Job Summary
The Residential Support Worker provides a range of assistance and supports to clients living with mental illnesses in a Program residence or independently in the community.

What You Will Do
Through a team oriented professional approach, the position provides support to the client to increase their independence, dignity and self-respect, in a safe and secure environment by:

1. Building a rapport with each individual and providing coaching or mentoring to enable each in meeting the daily requirements of their housing environment and a healthy lifestyle; directly assisting with their daily living activities, particularly in the acquisition of life skills in such areas as hygiene, medication management, cooking, meal planning, cleaning, shopping and budgeting; developing strategies or establishing plans for maintaining these daily activities and other aspects of their recovery plan, for increasing their awareness of safety issues and for ensuring that residents receive adequate support in various aspects of their life; orientation of new clients; participating in intake for the program.

2. Supporting and participating in house meetings, resident association activities or other group activities which promote the wellbeing of the residents and the smooth operation of the house; conducting and facilitating resident group meetings, assisting client-run initiatives, preparing schedules of house activities; maintaining a general awareness of the activities or whereabouts of residents in the house, identifying issues, communicating or noting incidents for director as required; maintaining petty cash, collecting rents and intervening in issues pertaining to maintenance or property; providing cleaning or minor house maintenance as required; teaching residents the appropriate fire safety protocols and conducting regular health and safety checks;

3. Maintaining a strong support network of social, family, medical and psychiatric community links and resources; providing reminders and occasionally accompanying clients on their medical, psychiatric or other appointments; assisting the client in navigating the social services and health care systems and liaising with community agencies, hospitals or significant others; providing 24 hour crisis support on a rotational basis through a pager or similar system;

4. Promote the psycho-social rehabilitation/ recovery approach, assist in harm reduction strategies, teaching conflict resolution, crisis management and problem solving on activities related to shared living; mediating issues or conflicts between residents, or between residents and neighbours, following-up on issues to ensure satisfactory resolution;

5. Participating in the administration of the program by completing and maintaining the required records or statistics such as resident files, activity sheets, reports, etc. In accordance with established procedures; participating actively as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities, orienting new staff, determining staff schedules; providing other related assistance as required.

What You Bring:
Must Haves
  • An understanding of harm reduction, and psychosocial rehabilitation principles
  • A relevant degree/diploma in human services,
  • At least three years of experience working with the client population
  • Knowledge of the issues faced by adults with special needs and a marginalized client group which can include addictions, cognitive impairments, HIV/AIDS.
  • An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.
  • Experience in working as part of a team of professionals, and in conducting meetings or facilitating groups.
  • Excellent interpersonal, communication and counselling skills are required to work in a team environment and to engage the clients in some highly personal areas, such as developing healthy living skills, life skills and in maintaining interpersonal relationships.
  • Good crisis intervention and problem solving skills are required.
  • Computer literacy sufficient to produce and maintain files, records or statistical information.
  • Written skills are used in keeping records or updating logs.
  • Good analytical, time management and organizational skills to contribute to program planning and to co-ordinate and follow-up on issues or activities with clients.
Work Environment:
  • Will be required to work in resident rooms and may be exposed to second hand smoke.
  • Required to be on after-hours on call, on a rotating schedule.
  • Walk up and down stairs, physically able to assist client with daily living activities
  • 10%-20% travel to different locations with transit and/or own vehicle, outdoor travel,
  • Lifting objects up to 50 lbs.

JOB OVERVIEW

Residential Support Workers are responsible for assisting in any and all aspects of daily living that are required by the resident. Support Workers will be responsible for support in the completion of daily routines, nutrition management, health management, appointment scheduling, observation and documentation of daily events and activities, medication administration and management, and housekeeping, as well as providing comfort and companionship to the residents.


DUTIES AND RESPONSIBILITIES


  • Residential Support Workers will need to be comfortable with differing levels of care delivery. Residential Support Workers should expect to provide support at levels ranging from total care with a residents activities of daily living to light support and supervision.

  • Residential Support Workers will be expected to become competent in any and all specialized supports required for a resident.

  • All support delivered within this service must meet the standards legislated by the Department of Social Development.

  • Support Workers must keep detailed daily observation records. This practice is of the utmost importance in delivering consistent quality support to residents.

  • Residential Support Workers will be expected to provide support with the administration of medication. Competency in all aspects of medication administration, including detailed record-keeping and inventory, is a requirement.

  • Staff must take an active role in planning, implementing, and evaluating Individual Service Plans that meet residents needs.

  • Employees will be required to engage in ongoing training for the purposes of professional development and competency in pertinent tasks. The Mandt System, First Aid CPR/AED, Universal Healthcare Precautions, Autism Intervention and Support training are a few examples of professional development training that may become available.

  • Residential Support Workers will be responsible for knowing all emergency procedures (e.g. evacuation protocol).

  • In the event of a medical emergency, Residential Support Workers will be expected to use their first aid training to assess the situation and proceed accordingly.

  • Welcome Home Residential Services provides services 24 hours a day, 7 days a week. Overnight staff will be required to remain awake and vigilant throughout the night.

Overview:

The Residential Support Worker is responsible for supporting individuals while facilitating an atmosphere of safety, independence, social interaction, and community involvement. The Residential Support Worker will work alone for twelve-hour shifts to facilitate 24/7 shift coverage.

For the Kenora Supportive Housing Program this includes participating as a member of a team of professionals whose focus is safety, stability, and support for individuals with serious mental illness who would otherwise experience homelessness or near homelessness. The Residential Support Worker will work twelve-hour shifts to facilitate 24/7 shift coverage. They will participate as a member of a team of professionals whose focus is safety, stability, and support for individuals with serious mental illness who would otherwise experience homelessness or near homelessness. The team will include the resident/client, residential support worker, Case Manager of the Kenora Supportive Housing Program, community case manager or ACT Team and other residential staff members.

For the Safe Bed Program, the individual will participate as a member of a team of professionals whose focus is safety, stability and support for individuals who are medically stable, not a safety risk to themselves or others, and are therefore, not suitable for hospital admission or criminal detention. The Residential Support Worker will work the following shifts: twelve-hour shifts to facilitate 24/7 shift coverage, eight hour shifts in the evening to facilitate overnight shift coverage from 7:00 p.m.- 3:00 a.m. and eight hour shifts on the weekends from 11:00 a.m. - .7:00 p.m. They will participate as a member of a team of professionals whose focus is safety, stability, and support for individuals in a mental health crisis and in immediate contact with the police. The team will include the resident/client, residential support workers, Team Lead, nursing staff (both internal and external),Counselling & Treatment Team staff, Case Manager for the Safe Bed Program and Addiction Specialist from the Canadian Mental Health Association, Kenora Branch, community services providers and other residential staff members.


Qualifications:

  • Diploma in social sciences or human services an asset, High School Graduation Diploma minimum
  • Experience working with individuals with mental health struggles and/or demonstrated ability to work with vulnerable populations
  • Personal qualities of empathy, understanding, patience
  • Ability to participate side by side with individuals to accomplish tasks and develop meaningful activities
  • Ability to carry out and role model activities of daily living
  • Case management, advocacy and recording skills
  • Understanding and applied knowledge of Recovery Model
  • Understanding of trauma informed practice and implementation
  • Good Interpersonal communication and relationship building skills
  • Ability to communicate professionally with community agencies
  • Knowledge of community resources
  • Cultural competence for diverse populations

Duties:

  • Adherence to all policies of Canadian Mental Health Association, Kenora Branch
  • Side by side support/teaching in activities and skills of daily living
  • Ensure medications are appropriately stored and delivered to clients at the appropriate times and dosages, and documented
  • Supporting clients with developing, participating, and following through with recovery plans/coordinated care plans
  • Outreach and support with clients as directed by the Safe Bed Program Team Lead & Chief Executive Officer
  • Ensure the safety and security of all residents during shift, including interpersonal safety and security of the residence
  • Monitor individual recovery plan/coordinated care plan compliance and changes in mental health symptoms
  • Interface with community services and programs with and on behalf of residents
  • Plan for groceries, meals, household maintenance and supplies in conjunction with other residential support staff
  • Ensure the completion of shift responsibilities and routines as identified in agency policy and procedures
  • Maintenance of client files and data files including updating of client information consistent with agency policies and procedures
  • Attend monthly staff meetings and educational/training opportunities as required
  • Documentation of all shift changes using the electronic scheduling software and follow agency policies and procedures
  • Participate on specific committees or working groups as assigned by the Chief Executive Officer
  • Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures; including but not limited to Fire, Disaster and Evacuation Plans

  • Supporting and participating in house meetings, resident association activities or other group activities which promote the wellbeing of the residents and the smooth operation of the house; conducting and facilitating resident group meetings, assisting client-run initiatives, preparing schedules of house activities; maintaining a general awareness of the activities or whereabouts of residents in the house, identifying issues, communicating or noting incidents for director as required; maintaining petty cash, collecting rents and intervening in issues pertaining to maintenance or property; providing cleaning or minor house maintenance as required; teaching residents the appropriate fire safety protocols and conducting regular health and safety checks;
  • Maintaining a strong support network of social, family, medical and psychiatric community links and resources; providing reminders and occasionally accompanying clients on their medical, psychiatric or other appointments; assisting the client in navigating the social services and health care systems and liaising with community agencies, hospitals or significant others; providing 24 hour crisis support on a rotational basis through a pager or similar system;
  • Promote the psycho-social rehabilitation/ recovery approach, assist in harm reduction strategies, teaching conflict resolution, crisis management and problem solving on activities related to shared living; mediating issues or conflicts between residents, or between residents and neighbours, following-up on issues to ensure satisfactory resolution;
  • Participating in the administration of the program by completing and maintaining the required records or statistics such as resident files, activity sheets, reports, etc. In accordance with established procedures; participating actively as a team member in program delivery by providing support and consultation to colleagues, identifying gaps in service and recommending solutions, participating in meetings, planning and evaluation initiatives, committees, events and activities, orienting new staff, determining staff schedules; providing other related assistance as required.
What You Bring
Must Have
  • An understanding of harm reduction, and psychosocial rehabilitation principles
  • A relevant degree/diploma in human services,
  • At least two years of experience working with the client population
  • Knowledge of the issues faced by adults with special needs and a marginalized client group which can include addictions, cognitive impairments, HIV/AIDS.
  • An understanding of the therapeutic relationship with an ability to negotiate with clients on goals and priorities.

About the Role and Stonehenge Therapeutic Community:

Our Residential Support Staff (RSS) provide support to clients attending our longer-term residential addiction treatment program in Guelph. Our approach to treatment is a group approach, based around peer support and mutual self help. Our team contributes to our clients' recovery journey by demonstrating positive and healthy behaviours (role modelling), engaging in supportive counselling, containment and crisis intervention, and participating alongside residents in community living activities (participating in mealtimes and outdoor recreation activities).


The Relief RSS team must be available to be scheduled for a minimum of 2 shifts per month, working from 12:30pm to 10:30pm. These hours are scheduled on Saturday, Sunday and/or Monday throughout the scheduling month. Additionally, Relief staff will be contacted to work last minute assignments as needed (to cover unexpected staff absences) or can pick up open shifts as they become available. The Relief team will also provide coverage for statuary holidays, staff vacations and staff absences. These additional requirements are in addition to the regularly scheduled shifts outlined above and will require flexibility to work on other shifts or dayparts as needed (example overnight, evenings or weekdays).

The Part Time RSS team will be scheduled for 4 shifts per month, working every second weekend (Saturday and Sunday) from 7:30am to 5:30pm. Similarly to the Relief team, they will also be required to provide coverage for holidays, vacations and absences, as well as attending the monthly staff meeting.


The Part Time Overnight RSS team will be scheduled for 3 shifts per week (30 hrs), which some weekday shifts as well as working every second weekend (Saturday and Sunday), on a set rotation. Hours of work are scheduled from 10pm to 8am. Overnight staff are eligible for enrollment in the Agency's health/dental benefit plan.

MISSION

Reporting to the Director of Client Services and Wellness, you will guide the work of a team of intensive case managers and case management support staff, based on the principles and best practices of Housing First for Youth. You will manage all case management functions at the organization, including intensive case management services and supportive services that support youth to achieve their individual goals.

Case Management Services provides intensive, client-centered case management to vulnerable and chronically homeless youth (ages 16–24) in the Region of Peel. Working within a trauma-informed, anti-oppressive, and harm reduction framework, Case Management Services supports youth in achieving housing stability and long-term well-being through assessment, planning, facilitation, care coordination, engagement, evaluation, and advocacy. Case Management Services are provided at all organizational sites, including emergency shelter, transitional housing, and the community.

The Pod Group is partnering with this organization to place a hands-on, collaborative, and compassionate individual to fulfill the Case Management Services Manager position.

PRIORITIES

  • Lead programs and services. Using Housing First for Youth and Harm Reduction principles and best practices, you will develop and manage the delivery of intensive case management services and supports for youth. Programs in this portfolio include the following:
    • Intensive Case Management Services – You will oversee the delivery of youth-driven, housing-first intensive case management services and supports, to include assessment using Social Determinants of Health domains, risk assessment, safety planning, and provision of services to support client goals.
    • Client Supports – You will operationalize flexibility and low-barrier access as the primary goals of case management interventions.
    • Ongoing Quality Improvement: You will consistently identify and address systemic barriers to ensure timely access to essential support and services that promote youth housing stability, overall wellness, and long-term self-sufficiency.
  • Provide team leadership. You will lead team meetings, case consultations, and skill-building activities across all organizational sites to ensure consistency and effectiveness of case management services.
  • Develop Internal Competencies & Capacity Building. You will assess, analyze and prioritize a training and capacity building plan to elevate the awareness and understanding of case management practices and competencies of all staff across the organization.
  • Team Development. You will implement accountability measures, such as observing case management meetings with youth, reviewing reports, and conducting caseload reviews. You will manage bi-annual file audits, provide constructive feedback, and evaluate direct reports to identify growth opportunities. Requirements

TALENTS & EXPERTISE

  • The education and experience. You have a Bachelor's Degree in Social Work, Psychology, Child and Youth Work, or related field. Masters in a related field is considered an asset. You have at least 3 years of experience in the housing and homelessness sector in a case carrying role, prior to management experience.
  • The management experience. You have a minimum 3 years of experience working as a supervisor/manager in the housing and homelessness sector. You have site and team management experience, including exposure to human resource management (union experience is a strong asset), financial management, program management, evaluation design, or systems design.
  • The case management knowledge. Y ou understand best practices and current literature in case management assessments and evidence-based case management modalities when working with youth. You demonstrate an understanding of issues relating to youth homelessness and the barriers youth face in attaining and maintaining their housing, as well as an understanding of the broader housing sector in Peel.
  • The leadership. You are a demonstrated leader with excellent communication, presentation, organization and interpersonal skills with a passion for supporting the most vulnerable and marginalized youth in our community. You are passionate about mobilizing individuals and teams to drive change and collaborating with partners and stakeholders at all levels to build consensus.
