of culturally sensitive and relevant health promotion materials and resources.
  • Knowledge of relevant legislation, policies, and frameworks related to Indigenous health.

Job Summary

Under the direction of the Program Manager the Community Mental Health Worker is responsible for the day to day running of the support and advocacy services, social, recreational, volunteer and food programs as well as the ensuring safety and welfare of participants in program by use of the principles of psychosocial rehabilitation practices and in line with the mission, objectives and policies of Coast Mental Health.

Typical Duties:

1. Ensures the implementation of the programs including, leisure activities, cooking, housekeeping and maintenance tasks by engaging the clients daily life skills activities.

2. Provides community based Psycho social rehabilitation services by, identifying and supporting clients in accessing and developing links into community programs and services.

3. Teaches and models life skills and strategies necessary for self care, independent living and interaction with outside social agencies.

4. Applies a range of effective communications strategies such as role modelling to establish a collaborative rapport with the clients.

5. Provides the support and information necessary to help clients build self-esteem and develop decision- making skills through role modelling and other techniques.

6. Assists clients to identify and gain access to formal and informal community supports such as leisure, vocational, educational services through active community integration; providing community services resources of community services.

7. Observes and records the physical and mental health status of clients and take necessary action.

8. Advocates for and with the clients to encourage them to maintain their rights and dignity.

9. Works with clients, peers, supervisors, other agencies and relatives in a cooperative and professional manner.

10. Develops individualized life skills programs with clients involving, where appropriate, family, friends and community resources.

11. Ensures all requirements of the QWIK process and administrative tasks are completed within the allocated timeframes.

12. Uses formal and informal assessment practices such as interviews to develop individual plans identifying the client’s areas of interest and need.

13. Distributes medications safely in accordance with medication policies and procedures.

14. Ensures the implementation of the program’s maintenance, housekeeping and cooking tasks.

15. Reports and addresses health and safety issues/concerns as they arise.

16. Performs administrative tasks, as assigned.

17. Maintains records as required.

18. Fulfills any other functions/duties that may develop in conjunction with the above.

19. May be asked to direct students and volunteers.

20. Performs other related duties as assigned.

Required Qualifications

    • Diploma in Community Social Service or an equivalent combination of education, training and experience or other qualifications determined to be reasonable and relevant to the level of work
