**Responsibilities and** **Accountabilities:**
+ Clean and sanitize processing, packaging and warehouse areas of the facility.
+ Clean and sanitize production equipment in the plant following strict lockout/tag-out procedures.
+ Must be able to work on ladder platforms (elevated work) for: overhead cleaning including vacuuming and brushing & washing of the building structure and equipment.
+ Sweep and wash (mop) floors.
+ Promote good housekeeping and manufacturing practices.
+ Maintain good personal hygiene; clean uniform, hair and beard nets are mandatory for plant GMP (Good Manufacturing Practices).
+ Other duties as assigned.
**Health and Safety**
+ Understand and apply Health, Safety and Environmental (HSE) Policies throughout the facility to prevent injury, illness and property damage.
+ Understand relevant HSE risks associated with the facility and corresponding control measures.
+ Communicate and act on imminent danger situations, hazards, non-conformities and incident reporting.
+ Adhere to relevant HSE programs.
+ Participate in HSE programs and controls, such as behavior-based safety programs, defect management and incident management.
+ Wear and distribute PPE as required.
**Food Safety**
+ Follow plant food safety (HACCP) and GMP compliance, including:
+ Use proper tools (including chemical & lubrication) that meet company food safety standards.
+ Maintain a clean and hazardous free work area while supporting and promoting company GMP's,
+ Safety, and Mondelēz policies.
+ Comply with Company policies, procedures and local legislation.
+ Report all quality and food safety related issues immediately to the Team Captain or Manager for corrections and corrective actions.

+ Ensure all Sanitation/ Process recording & documentation is competed accurately & on time as required.

Position Overview
The Case Manager, Welcoming Streets is responsible for the provision of individualized case management using the Housing First Model and outreach principles. The successful candidate will contribute to Durham Region’s Street Outreach Strategy.
Additionally, the Case Manager, Welcoming Streets will collaborate within an interdisciplinary team of health professionals across the community network that influence the health environment targeted to clients, as well as within the Agency’s team structure, in support of goals that align with and achieve the Agency’s strategy, Mission, Vision and Values.
This program supports vulnerable individuals in Durham Region. Unsheltered individuals are often struggling with complex issues, such as poverty, mental health and addiction, homelessness, and who are disengaged from the health and social service system.
Key Responsibilities
Outreach Services
- Deliver outreach services, including support, education, and referrals in a variety of venues frequented by at-risk individuals across the region, including parks, streets, businesses, libraries, and community agencies.
- Able to deliver mediation and conflict resolution techniques.
- Act as an advocate by aiding in accessing food, housing, and financial assistance resources, as well as addictions and mental health supports.
- Assist in the development, implementation, and evaluation of the program
- Identifies and connects at-risk individuals and families who are homeless or at risk of homelessness with housing supports.
- Provides outreach services that engage people by assessing client’s needs while using the meeting people where they are at approach.
- Promotes partnerships with local service providers to ensure optimal service delivery to clients
- Builds and maintains a network of positive working relationships with other community service providers, professionals, community groups, and landlords, to coordinate program delivery and to ensure effective referrals and a continuum of care.
- Represents the program and DCHC in external meetings and committees
- Provides the necessary resources and supports to stabilize clients housing and enhance housing retention.
- Develops and coordinates client service plans which include the provision of tools and supports to sustain their housing and prevent homelessness.
- Provides follow-up supports to ensure individuals have effectively transitioned to housing and receive related health and social service programs and services.
- Provides referral information/supports (e.g., legal advocacy, housing options and supports, social services etc.).
- Attends and participates in staff meetings, team workshops, agency trainings and professional development sessions and activities.
Administration, Quality Improvement and Reporting
- Document with DCHC’s EMR and HIFIS (The Homeless Individuals and Families Information System (HIFIS).
- Complete Incident Reports as required.
- Follow policies and standards regarding confidentiality and documentation.
- Ensure all necessary reports are completed accurately and in a timely manner.
- Attend relevant team meetings and planning activities.
- Participate in quality improvement and evaluation activities.
- Work with the health and safety guidelines provided by the agency, which includes annual mandatory training requirements.
Other duties within scope of this position include:
- Assists, as assigned, in the orientation, training and mentoring of new staff on the team.
- Consults regularly with local agencies or community resources concerning resource development and program planning which is pertinent to the population served.
- Adheres to and applies organization-wide policies and practices in support of organizational health and performances.
- Adheres to and applies established compliance measures within legislative requirements and equity principles (e.g., AODA, Human Rights Code, and Ontario Employment Standards).
- Champions and supports Employee Engagement efforts and initiatives.
- Ensures scope of duties/accountabilities adheres to established Health & Safety program, policies and procedures.
- Other duties as assigned by the manager and the director.
Key Qualifications
- Completion of a formal 2-year post-Secondary program at a Community College (social service worker diploma, mental health and addictions worker).
- Graduate certificate in Addictions and Mental Health is an asset.
- At least two (2) years of experience in working in a community setting, specifically with street involved populations is required.
- Demonstrated ability to work with marginalized and culturally diverse populations, including those with chemical dependency, mental illness, and HIV positive.
- Screening, intake, assessment, and crisis intervention skills.
- Thorough knowledge and experience of assigned client population, relevant program/services, harm reduction, health education and peer support models.
- Strong knowledge of issues affecting marginalized communities and relevant vulnerable sector clients.
- Experience working with diverse agencies and working successfully in partnership with these agencies.
- Working knowledge of the local health and social service sector, government programs and current legislation that may affect clients.
- Demonstrated ability to work under pressure, to anticipate potential problems/conflicts and take appropriate actions and to meet deadlines.
- Excellent interpersonal, presentation, and communication skills.
- Demonstrated ability to work independently and as a team member within an evolving role in a multi-disciplinary environment.

The Child, Youth, and Family Clinic is the result of a partnership between Carea CHC and The Durham Children’s Aid Society. The health clinic team will provide access to primary care, counselling services, and pediatric medicine as part of an evolving multi-agency hub which is designed to offer a variety of health and social supports for children, youth, and families in Durham, with an emphasis on vulnerable groups and those who experience barriers to care. This is an exciting time of growth! Clinic staff will be part of building a multi-service community resource dedicated to optimizing the health and wellness of children, youth, and families in Durham Region.
Position Overview
This role will contribute to mental health services for children, youth, and families in the setting of a new Child, Youth, and Family Health Clinic in Oshawa. Working as a member of an interdisciplinary team of health professionals, the Therapist provides psychotherapy to individuals and groups in order to assist people to achieve optimal psychosocial functioning. The Therapist practices the principles of client centered care within the context of a self-directed and interdisciplinary team approach. Additionally, the Therapist will collaborate with community partners and other health professionals across the region. The Therapist will be an effective member of the Agency’s team structure, in support of goals that align with and achieve the Agency’s strategy, Mission, Vision and Values. The collaborative practice of this position includes accepting referrals from, consulting with, or referring to other health care providers in order to promote comprehensive and continuous care.
Key Responsibilities
- Provide counselling services to children, youth, and families living in Durham Region.
- Provide brief solution focused sessions, group programs and psycho educational workshops, and collaborate with other team members to expand services.
- Work closely with community partners and existing networks and community partnerships to ensure that the target population is able to receive appropriate services.
- Contribute to providing more accessible targeted services to children, youth, young adults, and families through direct service, referrals, and navigation.
- Occasional community outreach in collaboration with internal and external partners
- Respond to community queries about counselling services; triage support depending on urgency of inquiries; Make community referrals as appropriate.
- Perform administrative duties related to client support.

Key Qualifications
- Completion of a Master's degree - Social work or Counselling Psychology.
- Two years providing counseling/therapy and assessments
- Must be a current member in good standing with appropriate regulatory College (e.g., Ontario College of Social Workers and Social Service Workers (OCSWSSW); College of Registered Psychotherapists of Ontario (CRPO).
- Demonstrated knowledge of scope and standards of practice with the above regulatory College.
- Evidence of skill in developing effective, collaborative therapeutic relationships with clients.
- Evidence of degree-curriculum, or post-graduate education in the theory and use of evidencebased and evidence-informed therapeutic counselling interventions (e.g., Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT) etc.). Experience in delivering therapy/counselling in a brief, solution-focused model of service provision.
- Demonstrated experience providing interventions to clients with complex mental health challenges. Knowledge of DSM-V diagnoses, concurrent disorders, and crisis intervention skills.
- Demonstrated competence in completing biopsychosocial assessments, preparing treatment plans, delivering above interventions, documenting, and communicating with partners in client circle-of-care.
- Demonstrated ability to manage a caseload, determine priorities, meet deadlines, work independently and as an effective member within an interdisciplinary team.
- Knowledge of issues affecting marginalized communities, policies, legislation, programs, and other issues related to scope of practice and social determinants of health.
- Knowledge of child, youth, and family resources in Durham strongly preferred.
- Access to a car and a valid driver’s license required.
- Familiar with electronic health records systems (e.g., PS Suite) and computer skills in use of Microsoft Office.

Job Title: Systems Analyst
Location: Remote
Duration: 6 Months
The ideal candidate will have extensive expertise as a Systems or Integration Analyst with a strong background in healthcare information systems . This role requires proven experience in data modeling, migration, patient data conversion, and healthcare system integration . Candidates must hold a valid PRINCE2 Foundation or Practitioner certification and a post-secondary degree in computer science, software development, business computing, or MIS . Prior government project experience (2–3 years) is preferred.
Must Haves (Mandatory Requirements):
- Post-secondary degree in computer science, software development, business computing, or management information systems (proof required).
- Valid PRINCE2 Foundation or Practitioner certification (proof required).
- Copies of degrees or certifications must be uploaded with submission.
Skills & Experience:
- 10+ years as a Systems/Integration Analyst in a project environment, including integration specifications, data modeling, and data migration.
- 10+ years in implementing clinical information systems in healthcare as an analyst.
- 5+ years analyzing patient data conversion and mapping from central client registry systems to new healthcare systems.
- 2+ years implementing an Initiate client registry system in a healthcare setting.
- 3+ years performing integration and data conversion analysis on RIS or PACS systems (HL7 standard).
- 5+ years implementing Allscripts or Altera Suite of products in healthcare, including integration and reporting analysis.
Preferred:
- 2–3 years of prior experience working on government projects or within a government environment .
