SUMMARY

The Nurse Practitioner (NP) provides direct and indirect comprehensive patient care services to populations with addictions or concurrent disorders using advanced assessment skills and a sound knowledge of clinical therapeutics. The services to support patient care are provided independently as well as in collaboration with various inter-professional teams across the organization and with community partners.

The NP will deliver nursing care services within their scope of practice as regulated by the College of Nurses of Ontario.
The NP is responsible for ensuring that nursing care provided to clients meets all legislative requirements and practice standards of the College of Nurses of Ontario (CNO).


ESSENTIAL DUTIES AND RESPONSIBILITIES


General Nurse Practitioner Duties and Responsibilities:

  • Provide comprehensive primary care within the NP scope, including assessments, diagnosis, treatment, and referrals for individuals with substance use and mental health concerns.
  • Develop and implement personalized treatment plans, including medication management, withdrawal support, and harm reduction strategies.
  • Conduct physical exams, order and interpret tests, prescribe medications, and educate clients and families on treatment and recovery.
  • Collaborate with physicians, specialists, and community agencies to coordinate care and support wrap-around services.
  • Offer supportive counseling, crisis management, and referrals to internal and external programs.
  • Provide services in street-level community settings, through home visits, and occasionally alongside community outreach programming accessing homeless individuals in shelters or encampments, and to support in reducing barriers to care.
  • Maintain accurate records and ensure compliance with professional and organizational standards.
  • Participate in case conferences, professional development, and interdisciplinary team meetings.

Withdrawal management Duties and Responsibilities:

  • Enhances community-based harm reduction supports for clients in the community experiencing serious substance use issues that lead to significant withdrawal symptoms.
  • Deliver clinical and nursing care in supportive housing and community settings, ensuring access to primary care and withdrawal medications.
  • Assess and manage co-occurring conditions, including mental health disorders, blood borne diseases and infections.
  • Utilize evidence-based interventions, including medication-assisted treatment and harm reduction strategies.
  • Engage clients in strengths-based, trauma-informed care while addressing social determinants of health.
  • Educate individuals and families on addiction, recovery, and stigma reduction.
  • Collaborate with interdisciplinary teams, specialists, and community resources to enhance patient care.
  • Provide clinical guidance on addiction-related issues to healthcare professionals and community groups.
  • Advocate for individuals facing substance use challenges.
  • Prescribe and manage controlled substances, ensuring risk mitigation and compliance with medical cannabis regulations.


EDUCATION

  • Master's Degree in Primary Health Nurse Care Practitioner program.
  • Successful completion of the Prescribing Narcotics and Controlled Substances course required.
  • Completion of a Suboxone on-line education program as asset.


LICENSES, CERTIFICATIONS AND CREDENTIALS

  • Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
  • Membership in specialty related organizations e.g. NPAO, RNAO an asset
  • Non-violent crisis intervention, First Aid, CPR and ASIST certifications are assets
  • Current vulnerable sector check
  • Valid drivers license and access to a reliable vehicle


RELATED WORK EXPERIENCE

  • Minimum of three years of community health care and/or primary care experience required
  • Experience in addiction medicine and harm reduction


REQUIRED SKILLS AND KNOWLEDGE

General Nurse Practitioner competencies:

  • Extensive knowledge and understanding of harm reduction principles and practices, substance use and overdose response.
  • Knowledge of evidence-informed guidelines, legislation, regulation and organizational policies when prescribing controlled drugs/ substances (CDS).
  • Understanding of best practices related to the use of CDS and of other pharmacological and non-pharmacological therapeutic options.
  • Commitment to diversity, equity, and accessibility in patient care.

Withdrawal Management Competencies:

  • In-depth understanding of addiction neurobiology.
  • Advanced assessment and intervention skills for addiction treatment.
  • Ability to evaluate emotional, cognitive, and behavioral states, crisis levels, and substance withdrawal symptoms.


WORK CONDITIONS

  • Community: public places, community partner sites and clients homes and/ or ADAPT busy office environment.
  • Able to fulfill all duties and work hours from a remote/home location, whenever required.
  • Able to work 37.5hrs/week; flexibility is required when meeting clients needs in evenings and on occasional weekends.
  • Manual dexterity required to use desktop computer and peripherals.
  • Various physical actions of mobility are required (minimal lifting, sitting for periods of time, moderate wrist motions).

In this position, you will engage in collaborative work within the community, with the Mobile Community-Based Rapid Access Addictions Medicine program. This team will be focused on connecting with vulnerable populations accessing Good Shepherd and other community programs focused on serving those experiencing homelessness and precarious housing.

The Nurse Practitioner will be working with their NP colleague providing on-the-spot care to clients, collaborating with their circles of care, and improving overall health outcomes of marginalized individuals. This program allows for independent work, flexibility in care provision, and an independent environment for the NPs.

How You Will Make an Impact

  • To work within the full scope of the RN (EC) practice to provide primary care to all individuals served by IHT and HoW and other appropriate social services in accordance with the College of Nurses of Ontario's Expectations for Consultations with Physicians by Registered Nurses in the Extended Class.
  • Provide assessment, brief intervention, and motivational interviewing for clients presenting with substance use disorders, especially opiate specific concerns.
  • Participate in intensive outreach and case finding for OAT delivery, particularly for women and other populations who face barriers accessing supports for substance use disorders
  • Establish and enhance partnerships with local agencies serving women and other populations facing multiple barriers
  • Provide same-day follow-up to referrals, prescription, and dispensing of OAT and other addictions medicine
  • Encourage, schedule, and decrease barriers for follow-up appointments
  • Provide harm reduction supplies and education Assess the need for orders and interpret appropriate diagnostic, screening and lab tests and reports
  • Formulate diagnoses and communicate and interpret these to clients and any supports (e.g. other service providers, family)
  • Treat illnesses, within the scope of practice, through the provision of pharmacological and complimentary therapies;
  • Provide education, information and support to individuals that supports health and wellness (e.g. explain need for medication compliance and the negative consequences associated with substance use; identify and support the adoption of health lifestyle patterns);
  • Provide information to individuals about basic health and wellness issues (e.g. immunization, nutrition, healthy parenting, hygiene, health maintenance);
  • Utilize a holistic approach and a client centered approach to care, paying special attention to psychological, social, cultural and environmental factors;
  • Triage, communicate and collaborate with hospitals and other community health services (e.g. CCAC, hospitals, specialists, and other family physicians);
  • Assist in linking clients with community primary care physicians as necessary.
  • Observe all health and safety rules and work in compliance with the provisions of the occupational Health and Safety Act and its regulations

POSITION SUMMARY:
The Program Assistant – Visiting Nursing (PA-VN) is an administrative resource providing general clerical and admin support to the Visiting Nursing program.  The PA-VN is responsible for data entry, collection of data, running reports, internal customer service, and other administrative tasks.  The PA-VN is also responsible for the scheduling of the clinical field staff, assisting with payroll processing, and being the first point of contact for clinical field staff and patient inquiries.   Additional responsibilities may include organization of events, ordering equipment / supplies, or participating on committees.

  • Transferring information from various sources into computer software (e. g. Procura, Excel)
  • Using software to run reports and generate information as requested
  • Preparing letters, notices, and other general documents for use in the Program
  • Entering data into pre-established templates, spreadsheets and other forms
  • Generating new reports, summaries, lists and forms as needed
  • Coordinating program scheduling of field staff hours
  • Fielding clinical staff calls regarding scheduling, relaying scheduling information to Supervisor or Sr. Manager
  • Performing the data entry function as related to payroll information at the program level
  • Collaborating with the Sr. Manager and/or the Payroll Coordinator in the accurate and efficient processing of payroll related information
  • Performing general reception and phone duties within the program
  • Assisting with program documentation, file management, mail and/or deliveries
  • Working with team members to find continuous improvement initiatives and actions, making suggestions and offering team input
  • Assisting other staff members in the execution of their duties as required
  • Assisting with various program audits and surveys (hand hygiene audits, client satisfaction survey etc.)
  • Troubleshoot patient and staff issues as it relates to scheduling as needed
  • Ordering program supplies, as directed by the program Manager or Supervisor
  • Participating in program discussions, working groups, committees or other collaborative activities

Unit Summary:

Metrics confirm that our Emergency Department is one of the top performing hospitals in the province. The Emergency Department is a 42-bed unit that serves the community of Burlington and the surrounding area. Last year, we had approximately 50,000 visits and our volume continues to grow. Our Emergency serves patients of all ages and treats patients of every medical and mental health illness. We have several different areas in our department that provide care specific to the needs of the patients in that area. Our PES unit (Psychiatric Emergency Services) is a unique 4-bed mental health unit in our department. Our dedicated multi-disciplinary team collaborate to provide safe, high quality care to our patient population. This ambitious team delivers care reflecting our values of compassion, accountability, respect and excellence. The unit is supported by 24 hour Charge Nurses, a Professional Practice Educator and a Unit Manager. Qualifications:

  • Current Certificate of Registration with the College of Nurses of Ontario as a Registered Nurse.
  • Must have current or recent clinical experience of at least two years in the Emergency, I.C.U. or step down units. Preference will be given to candidates with current Emergency nursing experience.
  • Current Basic Life Support (BLS) certification required; Advanced Cardiac Life Support (ACLS) and Pediatric Advanced Life Support (PALS) certifications preferred.
  • Must complete Coronary Care 1 within 1 year of start date.
  • Demonstrated the ability to competently assess the patient, intervene appropriately.
  • Demonstrated ability to function effectively in emergency situations.
  • Demonstrated clinical assessment and patient/family teaching skills.
  • Demonstrated ability to function effectively in a multidisciplinary team.
  • Demonstrated effective documentation skills.
  • Demonstrated excellent communication skills.
  • Demonstrated a caring, compassionate, customer focused attitude toward patients and family.

  • Demonstrated skill and knowledge of I.V. Therapy and venipuncture is required.
  • Demonstrate the knowledge, experience and ability to provide care for patients with a variety of needs including but not limited to : initiation and maintenance of IV's, venipuncture, administration of medication delivered via various routes, blood and blood product administration, patient controlled analgesia, capillary blood glucose monitoring, NG tubes, chest tubes, enteral feeds, wound management, oxygen therapy.
  • Delegated Controlled Acts certification to be successfully completed within six months of employment date and to be recertified annually.
  • Demonstrated reliable attendance, punctuality, availability, and performance is required.

Registered Nurse (RN) reports to the Director of Care and is expected to coordinate and oversee all aspects of resident focused delivery of care. Registered Nurse should also promote the values, goals, and philosophy of the Home.

Responsibilities:

  • Development and modification of the care plans in response to residents’ individualized health care needs and change in health care status
  • Liaises with other health care personnel, agencies and families
  • Maintains good relationships with team members, residents, families and general public
  • Engages in many meaningful interactions with residents during your entire shift
  • Supervises all activities on the unit, takes charge as required
  • Monitors performance and completes performance evaluation for the PSW’s and RPN’s
  • Directs, assigns and evaluates the work of the PSW’s and RPN’s
  • May be in charge of Home as required. Utilizes leadership skills to identify problems and problem- solving on the unit
  • Monitors that care is delivered is consistent with the resident Bill of Rights.
  • Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
  • Required to participate in the improvement of the indicators and achieve satisfactory results.
  • Actively listens to residents and family concerns, take appropriate actions to resolve the issue in accordance with policies of the Home
  • Conducts unit rounds at the beginning and end of shift.
  • Processes Physician orders and administers medications and treatments in compliance with the policies of Pharmacy provider, the Home and the Standard of the College of Nurses. Documents and reports all errors and omissions
  • Maintains accurate and complete records of nursing observations and care in compliance with the legislative requirements and the policies of the Home
  • Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) or POA, and interdisciplinary team
  • Completes health care assessments (RAI-MDS and other assessments), develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team.
  • Demonstrate understanding of the Ministry of Long-term Care mandatory reporting and critical incidents and follow steps outlined in the policies of the Home.
  • Inspires others to be motivated, work with passion, and empower team members.
  • Any and all other tasks as assigned.

Qualifications:

The qualifications needed to join our family are as follows:

  • Current Certificate of Competence with the College of Nurses of Ontario as Registered Nurse or a Registered Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Basic CPR
  • Experience in long term care setting an asset
  • Ability to work with and relate to interdisciplinary team members

  • Well-developed written and oral communication skills
  • Knowledge of RAI-MDS – AIS certified, if not must be completed successfully within one (1) year of being hired.
