Position Overview:
Join an innovative healthcare organization as a Nurse Practitioner in a cutting-edge transitional care facility located in West Bedford, Nova Scotia. This modern, purpose-built center provides short-term care to adult patients transitioning from acute care settings back to their homes or communities. It is not a hospital or long-term care facility.
This full-time role places Nurse Practitioners in a leadership position as the Most Responsible Health Care Provider (MRP), offering a rare opportunity to deliver evidence-based, patient-centered care in a highly collaborative and supportive environment.
What You'll Be Doing:
- Act as the primary healthcare provider for patients in transitional care.
- Independently assess, diagnose, prescribe, and treat within your legislated scope.
- Lead clinical teams on-site, providing direct supervision and clinical expertise.
- Collaborate with interdisciplinary teams including restorative care professionals, nurses, and allied health staff.
- Create care plans aligned with patient and family goals to help patients return home or to their communities.
Qualifications:
- Education: Master's Degree in Nursing with Nurse Practitioner Certification.
- Licensure: Eligible for registration with the Nova Scotia College of Nursing.
- Experience: 2-3 years as a Nurse Practitioner preferred (acute or rehabilitative care ideal).
- Practice Setting: Must be comfortable with hands-on care in an on-site setting.
- Note: Candidates with Pediatric NP backgrounds cannot be considered due to licensing limitations.

Work Schedule & Setting:
- Schedule: Monday to Friday, 08:00-16:00

- On-Call: No on-call requirement at this time
- Overtime: Paid as per the NSNU Collective Agreement

The Immunization Nurse will review each client’s medical history and discuss the Centre for Disease Control & Prevention requirements and/or recommendations for safe travel to countries such as Kenya, India, Peru, Thailand and more!
Job Duties
Utilize proper travel medicine and general medicine processes to provide immunizations and travel health counseling in a safe and effective manner.
Provide friendly, efficient, and safe service to patients and clients.
Diligently participate in the company’s training to ensure up to date knowledge of travel medicine and vaccination.
Adhere to safety protocols as set by the company’s medical directives and regulations.
Demonstrate outstanding customer service and communication skills.
Practice appropriate storage and handling of vaccines, includes but not limited to carrying and lifting vaccine boxes.
Accepting delivered packages and ensure all inventory is accounted for.
Carrying and organizing clinic supplies.
Ensures the clinic and storage room is clean and well organized.
May be responsible for meeting and unpacking packages, vaccines, furniture and fixtures during clinic set up.
Responsible for organization and maintenance of clinic space to facilitate smooth client flow.
Collaborate with the Nurse Management Team to train new staff.

Qualifications and Requirements
Previous experience working in travel Medicine, either in a private clinic or Public Health setting is an asset.
Must be willing to travel to the clinic location in Halifax.
Ability to follow Medical Directives in order to safely administer vaccines and dispense medications to comply with Passport Health Standard of Care.
Registered Nurse certification with current license in good standing.
Must have excellent computer skills.
First Aid/CPR certified.
Must successfully pass background check.

The OpportunityReporting to the Manager, Ambulatory Surgical, the Registered Nurse works collaboratively with the Heart Centre team to provide comprehensive family centred care to children and their families within the IWK. Responsibilities include but are not limited to: accurately assessing patient/family needs during clinic visits and telephone consultations; developing an individualized plan of care and continuously evaluating the plan of care making revisions as necessary, including the planning and coordination for follow-up care; liaising with the inpatient unit in the coordination of care for the patient population; participating in cross coverage of the electrocardiogram service, ambulatory cardiology clinic, cardiac catheterization lab and/or stress lab, as needed, as well as participating in "on call" schedule for emergency procedures; acting as a clinical resource regarding patient care issues; participating in team specific, program, and Health Centre committees (E.g., Care Team and Clinic Meetings, Quality Committees, Accreditation Team, etc.); facilitating decision-making by promoting collaboration and cooperation amongst team members; participating and implementing quality initiatives with the team including the development and maintenance of policies, protocols, standards, guidelines, and educational materials for patients and families; participating in and promoting evidence based practice; in collaboration with the interprofessional team members, and Clinical Leader of the Children's Heart Centre, participates in identifying issues that have potential or actual impact on clinical care; supporting educational needs for a variety of learners in clinic setting; and overseeing the smooth and efficient running of the Children's Heart Centre.

The OpportunityReporting to the program Manager, the Resource Nurse is responsible and accountable for the application of current theory and research in the delivery of family-centred patient care consistent with the philosophy, objectives, vision and mission of IWK Health. The RN's in the nursing resource pool will support four inpatient units: the Neonatal Intensive Care Unit (NICU), the Prenatal Special Care Unit (PSCU), the Family Newborn Care Unit (FNCU) and the Birth Unit.
The RN practices according to the Canadian Nurses Association Code of Ethics for Registered Nurses, the Nova Scotia College of Nursing Standards of Nursing Practice, specialty standards as applicable and the policies and procedures of IWK Health.
Hours of Work11.25 hours rotating shifts and Monday to Friday, weekends, days and nights.
Your Qualifications- Bachelor's Degree in Nursing required (candidates with equivalent experience and education will be considered).
- Licensed, or eligible for licensure, by the Nova Scotia College of Nursing (NSCN) required.
- Minimum two (2) years' experience (within the last five (5) years) in Maternal Newborn or Neonatal Nursing preferred.
- Critical care nursing experience an asset.
- Current successful completion of a Heart and Stroke Foundation of Canada Basic Life Support Level (BLS) Program within the last 12 months, or successful completion of a BLS Renewal Challenge Certificate, required.
- Preference will be given to candidates with NRP and IBCLC Certification.
- Preference will be given to candidates who have Fetal Health Surveillance course.
- Completion of Bereavement Course, or willingness to obtain, preferred.
