Salary:

We provide primary health care through a range of options including prevention, assessment, diagnosis
and treatment. Our team includes health professionals, educators and support workers representing
both traditional Indigenous and contemporary models of care, and uses wholistic health and
community development concepts.


The position works within an interprofessional team comprised of registered and non-registered
healthcare professionals, both directly and under the facilitation of medical directives, to meet the
home care needs of individual clients at the community level. This position provides primary health
care services in the clients home environment in specified communities within the professional scope
of practice established by the College of Nurses of Ontario.


This position is part of the Ontario Nurses Association Bargaining unit.


NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines
including tobacco, sweetgrass, sage and cedar within the work setting.


QUALIFICATIONS:
current registration as a Registered Nurse with the College of Nurses of Ontario;
willingness to work to a full scope including performing delegated acts through medical directives;
deep understanding and experience of Indigenous culture, values, and perspectives as well as
traditional and contemporary health care practices, and wholistic health and wellbeing;
ability to speak and understand Anishinaabemowin a definite asset;
knowledge of historical trauma and the impacts of that history on the health of people, families, and
communities;
experience working in an Indigenous organization; understanding and/or willingness to learn of
Indigenous culture and values;
proof of immunization in compliance with policy requirements is mandatory;
valid drivers license, own vehicle, willingness and ability to travel required;
criminal records check and current drivers abstract required;
commitment to ongoing training and professional development relevant to job requirements; and
positive attitude and capacity to act as a healthy lifestyle role model.

It was formed in 1995 to facilitate joint action by the member Nations on matters of mutual concern.

Job Description:

The Home Care Nurse will work to the full RN scope of practice to provide culturally sensitive home care services. Will be responsible for supervising and providing direction to HCAs. Work in a team with other home care providers to ensure client needs are meet.

Responsibilities:


  • Case management of clients including admissions, RAI assessments, care planning, family conferences, wound management, and palliative care.

  • Coordinate with LPNs and HCAs to ensure clients are receiving the day-to-day home care services and treatments required.

  • Supervision and scheduling of HCAs for client support and visits.

  • Completion of assessments to determine appropriate living options to best provide for the client and family needs.

  • Complete assessments and applications for coordination of Transition care and Placement.

  • Receive referrals for clients across their life span with varying health care needs.

  • Discharge planning to ensure successful return from hospital to home setting.

  • Collaborate and coordinate services with other members of the health care team and agencies as required.

  • Work independently as required.

  • Ability to maintain accurate health records and fulfill reporting requirements.

  • Effective interaction with clients, families, staff, supervisors and supporting agencies.

  • Ensure that all program initiatives reflect the cultural values, traditions and customs of the Nation.


Skills and Abilities:


  • Excellent and effective decision-making skills, oral and written skills, assessment skills, interpersonal skills, organizational skills, teaching skills and supervisory skills are required
  • Ability to work independently and as a part of a team with other care providers.
  • Willing, capable, and flexible in working to meet the needs of the health department.


Qualifications:


  • A minimum of three years of experience in Home Care Nursing is preferred. A combination of other education and experience will be considered.

  • Demonstrated knowledge and understanding of Cree culture, organizations and communities. Familiarity with Cree language will be considered an asset.
  • Demonstrated knowledge and understanding of indigenous health care concerns.
  • Completion of an accredited nursing education program. Active or eligible for registration and practice permit with CRNA.
  • Valid and current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
  • Computer literacy certification and/or training in entry level usage.
  • Case manager training, RAI assessments and experience with care planning.

  • Responsible for the management of regulated and unregulated care providers, conducting performance appraisals and implementing progressive disciplinary action.
  • Responds to complaints, compliments, and patient referral issues to facilitate an effective resolution.
  • Support patients and their caregivers, ensuring that their questions are answered and a seamless care approach is in place.  Build a trusting relationship to understand the patient goals
  • Effectively build and manage key internal and external stakeholder relationships
  • Address quality, safety, and risk issues, developing improvement plans as needed and participate in Quality-of-care Reviews as required with partners.
Qualifications

  • Registered Nurse (RN) in good standing with the College of Nurses of Ontario.

We have full-time travel roles available in the following specialties:
ICU (Intensive Care Unit)
ER (Emergency Room)
NICU (Neonatal Intensive Care Unit)
Oncology
PACU (Post Anesthesia Care Unit)
Surgical / Critical Care
Maternity / Labor & Delivery
Medical Imaging (X-ray, Ultrasound, etc.)
Home Health / Community Care

Key Responsibilities:
Specialized Care: Provide direct nursing care in Home Health / Community Care - Primary Care and Home Services, and other units.
Team Collaboration: Work with multidisciplinary teams to implement personalized patient care plans.
Administer Treatments: Administer medications, monitor patients, and adjust care plans as needed.
Educate Patients: Educate patients and families on health management, disease prevention, and post-care plans.

Locations:
We offer short-term travel opportunities across British Columbia, including:
Interior Health: Kelowna, Kamloops, Penticton, Vernon, Cranbrook
Fraser Health: Surrey, Langley, Abbotsford, Chilliwack, Maple Ridge
Vancouver Coastal Health: Vancouver, Richmond, North Vancouver, West Vancouver
Vancouver Island Health: Victoria, Nanaimo, Campbell River, Courtenay
Northern Health: Prince George, Fort St. John, Dawson Creek, Terrace

Qualifications:
Registered Nurse (RN) licensed to practice in BC (or eligible for registration).
Experience in Home Health / Community Care - Primary Care and Home Services, or other relevant specialties is preferred.
BCLS Certification required.
Bilingual (English/French) is an asset for positions in bilingual regions.
