Role Description

This is a full-time on-site role for a Nurse Practitioner, located in Toronto, ON. The Nurse Practitioner will provide primary care healthcare services, perform patient assessments, diagnose and manage acute and chronic conditions, and prescribe medications and therapies. Responsibilities also include educating patients and families about health conditions and wellness practices, collaborating with physicians and allied health professionals, and maintaining accurate medical records.


The Nurse Practitioner will have their own roster of patients and will be responsible for independently managing their primary care. They will be integrated into our team in the same way as our primary care physicians, who collectively care for 10,000 patients in the community. The NP is expected to build and maintain a panel of approximately 600 patients within 12 months, and will hold full responsibility for the ongoing primary care of those patients. This is a independent position and would assume likely 30-40 patient visits per week split in person and virtual as required.


This role is for an NP who wants independence in their practice and care for their panel of patients while having support of experienced clinicians and specialists in our multidisciplinary setting.


Salary

The salary is $11,500 monthly plus 73% of all private pay services and OHIP billing (once initiated by the ministry).


Qualifications

  • Advanced clinical skills, including patient assessment, diagnosis, and management
  • Proficiency in prescribing medications and developing treatment plans
  • Strong communication and patient education skills
  • Ability to work collaboratively in a multi-disciplinary team
  • Excellent organizational and time-management skills
  • Master's degree in Nursing and current Nurse Practitioner licensure

Coordinate and participate in clinical research studies ensuring that studies are carried out according to
protocol, contracted scope of work, good clinical practice, sponsor standard operating

Essential Functions
- Provides clinical research support to investigators to prepare for and execute assigned research studies

- Assists the investigators to safeguard the well-being of subjects and maintain standards required of
clinical research sites
- Maintains a safe environment in accordance with site policies
- Acts as an advocate for research subjects
- Addresses subject questions in a pro-active manner and take remedial action as required
- Assists and advise site staff in nursing practices and on the delivery of study care to subjects
- Reports any deviations from normal research practices to senior staff and implement agreed changes in
the study care program
- Assists investigator in verifying that research study objectives are met on time, within budget and
according to application protocol requirements, clinical research regulations and quality standards
- Assists in providing training to new investigator site staff members on study-specific topics and
requirements
- Maintains adherence to investigator site staff training requirements by auditing and maintaining training
records
- Provides guidance and supervision to lower level site staff to help provide that processes are carried out
in line with protocols and SOP's, where applicable
- Participates in study process reviews and enhancement efforts to support control of site unit budget,
development of the research facilities and site services and the culture towards a high performing research
study team
- Prepares for and attend study monitoring visits, study audits and regulatory inspections with clinical
research regulatory agencies
- Adheres to standard operating procedures (SOPs) and other directives throughout this process
- Assists research site with coverage planning related to staffing and scheduling for research projects

Key Areas of Accountability

VHA's Clinical Leads are required to do the following tasks, but not limited to:

Position Summary:

  • The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.
  • The NP lead clinical services would include ongoing assessments that will be documented in the plan of care as well as a review and an update of the plan of care for each resident at least twice per year.

  • The Nurse Practitioner functions within their expanded scope in a highly autonomous manner to use their expert skill to formulate clinical decisions and appropriate diagnosis and manage acute/chronic illness and wellness promotion for all residents. Care is conducted in the patient’s suite and SRRs in-clinic.


Education:

  • Master's Degree in Nursing, and successful completion of the Ontario Primary HealthCare or Adult Nurse Practitioner Certificate Program.
  • Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario).
  • Thorough understanding of the RN(EC) scope of practice and core competency framework.
  • Current CPR and First Aid Certificates.

We are seeking a Nurse Practitioner (Casual position)

What will be your role?

In this role you will work in a dynamic, fast-paced environment where you will achieve excellence in providing home care services to VHA clients. In addition, you will act as a subject-matter expert for new initiatives and projects related to home care, community clinic, digital health, and in fostering vendor partnerships. Reporting to the Business Development manager, your clinical leadership will be crucial in planning, implementation, evaluation and monitoring activities related to home care service delivery. You will collaborate with both internal and external partners to help shape business cases that bring innovative models of care to life.

In collaboration with key stakeholders, you will address quality, safety and risk management issues, including the development of improvement plans. Clinical work in the field will allow you to validate new workflows that directly impact clients and their caregivers.

The ideal candidate will bring experience in mostly adult and geriatric care, complemented by exceptional clinical, leadership and communication skills. Experience in change management or health-system innovation is a strong asset.


 

Key Areas of Accountability:

  • Familiar with the principles, practices, and methods of community nursing and service delivery to patients as well as program development, implementation and evaluation

Position Summary:
The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high-quality, client-centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.

Main Responsibilities/Duties:

  • Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.
  • Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.
  • Prescribe medications and therapeutic interventions within the NP scope of practice.
  • Manage and coordinate care for patients with chronic and complex health conditions.
  • Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals.

Education and Employment Requirements:

  • Successful completion of a post-graduate Primary Care Nurse Practitioner program.
  • Current Extended Class Registration with the College of Nurses of Ontario.
  • Proof of current membership or alternative professional liability coverage.
  • Current CPR certification (Level C or HCP).
  • Demonstrated experience in community and/or primary care.
  • Experience in chronic disease management and prevention.
  • Strong ability to work both independently and collaboratively within a healthcare team.
  • Excellent oral and written communication skills in a clinical setting.
  • Proficiency in the use of electronic medical records (EMR); Accuro experience is an asset.
  • Passion and experience in Refugee Health and Pediatric Care are considered strong assets.

Purpose

Reporting to the Clinical Director, the Nurse Practitioner (NP) provides care for unattached babies and children in the context of a broader pediatric team. NP provides and manages primary care for marginalized clients with varying degrees of social and medical complexities in accordance with current Canadian Community Health Nursing Standards of Practice. The NP's approach to client care is holistic and utilizes current client education and therapeutic techniques. The NP collaborates, consults, and refers to a multidisciplinary team (internally and externally) including RNs, Dietitian/Lactation Consultant and Family Support Worker. This individual is a natural leader who practices to the full scope of practice, remains informed on evidence-informed practices, has the willingness to participate in quality improvement and research initiatives, and has a constant desire to learn both as an individual and as a collective team. A passion for pediatrics, newcomer health and reducing other healthcare barriers.

Accountabilities and Responsibilities

Duties and responsibilities include, but are not limited to:

  • Provides client diagnosis, assessment, planning, implementation, and evaluation in person and virtually in accordance with the College of Nurses of Ontario standards of practice and under specific medical directives while maintaining and managing care for a full team roster.
  • Delivers advanced care to specialized populations
  • Responsible for collaborating with all team members in a respectful and professional manner, which includes consultation with family physicians when patient care requires assessment or treatment that is beyond the NP scope of practice or current level of expertise.
  • Maintain professional competencies and a professional license and liability insurance to practice (where applicable).
  • Act as a member of an interdisciplinary team that assists with the development, analysis, and implementation of patient programming and services.
  • Provide point of care in other community settings as identified by leadership.
  • Responsible for maintaining high-quality standards and timely documentation in the Electronic Medical Records (EMR).
  • Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
  • Participate in team and staff meetings and other meetings, and committee work as appropriate to support both service delivery and organizational goals.
  • Provide input for the development and implementation of NFCHC’s programming and services.
  • Participate in quality improvement activities as identified by the NFCHC.
  • Develop and maintain strong ties to other professionals and community members.
  • Work in a manner that demonstrates commitment to NFCHC’s mission, vision, and values.
  • Contributes to the CHC’s strategic goals and objectives and values the equality and diversity of the client community and NFCHC staff.
  • Participate in professional development opportunities as identified in the performance management process.

Education and Employment Requirements

  • A degree in nursing from a recognized college/university or a combination of a nursing diploma and additional relevant courses/training.
  • Current extended class registration with the College of Nurses of Ontario.
  • Registered and in good standing with the CNO.
  • Current professional liability insurance.
  • A minimum of 3 years of experience practicing in a clinical setting, community setting preferred.
  • A minimum of 3 years of experience in providing care to a pediatric population preferred.
  • Demonstrated experience in collaboration and working within an interdisciplinary team.
  • Demonstrated experience and highly proficient in Electronic Medical Records and use of virtual care tools including AI scribing.

Qualifications:

The qualifications needed to join our family are as follows:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
