What you’ll be doing:

  • Identify and plan for discharge needs including coordination of referrals to appropriate community resources and outpatient counselling
  • Supervise students, interns and non-regulated support personnel as appropriate
  • Awareness and acknowledgement of the value of using measurement-based care when supporting patients/clients to help guide clinical decision making; experience an asset

What we’re looking for:

  • Master’s degree of Social Work, Counselling or Psychology
  • Registration with CRPO 
  • Experience with facilitating evidence-based therapy groups and individual psychotherapy
  • Experience treating First Responder, Health-care professionals and Military populations would be an asset
  • Demonstrated experience in treating trauma, stressor-related disorders and concurrent psychiatric and substance use disorders
  • Knowledge of standardized and non-standardized assessments
  • Experience with integrating case conceptualization model within trauma services
  • Knowledge and application of relevant mental health acts and community resources
  • Assessment of group dynamics and facilitation skills for group therapy
  • Formal training in, and experience providing with, CBT, DBT, Cognitive Processing Therapy (CPT), Mindfulness, Trauma-informed Yoga, Psychotherapy, and Counseling is preferred
  • Knowledge of medical and psychiatric conditions, evaluation techniques, and psychometrics properties needed to evaluate therapeutic interventions and functional status
  • Comprehension of psychiatric emergencies and crisis management skills would be preferred
  • Service-specific training and knowledge of psychosocial rehabilitation, childhood trauma and sexual abuse, recovery management Mental Health Act, PHIPA, Ontarians with Disabilities Act 2005, Health Care Consent ACT
  • Knowledge of concurrent addiction disorders for specific addiction assessment tools
  • Strong organizational, time management and presentation skills
  • Proficiency with Microsoft Word, Excel, Outlook and PowerPoint
  • Exceptional communication and customer service delivery skills
  • Demonstrated success working independently as well as contributing actively to team objectives

Our client is one of the country’s largest providers of mental health and addiction services, with inpatient operations in most provinces and an in-person and virtual outpatient program. They are currently seeking an experienced and ambitious Mental Health and Addiction Physician to join their growing team.

Your new role

In this new role you will be responsible and accountable for the leadership and overall success of the location working alongside the Executive Director and Medical Director. In this role you will act as the MRP for patients in treatment, follow them through weekly appointments and cultivate a strong therapeutic alliance. Your professional duties will include conducting: pre-admission, admission, withdrawal management, and weekly follow up assessments of patients.

What you will need to succeed

To be considered for this position, you must have at least 3 years practical and related experience in a similar role. You should have graduated from an accredited Canadian program and be in good standing with all the various regulatory bodies. To be successful, you should also have excellent knowledge of standards (e.g., ISO) and regulations. Demonstrated excellent interpersonal skills and the ability to work collaboratively with all members of the health care team. A passion for working with a vulnerable mental health population is the most important requirement.

Job Summary:

 S.R.T. MedStaff is currently seeking facility registered nurses (RNs) and registered practical nurses (RPNs) to join our dynamic team! We are currently hiring nurses to work in a downtown Mental Health Facility. The mental health nurse (RN/RPN) provides nursing care within the scope of his/her education and experience to patients within the facility setting.

  • Meet health surveillance requirements of the Public Hospitals Act
  • VSS within the past 2 years of hire confirming absence of a criminal history/record


 

Schedule:

  • Must be available to work a minimum of 6, 12-hour shifts over a 2 week period
  • Flexible to work all shifts (days, evenings, and nights) including weekdays, weekends, and statutory holidays

Job Description

The Centralized Intake Coordinator reviews and triages all referrals to the Mental Health team at Osler, providing timely and appropriate access to care. The position will coordinate referrals to the mental health therapists at all sites and conduct standardized assessments. This role collaborates with referral sources, members of the Mental Health interdisciplinary team to ensure timely consultation by the appropriate service within the program.

Accountabilities:

  • Review and triages all referrals to the appropriate mental health therapists for treatment based on the level of care required
  • Determine status of referral – discharge, refer to psychiatry regular or urgent, refer to psychiatry and group, refer to group only and provide patient with community resources/supports
  • Book the patient into the group through MSM (including virtual groups) at Peel Memorial Centre for Integrated Health and Wellness, Brampton Civic Hospital, Etobicoke General Hospital and Queen Square Family Health Team
  • Respond to family physicians calling the SCOPE line; navigate, direct call to appropriate service
  • Liaise/consult with referral source as needed to help patients and families navigate through the mental health and addiction services
  • Ensure patient needs are met, manage wait list and maintain communication with referral source
  • Redirect the patient to community supports/community psychiatry via a faxed letter to the referral source (in situations where there is a request for new psychiatrist or ADHD/ADD assessment and treatment)
  • Liaise/consult with colleagues in the various mental health programs as needed; answer enquiries
  • Participate in program planning and development; participate in quality improvement and program evaluation initiatives
  • Provide guidance and directions to patients and families on how to access in-house programs and available community supports
  • Perform other duties and responsibilities as assigned by management
Qualifications

  • Experience within the last five years, with minimum two to three years of experience working in a mental health program required
  • Must possess broad clinical skills and knowledge to review a patient's mental health diagnosis and symptomology to determine specific consultation requirements and potential for urgent referral or intervention
  • Registered Social Worker (BSW) or undergraduate degree in a recognized health care profession required
  • Member in good standing of a professional college required
  • Previous intake experience preferred
  • Excellent communication skills and demonstrated ability to communicate effectively with patients and colleagues
  • Experience using a computerized triage system required
  • Strong computer skills including Microsoft Office suite
  • Experience in working within an interdisciplinary setting
  • Ability to function both independently and as a team player

Additional Information

Position Summary:

TheMental Health and Addiction Peer Support Workeris responsible for providingprovide peer support to people who have been released from jail; harm reduction education, overdose prevention information/tools, community accompaniment, connecting clients to community supports, informal counseling, follow-ups and advocacy.If appropriate and/or possible, may provide direct client services at the Maplehurst Correctional Institution to assist with release.

Specific Peer Support as it applies to this position:The candidate must be someone with lived experience of incarceration and/or substance use, and/or the mental health.

Job Accountabilities

  • Provide peer/case management support to a caseload of 15-20 clients.
  • Produce and maintain logs, files, records or statistical information through the use of sufficient written and computer skills.
  • Connect directly with people released from custody, their family members, and/or loved ones to provide immediate support, conduct brief needs assessments, advocacy and make referrals accordingly.
  • Attend Reintegration Services site and the immediate vicinity of the jails Release and Property doors to approach potential service users and their loved ones.
  • Attend community meetings and assist with data collection as needed.
  • Present information about the agency and its programs as a representative of John Howard Society of Peel-Halton-Dufferin in public settings as directed.
  • Assist with research and evaluation endeavors as directed.
  • Actively participate in formal supervision sessions on a regular basis (i.e. every four weeks) and weekly bi-weekly team meetings as required.
  • Work closely and collaborate with Probation and Parole Officers, community partners, and other stakeholders in a professional manner.
  • Accompany service users to referral destinations, medical appointments etc.
  • Ensure client confidentiality as per agency policy.
  • Ensure ongoing and open communication with colleagues and supervisor to afford the best outcomes for clients.

Working Conditions and Environment

  • Hybrid role of working in the community, office and from home.
  • Commute between Peel and Halton to meet with clients in the community.
  • Maintain workplace confidentiality and work within the policies and procedures as outlined, while respecting the mission, vision and principles of the agency

  • Conduct self with integrity and respect as it applies to the role as an employee of the agency

  • Maintain a current understanding of resources/services available in the community

  • Ability to comply with deadlines as directed by management

  • Demonstrate effective time management skills

  • Participate in relevant training opportunities in order to keep skills and outlook current/effective

  • Provide supervision to student placements and/or volunteers as requested by manager

  • Represent the agency at community events, public speaking engagements, etc. at the direction of management.



Qualifications:

  • Formal education in relevant degree/diploma programs in Social Services/Human Services field required.
  • Minimum of 2 years experience in the social service field required.
  • Personal lived experience in the areas of homelessness and/or corrections and/or substance use and/or mental health.
  • Knowledge of local community resources across the city of Peel-Halton-Dufferin, especially in the area housing/homelessness/mental health and addiction.
  • Must display a non-judgmental and anti-oppressive attitude.
  • Familiarity and ability to support harm reduction approaches for people who use substances.
  • Excellent interpersonal and communication skills are required to work in a one-on-one context and within a team environment.
  • Strong personal/professional boundaries, crisis intervention, de-escalation and problem solving skills to intervene in conflict situations.
  • Valid drivers license and vehicle required.
