Reporting to the Clinic Manager and with the support of the Nursing Supervisor, the LPN provides safe, competent, compassionate and ethical nursing care. The LPN helps to manage a designated panel of patients in collaboration with the physician and team Additionally, the LPN will also have a teaching and mentoring component to the role whereby they will participate in the education of physicians, residents, MOAs and non-physician learners. There will also be a collaboration with other health care professionals/providers to help achieve our model of Patient-Centered Care.


Duties and Responsibilities


  • Coordinates daily team huddles with a physician, resident, MOAs
  • Performs daily procedures within scope of practice, such as management of minor acute and stable chronic conditions based on protocols
  • Performs clinical care, such as, but not limited to: wound care, assisting with procedures, injections, medication administration, point of care testing/sample collection, STI assessment, cryotherapy, prenatal visits, well child checks, etc.
  • Telephone triage and assessment; provide advice to patients and families
  • Provides patient education
  • Care coordination within team and with allied health, as required
  • Performs home and group visits as required
  • Proactive management of patient populations in collaboration with team
  • Reviews physician task list and manages test results/outstanding issues by protocol
  • Manages labs and screenings by protocol
  • Manages prescription renewals by protocol
  • Assists with form completion for physicians
  • Responsible for the orientation, training, and mentoring of MOAs and LPNs
  • Teaching Residents orientation, guidance, procedure assist, allowing them to observe
  • Works with the clinic manager to develop and operate quality control and CQI programs
  • Supports Significant Event identification and analysis processes



Qualifications


  • Completion of accredited nursing education program required
  • Active or eligible for registration and practice permit with a relevant regulatory college
  • Criminal Record and Vulnerable Sector check
  • Current Basic Cardiac Life Support Healthcare Provider (BCLS-HCP)
  • Certification in wound care, preferred
  • Certification in immunization, preferred
  • Up to date vaccinations
  • Minimum 3 years experience, preferred
  • Primary Care/Family Practice experience, preferred
  • /Experience working within a team-based approach to patient care
  • Knowledge of Primary Care Medical Home Model
  • Experience leading a team, teaching and mentoring
  • EMR Experience (Medaccess preferred)
  • Proficient in MS Office Suites
  • Excellent communication and time management skills

Position Summary:

The Licensed Practical Nurse provides direct nursing services to individuals with physical and intellectual disabilities that focus on health promotion, restoration, maintenance, and prevention of illness and disability and palliative care. The Nurse is responsible for providing total patient care but may delegate some aspects of care to other health care team members in a staff mix model e.g. Resident Assistants. In addition to providing direct practical nursing services, the Nurse functions as a key member of the professional health care team and is responsible to provide day-to-day instruction and feedback to the Resident Assistants assigned to their care team.

Qualifications

Educational:
- Graduate of an approved LPN educational program

Professional:
- Holds current active practicing membership with the CLPNM

Assessment duties include:
- Assess people supported using a variety of resources and focusing on biological, emotional, socio-cultural, spiritual and quality of life factors
- Use knowledge from practical nursing and data from other members of the health care team to analyze and interpret the data necessary to identify potential and actual health needs/problems
- Perform ongoing assessment to identify changes in person supported health status

Planning duties include:
- Develop individualized care plan in collaboration with other health team members
- Include the person supported and their family, as possible, in developing the individual, focused nursing/health plan of care
- Supports individual health and growth and development by building on personal, family and community strengths and established support systems
- Supervises the implementation of the individualized plan of health and personal care

Treatment duties include:
- Organize work for the efficient delivery of person supported care
- Provide direct nursing care to assigned group of individuals
- Provide health information to person supported, family and legal decision makers in an appropriate and timely manner
- Facilitate a least restrictive environment and promote awareness of people supported rights and a person-centred approach to care

  • LPN and in good standing with BCCNM
  • Current CPR/First Aid certification

JOB PURPOSE:
Reporting to the Clinic Manager, the Licensed Practical Nurse is responsible for providing exceptional customer service to our patients and physicians and executing on the organization’s strategic objectives as it relates to clinic operations and patient care. 

PRINCIPAL RESPONSIBILITIES:

  • Greeting and registering patients
  • Initiating forms, creating billings and collecting payment for uninsured services
  • Escorting patients to exam rooms and preparing patients for physician
  • Setting up and assisting with minor surgical procedures, pelvic examinations/IUD’s
  • Performing diagnostic testing as ordered by the physician including but not limited to urinalysis, simple blood glucose testing, pregnancy testing
  • Performs a variety of care interventions and medical procedures as permitted by the physician, within the designated scope of practice
  • Aware of organization’s core purpose and core values and performing to these Core Values on an ongoing basis
  • Accurately charting any medical procedures or interventions performed in the patient chart
  • Follows standards of cleanliness and safety
  • Scheduling and notifying patients of appointments
  • Scanning patient documents as required
  • Opening mail, receiving faxes and distributing appropriately
  • Completing daily, weekly and monthly chore lists
  • Participating in the training and orientation of new medical staff and students
  • Understands physician profiles and executes on requests in order to properly support physicians
  • Actively participates in company initiatives
  • Performs other duties as assigned
KNOWLEDGE, SKILLS AND EXPERIENCE:
  • A two (2) year diploma from an accredited institution
  • Current registration and in good standing with the College of Licensed Practical Nurses of Alberta
  • Minimum of one (1) year customer service experience
  • Experience with Med Access software is preferred
  • Excellent communication, listening and problem solving skills
  • Excellent clinical, customer service and organizational skills
  • Ability to handle confidential information in compliance with the Health Information Act guidelines
  • Strong computer skills including Excel
  • CPR and immunization
