  • Consult with and be a resource for other health care professionals including physicians, nursing staff, pharmacist, social workers, etc.
  • Conduct in-service education on nutrition topics to dietary and nursing staff (1 topic bi-monthly)
  • Provide advice and communication to other health professionals regarding therapeutic diet recommendations, swallowing assessments and any other nutrition related topics.
  • Develop, implement and support departmental programs to improve resident nutritional status as well as matters related to risk indicators, quality improvement, productivity, menu development, clinical manual coordination, standards of care and resident menu satisfaction survey asper MOH & LTC standards and guidelines.
  • Participate in relevant clinical teams, resident care conferences.
  • Other related duties required or assigned
  • Some assigned management duties of the food service department on weekdays and/or weekends
Qualifications

What You’ll Need to Succeed: 

  • Bachelor of Science with focus in Food & Nutrition or equivalent
  • Completion of a recognized Dietetic Internship
  • Member of Dietitians of Canada
  • Current registration with the College of Dietitians of Ontario
  • Experience as a Clinical Dietitian, preferably in long-term care
  • Excellent verbal and written communication skills
  • Proficient with Microsoft Outlook suite 
  • Excellent organizational skills
  • Strong problem solving and decision-making skills
  • Ability to work well within a cross-functional team
  • Knowledge and understanding of MOH & LTC standards and guidelines

As a Registered Practical Nurse (RPN) joining our Village, you will:

  • Maintain current registration with the College of Nurses of Ontario.
  • Have existing Heart Save Level of CPR.
  • Demonstrate an ability to prioritize tasks effectively, properly plan for, and allocate the right amount of time and effort to complete tasks. Remains calm and manages stress appropriately
  • Deliver safe administration of medication and treatments with a focus on accuracy, quality and meeting our residents’ quality of life requests.
  • Follow plans of care (or service plans) in accordance with the standards of the College of Nurses and able to identify and report any changes in the resident’s well-being to the Charge Nurse or Village leadership team.
  • Have an understanding of relevant policies, procedures, legislation, and regulations applicable to the Long-Term Care sector, including the Fixing Long-Term Care Act, 2021, and the Occupational Health & Safety Act (OSHA) of Ontario.
  • Support and embrace our resident-centred social culture and be proud to deliver the highest quality of personal care to our residents.
  • Work as a key member of our Village and Neighbourhood multidisciplinary team to understand residents’ individual needs, and advocate for residents to actively participate in decisions about their care, support new resident move-ins, facilitate transfers to hospital, or any changes in resident medical needs.
  • Collaborate with the Neighbourhood Physician during weekly rounds and update care plans and directions accordingly.
  • Have proficient communication and computer skills in order to update care plans in the current system.
  • Understand the physical, mental, and emotional demands of the RPN role.

Receiving and transcribing verbal prescriptions from practitioners
Ensuring prescriptions are complete and authentic
Providing injections/vaccinations as applicable by regulations in certain jurisdictions
Transferring prescriptions to and receiving prescriptions from other pharmacies
Ensuring the accuracy of prepared prescriptions
Conducting the final check of prepared prescriptions
Confirming the accuracy of drug and personal health information in patient records
