Role Description

This is a full-time on-site role for a Nurse Practitioner, located in Toronto, ON. The Nurse Practitioner will provide primary care healthcare services, perform patient assessments, diagnose and manage acute and chronic conditions, and prescribe medications and therapies. Responsibilities also include educating patients and families about health conditions and wellness practices, collaborating with physicians and allied health professionals, and maintaining accurate medical records.


The Nurse Practitioner will have their own roster of patients and will be responsible for independently managing their primary care. They will be integrated into our team in the same way as our primary care physicians, who collectively care for 10,000 patients in the community. The NP is expected to build and maintain a panel of approximately 600 patients within 12 months, and will hold full responsibility for the ongoing primary care of those patients. This is a independent position and would assume likely 30-40 patient visits per week split in person and virtual as required.


This role is for an NP who wants independence in their practice and care for their panel of patients while having support of experienced clinicians and specialists in our multidisciplinary setting.


Salary

The salary is $11,500 monthly plus 73% of all private pay services and OHIP billing (once initiated by the ministry).

  • Advanced clinical skills, including patient assessment, diagnosis, and management
  • Proficiency in prescribing medications and developing treatment plans
  • Strong communication and patient education skills
  • Ability to work collaboratively in a multi-disciplinary team
  • Excellent organizational and time-management skills
  • Master's degree in Nursing and current Nurse Practitioner licensure
  • Experience in primary care or specialized fields preferred

South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a wide range of multidisciplinary services, including primary care, health promotion, social services, community development, home support and advocacy to address the social determinants of health. We focus on improving the health and well-being of people in South-East Ottawa, especially those who face barriers to accessing care. Our Primary Care team is seeking several Nurse Practitioners to work as members of the interprofessional primary care team, which includes physicians, nurse practitioners, nurses, administrative staff, and allied health professionals. The NP operates as the Most Responsible Provider and will serve clients in collaboration with the interprofessional team, in walk-in and as rostered patients, provide coverage for clients of other providers when on leave, support same-day/urgent appointment requests, participate in interdisciplinary case reviews, and support clients in accessing services available across the centre.


Qualifications:

  • Baccalaureate in Nursing and Diploma/Master's of Nursing (Nurse Practitioner)
  • Registered with the College of Nurses of Ontario Extended class
  • Minimum two years experience as a nurse practitioner in a community or primary care setting; Experience in mental health is an asset
  • Strong clinical skills and diagnostic abilities: Performs medical examinations and evaluations, diagnoses, treatment, follow-up, consultation and health education
  • Previous experience working with culturally diverse and marginalized populations
  • Knowledge and training in trauma-informed care, psychotherapy modalities are an asset
  • Knowledge and understanding of the Community Health Centre model of health and well-being
  • Ability to work independently and be self-directed
  • Effective organizational, critical thinking, problem-solving and decision-making skills
  • Excellent communication skills and ability to adapt communication styles to meet the needs of clients, health team members and community partners
  • Fluency in English, French, and other languages is an asset
  • Proficiency in the use of computers and various software applications
  • Ability to work flexible hours, including occasional evenings and weekends
  • Police Record Check for the Vulnerable Sector required

The Nurse Practitioner (NP - MRP) will be the primary care provider to an individual panel of patients and function as a member of the broader primary care team providing accessible and essential primary health care services in the Thames Valley Family Health Team (TVFHT). The NP provides services to patients who include the individual, the family, and the community, throughout their life span with a strong emphasis on health promotion, patient education and disease prevention.

REQUIREMENTS:

  • Current registration with the College of Nurses of Ontario as a Nurse Practitioner is required.
  • Work to full scope including ability to prescribe controlled substances as authorized by CNO without restrictions
  • 2 years experience as a Nurse Practitioner preferred
  • Council of Ontario University Program in Nursing Primary Health Care Nurse Practitioner Program Certificate (or recognized equivalent)
  • Baccalaureate of Science in Nursing required, Master of Science in Nursing preferred
  • Current Membership with RNAO or NPAO and evidence of professional liability insurance
  • A solid understanding of the roles and responsibilities of MRP (most responsible practitioner)
  • Demonstrated experience in Community and/or Primary Care Setting
  • Availability to work extended hours where required, including after-hours clinic
  • Be available for follow up of critical value lab results after hours by telephone
  • Work is completed on site; this is not a virtual role.
  • Have reliable transportation to allow travel between sites as needed
  • Practice at full scope with Most Responsible Provider (MRP)

SKILLS/ABILITIES

  • Knowledge and proficiency in current, evidenced-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
  • Strong leadership, organization, research, evaluation, time management and interpersonal skills
  • Experience in the use of EMR, Microsoft Word, Excel and Outlook.
  • Desire and ability to update knowledge and skills through various means including technology-based opportunities, courses, workshops and conferences
  • Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
  • Excellent verbal and written communication skills
  • Exhibit an ability to be open and non-judgmental
  • Knowledge of the goals and structure of the organization
  • Experience and a commitment to leverage other Interprofessional Healthcare Providers (IHP) in the provision of care to panel of patients.

Hourly Rate: $110,436 - $134,678 (based on years of experience).
