Sous l’autorité de la Directrice des Services de santé primaire, l’infirmière praticienne/infirmier praticien (IP) fournit des soins de santé primaire axés principalement sur la promotion de la santé et la prévention des maladies conformément aux pratiques et aux normes en matière de soins énoncés dans les Normes d’exercice à l’intention des infirmières et infirmiers autorisés de la catégorie spécialisée de l’Ontario. Elle/il détermine et gère les problèmes de santé courants et les maladies indépendamment ou en collaboration avec le médecin. Elle/il travaille au sein d’une équipe interdisciplinaire pour traiter de questions qui touchent la santé des membres de la collectivité.
Responsabilités principales :
Fournir des soins de santé primaire globaux aux clients de tous les âges selon le champ de pratique de l’IP
Déterminer les problèmes de santé des clients selon les facteurs de risque
Faire participer les clients à la détermination des facteurs de risque et des problèmes de santé, à
l’établissement d’objectifs et aux choix des interventions
Prescrire, interpréter et assurer le suivi des examens de dépistages et diagnostiques si nécessaires
Tenir des dossiers complets, précis et à jour sur la clientèle
Rencontrer les clients au Centre et contribuer à la coordination de cas
Traiter les clients souffrant de maladies chroniques stables et en assurer la gestion et le suivi selon les directives du médecin
Référer aux programmes de santé et autres services offerts par le Centre
Participer au contrôle des substances réglementées conformément aux politiques et procédures en vigueur de Santé Canada
Participer à l’élaboration et à l’évaluation des politiques, procédures, pratiques d’excellence et protocoles
Recueillir et traiter les renseignements statistiques requis par le Centre et les bailleurs de fonds
Participer aux rencontres d’équipe et du personnel, siéger sur des comités internes et/ou externes
Formation/expérience/exigences :
Baccalauréat en sciences infirmières et certificat d’IP obtenus dans une université reconnue
Permis de pratique de la catégorie spécialisée de l’OIIO en règle
Expérience en santé mentale et en toxicomanie serait un atout
Connaissance approfondie et à jour des méthodes d’examen physique et de traitement
Bon jugement, capacité de prendre des décisions, travail autonome et au sein d’une équipe multidisciplinaire
Parler et écrire couramment le français et l’anglais
Très bonne connaissance des logiciels informatiques de base
Être en mesure de travailler des heures flexibles
Détenir un permis de conduire valide et avoir accès à un véhicule automobile

Under the direction of Primary Care Services Director, the Nurse Practitioner NP) will provide primary health care focused mainly on health promotion and disease prevention in accordance with the practices and standards of care set out in the Standards of Practice for registered nurses in the Ontario Specialist Category. She / he will determine and manage common health problems and diseases independently or in collaboration with the doctor. She / he will work in an interdisciplinary team to address issues affecting the health of community members.
Primary Duties and Responsibilities
Provide comprehensive primary health care to clients of all ages within the scope of practice of the NP
Identify client health issues by risk factors
Involve clients in identifying risk factors and health problems, setting goals, and choosing interventions
Prescribe, interpret and monitor screening and diagnostic examinations if necessary
Keep complete, accurate and up‐to‐date client records
Meet with clients at the CSC and contribute to case coordination
Treat clients with stable chronic conditions, manage and follow them as directed by the physician
Refer clients to the health programs and other services offered by the CSC
Participate in the monitoring process of controlled substances in accordance with Health Canada's policies and procedures in effect
Participate in the development and evaluation of policies, procedures, best practices and protocols

Education, Experience and Specific Requirements:
Bachelor of Science in Nursing and NP Certificate from a recognized university
Practice Permit for the Specialty Class of the CNO
Experience in dealing with mental health and addictions issues would be an asset
In‐depth and up‐to‐date knowledge of physical examination and treatment methods
Good judgment, decision‐making, and interpersonal skills,
Excellent ability to work independently and within a multidisciplinary team
Speak and write fluently French and English
Very good knowledge of basic computer software
Be willing to work flexible hours to meet client needs
Have access to a motor vehicle and hold a valid Ontario driver's license
Provide a criminal record check including the vulnerable sector check
A valid first aid and CPR certificate would be an asset

We are currently seeking a registered X-Ray technologist to work in our Burlington and Hamilton clinics. This role can be full-time, part-time or a casual relief position. The successful candidates will be motivated, quality driven and possess excellent patient care skills. This position will be in General X-Ray but BMD and/or mammography experience/training is an asset and such duties will be reflected in the wage rate. We are offering a $3 000.00 signing bonus for all full-time hires and 1,500.00 for all part-time hires. You will enjoy working in a fast paced, quality focused clinic with other like minded staff. Compensation will be based on skill set and experience.
Requirements
* Graduate of approved training program in Radiography
* Current registration and in good standing with the College of Medical Radiation Technologists of Ontario (CMRITO)
* Must have the knowledge, skills and judgment as identified in the CMRITO "Essential Competencies" to perform the services and procedures that fall within the scope of practice of the profession
