- Assumes a teaching role for selected programs
- Collaborates with team members to develop creative strategies for learning
- Monitors and evaluates integration of learning into clinical practice for example: through documentation audits, review of use of learning management system by nursing teams.
- Partners with members of the management team to establish recommendations and plans for improvement and/or achievement of clinical excellence
- Currently registered and in good standing with the College of Nurses of Ontario
- B.Sc.N, Masters degree in Nursing (Msc) required
- Current registration with the College of Nurses of Ontario
- Minimum 5 years home care experience
- 3+ years’ experience in a nursing educator role required

- Must have a vehicle, G2/G license and be willing to travel between locations

Job DescriptionReporting to the Clinical Services Manager, you will be responsible to provide patient care and education within your scope of practice in a variety of acute medical, CCC, palliative and surgical patient care areas. You will be provided in advance with a six week working schedule.
Accountabilities:
- Float to units as determined by daily staffing needs, which includes reassignment of units during a shift at all Osler sites
- Maybe required to work temporary unit placements or project work assignments for either the medical or surgical units
- Current registration and in good standing with the College of Nurses of Ontario
- Able to function to the full scope of practice as per CNO
- Current CPR certificates required
- Fluent in English (reading, writing, speaking and comprehension)
- Recent experience within two years of working in acute medicine and/or surgery
- Flexibility, willingness and ability to float to acute medical, palliative, CCC and surgical units as required on a daily basis

Key Areas of Accountability
VHA's Clinical Leads are required to do the following tasks, but not limited to:

Required Qualifications:

RN diploma/degree from a recognized college or university
Aware of relevant legislation related to practice standards, quality and safety issues and human rights standards
Effective communication and interpersonal skills with the ability to communicate at all levels, both written and verbally

Position Summary:
- The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.
- The NP lead clinical services would include ongoing assessments that will be documented in the plan of care as well as a review and an update of the plan of care for each resident at least twice per year.
- The NP will complete a comprehensive geriatric assessment (CGA) for the management of care for frail seniors.
- The Nurse Practitioner will coordinate and work in collaboration with inter-professionals such as the other members of the care team, a social worker, an occupational therapist, a pharmacist, and a BSO Clinician to offer geriatric clinical integrated care to support frail older adults and seniors who live at SRR.
- The NP will provide direct patient care focusing on promoting health, maximizing patient safety, and function that will support frail seniors living in the retirement home community.
- The Nurse Practitioner functions within their expanded scope in a highly autonomous manner to use their expert skill to formulate clinical decisions and appropriate diagnosis and manage acute/chronic illness and wellness promotion for all residents. Care is conducted in the patient’s suite and SRRs in-clinic.
Education:
- Master's Degree in Nursing, and successful completion of the Ontario Primary HealthCare or Adult Nurse Practitioner Certificate Program.
- Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario).
- Thorough understanding of the RN(EC) scope of practice and core competency framework.
- Current CPR and First Aid Certificates.

Position Overview

- Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
- Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client's permanent health record
- Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals, or other services in the interest of the continuum of care
- Follow best practice guidelines
Qualifications:
- Graduation from a Nurse Practitioner program
- Current or willingness to obtain licensure as a Nurse Practitioner with the College of Registered Nurses of Newfoundland and Labrador
- Graduation from an approved School of Nursing
- Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
- Experience working in emergency or urgent care considered an asset
- Demonstrated knowledge of home and community health services
- Family All Ages (FAA) is required
- Experience practicing in a rural setting considered an asset

Job DescriptionThe Nurse Practitioner will demonstrate a high level of autonomy and expert skill in formulating clinical decisions and appropriately managing the care of eating disorder clinic patients who require assessment and ongoing monitoring of physical status. - Nursing experience in working with patients who meet DSM5 diagnostic criteria for an eating disorder is required

- Exemplary clinical nursing skills, knowledge and expertise, especially in patient assessments and consultation with family physicians
- Experience in the provision of psychoeducational and psychotherapy (individual and group) is preferred
- Successful completion of a Nurse Practitioner program from a recognized university

We are seeking an exceptional Nurse Practitioner (NP) to provide high quality, compassionate care to our patients and work collaboratively with our Cardiologists and other healthcare professionals in the management of complex cardiovascular cases to ensure the best patient experience out of any other medical clinic in Canada.
Responsibilities:
Working closely with the assigned Cardiologist, the principal responsibilities of the NP may include but are not limited to the following:
- Conduct comprehensive patient assessments, take medical histories, perform physical examinations, and interpret findings
- Order diagnostic tests and review incoming results
- Maintain accurate and up-to-date patient records using electronic medical records (EMR) systems
- Provide education and preventative care to patient
- Formulate clinical treatment plans
- Promptly follow up on incoming results
- Communicate effectively with patients, families, and interdisciplinary team members
- Administrative duties, including form completion, prescriptions renewals, and other administrative duties as deemed necessary for efficient clinic operation
- Perform other duties as assigned.
Education and Qualifications:
- Current and valid Nurse Practitioner license in Ontario
- Certification from an accredited Nurse Practitioner program
- Minimum three (3) or more years of experience as a Nurse Practitioner in a cardiovascular setting
- Demonstrative expertise in cardiovascular assessment, diagnosis and treatment
- Strong clinical, organizational, interpersonal and communication skills
- Ability to provide patient focused provision of service
- Ability to multi task within a fast paced environment
- Excellent time management skills, ability to work independently, professional and courteous
- Ability to use good judgement with excellent decision-making and problem-solving skills
- Ability to work collaboratively with all One Heart Care team members
- Ability to use the information technology provided
- Proficiency in Microsoft Office, EPIC and electronic medical records (EMR) systems. Working knowledge of EPIC and/or Cerebrum an asset.
