**Home Health Aide - Per Diem**
**Location:** Bellingham, WA
**Status:** Per Diem/Relief | Day Shift | 0.00 FTE
**Hourly Rate:** $22.10 - $33.12 (DOE)

In this per diem role, you'll provide compassionate, hands-on care to home health and hospice patients under the supervision of a registered nurse. From personal care to light household support, your work will help patients live with dignity and ease during challenging times.
**What You'll Do:**
+ Assist with personal care tasks like bathing, grooming, dressing, and toileting
+ Take and record vital signs, measure fluid intake/output, and assist with oral medications
+ Support mobility, transfers, exercises, and the use of assistive devices (walker, cane, etc.)
+ Help with light housekeeping, meal preparation, and feeding as outlined in the care plan
+ Monitor and report changes in patient condition to the RN or case manager
+ Communicate clearly with patients, families, and the care team to ensure seamless support
+ Accurately document care in compliance with regulatory and agency standards
**What You Bring:**
+ Washington State CNA certification (required)
+ Current Basic Life Support (BLS) certification (required upon hire)
+ Valid driver's license and proof of auto insurance
+ At least 2 years of CNA or home health aide experience in a hospital, nursing home, or home care setting
+ Compassion, dependability, and strong communication skills

We are looking for a Home Health Aide to join our team! You will directly work with patients, following a one-on-one care plan in patient homes. You operate with a big heart, not only providing personalized and attentive care but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs. Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health or Hospice experience is a plus.
Responsibilities
- Assist patients with daily activities such as moving in and out of beds, baths, wheelchairs, or automobiles
- Care for patients by changing bed linens, doing laundry, cleaning the home, or assisting with personal care
- Maintain records of patient care, condition, progress, or problems to report and discuss observations with the supervisor
- Build relationships with patients by conversing or reading aloud to help keep them mentally healthy and alert
- Give medications and immunizations
- Engage patients in exercises or other activities
- Graduated from an accredited Home Health Aide program
- High School Diploma or GED
- One year prior professional experience
- Drivers license required
- CPR certification required
