Position Overview
The Nurse Practitioner is responsible to provide appropriate assessment and care to patients in a virtual clinic environment. - Coordinates treatment plans and follow up based on appropriate assessments, client and family response and outcomes, collaborating with other healthcare professionals if needed
- Communicates findings and diagnosis to client and/or inter-professional team. Discusses prognosis, treatment options and outcomes with client and/or other team members within the circle of care and documents within the client's permanent health record
- Advocates for clients, assessing their needs and if applicable, referring to appropriate care providers, professionals, or other services in the interest of the continuum of care

Qualifications:
- Graduation from a Nurse Practitioner program
- Current or willingness to obtain licensure as a Nurse Practitioner with the College of Registered Nurses of Newfoundland and Labrador
- Graduation from an approved School of Nursing
- Preferred 1 year of recent experience as a practicing Nurse Practitioner preferred. Previous clinical experience will be considered
- Experience working in emergency or urgent care considered an asset
- Demonstrated knowledge of home and community health services

Position Summary:
- The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.

Summary

Reporting to the Director of the Women and Children’s Team, the Nurse Practitioner (NP) role in the Sickle Cell Disease program is responsible for providing clinical care to support both paediatric and adult patients at SHN. The NP will work in both the paediatric and adult Sickle Cell Disease clinics with the interprofessional teams to provide seamless care for patients with Sickle Cell Disease. The Humber River Health Oncology program provides comprehensive cancer care services in our outpatient clinic made of 28 treatment spaces and care for over 15,500 patients annually, including lab, systemic therapy, medical oncology, and supportive care visits. Our treatment and services include:
Chemotherapy, Immunotherapy, and Targeted therapies
Symptom management related to systemic treatments
Assessment and follow up from inter-professional cancer care team
Supportive care related to complex cancer pain and cancer-related symptom management
We are currently seeking an Oncology Nurse Practitioner (NP) to partner with another Oncology NP and work collaboratively with the interdisciplinary team, oncologists, palliative/supportive care physician and program leaders to provide direct and comprehensive care and services to our patients.
You will use advanced practice skills to provide assessment, coordination and provision of services to patients and families including crises intervention and short-term management while integrating best practice guidelines and current evidence-based research into clinical practice for in-patient and ambulatory clinic for oncology and oncology palliative population.
You will have the opportunity to participate and lead quality improvement initiatives and make recommendations for change in consultation with the Program Director, Manager and Physician Leader.
Your leadership skills will be utilized to provide training, coaching and education related to evidence-based Oncology practice and knowledge among team members.
Qualifications:
NP - Adult, RN(EC), Registration in good standing with the College of Nurses of Ontario
Minimum 3 years experience in acute oncology and systemic therapy experience
Masters degree in Nursing
Member of the Registered Nurses Association of Ontario, preferred
Oncology Nursing Certification (CNA), preferred
Knowledge of oncology, community resources, crisis work and a working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act

Responsibilities:
Working closely with the assigned Cardiologist, the principal responsibilities of the NP may include but are not limited to the following:
- Conduct comprehensive patient assessments, take medical histories, perform physical examinations, and interpret findings
- Order diagnostic tests and review incoming results
- Maintain accurate and up-to-date patient records using electronic medical records (EMR) systems
- Provide education and preventative care to patient
- Formulate clinical treatment plans
- Promptly follow up on incoming results
- Communicate effectively with patients, families, and interdisciplinary team members
- Administrative duties, including form completion, prescriptions renewals, and other administrative duties as deemed necessary for efficient clinic operation
- Perform other duties as assigned.
Education and Qualifications:
- Current and valid Nurse Practitioner license in Ontario
- Certification from an accredited Nurse Practitioner program
- Minimum three (3) or more years of experience as a Nurse Practitioner in a cardiovascular setting
- Demonstrative expertise in cardiovascular assessment, diagnosis and treatment
- Strong clinical, organizational, interpersonal and communication skills
- Ability to provide patient focused provision of service
- Ability to multi task within a fast paced environment
- Excellent time management skills, ability to work independently, professional and courteous
- Ability to use good judgement with excellent decision-making and problem-solving skills
- Ability to work collaboratively with all One Heart Care team members
- Ability to use the information technology provided
- Proficiency in Microsoft Office, EPIC and electronic medical records (EMR) systems. Working knowledge of EPIC and/or Cerebrum an asset.

Position Summary:
The Nurse Practitioner (NP) plays a critical role in providing comprehensive primary healthcare services, with a specialized focus on refugee health and pediatric care. The Nurse Practitioner will maintain and manage their own roster of patients, working within a multidisciplinary team to deliver high-quality, client-centered care. This role requires a commitment to providing accessible healthcare services and working collaboratively to improve health outcomes for vulnerable populations.
Main Responsibilities/Duties:
- Provide comprehensive primary healthcare services to individuals and families, with a focus on refugee and pediatric populations.
- Conduct health assessments, diagnose conditions, and develop treatment plans in accordance with best practices and clinical guidelines.
- Prescribe medications and therapeutic interventions within the NP scope of practice.
- Manage and coordinate care for patients with chronic and complex health conditions.
- Collaborate with an inter-professional healthcare team, including physicians, nurses, social workers, and community health professionals.
- Provide patient education, counseling, and advocacy to promote health and well-being.
- Support the development and implementation of health programs tailored to the needs of refugee and pediatric patients.
- Maintain accurate and timely electronic medical records (EMR) and documentation.
- Participate in quality improvement initiatives and professional development activities.
- Support and contribute to our company’s commitment to workplace equity, diversity, and inclusion.
- Successful completion of a post-graduate Primary Care Nurse Practitioner program.
- Current Extended Class Registration with the College of Nurses of Ontario.
- Proof of current membership or alternative professional liability coverage.
- Current CPR certification (Level C or HCP).
- Demonstrated experience in community and/or primary care.
- Experience in chronic disease management and prevention.
- Strong ability to work both independently and collaboratively within a healthcare team.
- Excellent oral and written communication skills in a clinical setting.
- Proficiency in the use of electronic medical records (EMR); Accuro experience is an asset.
- Passion and experience in Refugee Health and Pediatric Care are considered strong assets.
