Our services include supports for seniors, caregivers, and people living with mental health and addictions concerns. We pride ourselves on being at the forefront of client-centred care in our sector.
Reporting to a clinical program manager, the Mental Health Worker will provide on-site assessment, intake, discharge planning and support as well as liaison with existing supports and assistance with goal setting and planning, to clients of this program in West Toronto. The responsibilities of this position include, but are not limited to:
What You'll Do:
You will be entrusted with a wide range of responsibilities, including:
- complete Intake and client assessments
- co-ordinates client access to short-term crisis beds;
- provides a variety of individualized on-site supports including, but not limited to, addressing immediate short-term housing needs, accessing financial aid, assessing medical supports, working with families, crisis prevention / intervention, advocacy, referrals to other services, etc.;
- liaises and collaborates with other mental health and justice service providers, mobile crisis teams, hospitals, physicians, community workers, substance abuse programs, landlords and family members on behalf of clients;
- delivers all services in a professional manner and according to established guidelines and associated benchmarks;
- actively participate in household tasks including cleaning and domestic routines.
- attend bi-weekly team meetings
The successful candidate will possess:
- University degree and/or Community College Diploma in social services or related field
- Minimum two years related experience in direct service delivery to adults with serious mental illness and addictions.
- Knowledge of mental illness and addictions, the mental health system/s and other concepts related to the effects of serious mental illness and addictions
Key skills and abilities we are looking for:
- Excellent verbal and written communications skills
- Experience in provision of residential / housing supports to adults with serious mental illness and addictions.
- Knowledge of housing, health care and community resources
- Experienced in working effectively as a member of a team, which includes internal and external stakeholders.
- Applied computer skills (Microsoft Office, Client Information Systems, Web browsing)
- Fluency in a second language is a strong asset and preferred.
Special Requirements:
- Criminal background check
- Must be willing to work flexible hours including weekends, evenings and statutory holidays as required.

Job DescriptionThe Nurse Practitioner will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing the care of eating disorder clinic patients who require assessment and ongoing monitoring of physical status. The Nurse Practitioner abides by the practice standards as set out by the College of Nurses of Ontario.
Accountabilities:
- Assessing patient medical status as it relates to Eating Disorders diagnoses
- Collecting and interpreting data about the health of the patient
- Collaborating with the clinic physician and/or the family physician to provide follow up, providing psycho educational and treatment groups, and ongoing support to patients and their families
- Acting as resource and providing educational support to other areas of the hospital, family practitioners and the community
- Participate in program development and evaluation
- Perform other duties as required
- Masters degree in nursing/ACNP/current extended class registration RN (EC) required
- CNA certification in mental health would be an asset
- Nurse Practitioner designation to treat across the lifespan is required
- Nursing experience in working with patients who meet DSM5 diagnostic criteria for an eating disorder is required
- Nursing experience in working with other mental health diagnoses
- Exemplary clinical nursing skills, knowledge and expertise, especially in patient assessments and consultation with family physicians
- Experience in the provision of psychoeducational and psychotherapy (individual and group) is preferred
- Successful completion of a Nurse Practitioner program from a recognized university
- Demonstrated ability in the core NP competencies as outlined in the CNA framework
- Demonstrated clinical leadership skills, and strong analytical and conceptual skills
- Demonstrated ability to work independently
- Demonstrated efficient work framework
- Demonstrated excellent organizational and communication skills
- Demonstrated commitment to Osler's Vision, Mission and goals
- Proficiency in computer skills and data management required (e.g. Microsoft Excel, Word, Access)

They are currently seeking an experienced and ambitious Mental Health and Addiction Physician to join their growing team.
Your new role
In this new role you will be responsible and accountable for the leadership and overall success of the location working alongside the Executive Director and Medical Director. In this role you will act as the MRP for patients in treatment, follow them through weekly appointments and cultivate a strong therapeutic alliance. Your professional duties will include conducting: pre-admission, admission, withdrawal management, and weekly follow up assessments of patients.
What you will need to succeed
To be considered for this position, you must have at least 3 years practical and related experience in a similar role. You should have graduated from an accredited Canadian program and be in good standing with all the various regulatory bodies. To be successful, you should also have excellent knowledge of standards (e.g., ISO) and regulations. Demonstrated excellent interpersonal skills and the ability to work collaboratively with all members of the health care team. A passion for working with a vulnerable mental health population is the most important requirement.

Job Summary:
S.R.T. MedStaff is currently seeking facility registered nurses (RNs) and registered practical nurses (RPNs) to join our dynamic team! We are currently hiring nurses to work in a downtown Mental Health Facility. The mental health nurse (RN/RPN) provides nursing care within the scope of his/her education and experience to patients within the facility setting.
Responsibilities:
- Provide nursing care to patients according to the written plan of care
- Evaluate effectiveness of treatment plan(s) and adjust accordingly in consultation with the patient/family/caregiver and other health care professionals
- Report/communicate observations to the appropriate individual(s) as necessary
- Act as a patient advocate by protecting and promoting patient rights
- Treat patients with compassion and respect, recognizing and valuing diversity
- Maintain confidentiality of patient and/or hospital information
- Follow S.R.T. MedStaff’s and the assigned facility’s written policies and procedures
- Work as a team player with other health care professionals and service providers
Qualifications:
- Current registration with the College of Nurses of Ontario, in good standing
- Minimum 1-year full time work experience as an RN/RPN within the past 2 years
- Current CPR (BCLS) Certificate; ACLS/NRP for Specialty Nurses
- Meet health surveillance requirements of the Public Hospitals Act
- VSS within the past 2 years of hire confirming absence of a criminal history/record
Schedule:
- Must be available to work a minimum of 6, 12-hour shifts over a 2 week period
- Flexible to work all shifts (days, evenings, and nights) including weekdays, weekends, and statutory holidays
