RN Responsibilities and Duties:
- Must be willing to travel to 2 of the following communities for 2 hours shifts: Coquitlam, Port Coquitlam, Pitt Meadows, Maple Ridge, and Mission.
- At least 2 days per week of availability totalling 10hrs per week availability to work
- Execute on the client’s care plan as outlined by the Care Designer, including all nurses duties plus personal care.
- Complete records and documentation accurately and thoroughly
- Provide delegations of tasks or other training to care team
- Assess clients needs and report back to Care Designer with recommendation or complex care plans
- Function within the limits
Requirements
- Class 5 Drivers Licence & reliable transportation.
- Double COVID Vaccination required
- Current RN licence in BC
- TB test within 2 years
- Current First Aid with CPR
- Excellent written and verbal English skills. Other languages an asset.
- Technically savvy with mobile apps and G-suite
- Previous home care nursing experience an asset.
- 2 Professional References
- Wound or palliative care experience is required
- VACC dressing experience is preferred

Job Summary

We are currently looking for interested Licensed Practical Nurses who are interested in pursuing Specialty Nursing Education in Hemodialysis (Renal).
Fraser Health sponsorship is 100% tuition and wages. Upon successful completion, you will be offered either a regular full-time or part-time position (minimum 0.50 FTE) in an existing vacancy in the Specialty area. Positions will depend on organizational and patient care needs as well as academic institution course availability.
Who can apply? Fraser Health LPN employees and external LPN applicants who meet the following criteria:
Current active practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM)
Current CPR certification
Preferred minimum 1-2 years acute care nursing experience
For current FHA employees, 1 reference will be obtained from your current manager by Talent Acquisition & Onboarding
For external applicants, two references will be collected by Talent Acquisition & Onboarding
Commit to a comprehensive education program
A commitment to a minimum of 18 month regular full-time or part-time FH employment in the specialty area upon successful completion of the program

Detailed Overview
Under the direction of the Manager, Clinical Operations or designate and in collaboration as a member of a multidisciplinary team; performs assessments, plans, implements and evaluates nursing care. The LPN operates in accordance with the competency guidelines and full scope of practice within the Standards of Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM), and according to agency operating policies and standards and unit specific protocols.
Responsibilities
Assesses patient/resident/family's actual and potential strengths and limitations including physiological, psychological, socio-cultural, and spiritual needs.
Provides a supportive environment for patient/resident/family. Promotes patient/resident/family involvement in health care decisions; provides information and opportunity to discuss concerns; respects personal choices.
Contributes to the development and ongoing changes to the individualized plan of care, including attending and providing input into multidisciplinary care planning meetings and family conferences.
Provides nursing care according to identified policies and procedures, technology and FHA standards and evaluates outcomes. Records observations and reports problems and/or changes to designated staff.
Performs admissions, discharges, and transfers of patients/residents according to agency operating policies and procedures.
Documents and updates all relevant care and procedure information to ensure patient/resident safety and comfort.
Operates and maintains effective functioning equipment related to patient/resident care such as bathtubs, scales and mechanical lifts.
Assists in maintaining a safe and clean environment for patients/residents and staff.
Assists in the orientation of new staff, students and other members of the Health Care Team.
Participates in continuous quality improvement and program evaluation activities, education sessions, team conferences, meetings and committees.

Qualifications
Education and Experience
Graduate of a recognized practical nursing program or equivalent combination of education, training and experience. Current practicing registration as a Licensed Practical Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Skills and Abilities
Ability to communicate effectively, both verbally and in writing.
Ability to deal with others effectively.
Ability to organize work and implement a plan of care.
Knowledge and competencies to work at full scope of practice as currently required by the BCCNM, including administering medications.

Vancouver Community Adult Mental Health and Substance Use (MHSU) Team:
The Vancouver Comunity MHSU Teams support adults 19 years of age and older with moderate to severe mental health, substance use, and concurrent mental health and substance use disorders or conditions. This program is to be neighbourhood-based and works in an interdisciplinary team to encourage collaboration and partnership with local community services, including family physicians, and other care providers.
This role is a mix of on-site and outreach. Supports adults 19 years of age and older.

As a Mental Health and Substance Nurse, you will:
Provide quality, holistic nursing care for clients including those with moderate to severe mental health conditions and/or substance use disorder along the continuum of care services, effectively applying the nursing process to identify and mitigate risks to client health and promoting safety and wellness through collaborative, client-centred, strengths-based, interdisciplinary care planning.
Qualifications Education & Experience
Current practicing registration and authorization to practice as Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
Two (2) years recent experience providing nursing care for individuals with mental health conditions and/or substance use disorders, or an equivalent combination of training, education, and experience.

Knowledge, Skills & Abilities
Demonstrated knowledge of mental health and substance use theory and practice including therapeutic communication, crisis intervention and management, psychopharmacology and pharmacology, interdisciplinary care coordination and case management, individual and group counseling, key VCH and non-VCH community resources, and the nursing process.
Demonstrated knowledge of frameworks guiding nursing practice including client and family-oriented care, healthy equity, and psychosocial rehabilitation,
Demonstrated knowledge of Trauma and Violence Informed Practice, harm reduction approaches, recovery oriented and strengths-based care.
Demonstrated ability to promote Indigenous cultural safety.
Demonstrated ability to provide competent and culturally safe care in a variety of settings including with diverse populations.
Knowledge of relevant legislation, regulatory, policy and decision support tools and professional and practice standards contextualizing and enabling care in this setting (i.e. Mental Health Act, Adult Guardianship Act, BCCNM Standards).
Demonstrated skill in clinical techniques such as motivational interviewing, cognitive behavioural therapy, or individual supportive counselling, group facilitation or psychoeducation.
Demonstrated skill in the application of the nursing process including appropriate screening, assessment, risk identification, care planning, intervention(s), and evaluation.
Demonstrated skill in the prudent selection and application of standards, assessments and measures defined by the organization for the service/practice setting.
Demonstrated ability to identify and mitigate key risks (e.g. overdose risk/prevention, psychosis, suicide risk, medication side effects) through evidence-informed nursing practice, interdisciplinary care coordination, client- and family-centred care planning and case management.
Demonstrated ability to worth with clients and families using a variety of care modalities including but not limited to in-person, clinic, virtual and outreach visits.
Demonstrated ability to work independently and collaboratively in an interdisciplinary team environment with diverse people through effective verbal and written communication, consultation, problem-solving, facilitation, conflict mediation and resolution.
Demonstrated ability to adapt to unexpected and challenging events, and reconsider workload priorities.

Qualifications Education & Experience
Grade 12, medical terminology and graduation from a recognized Nursing Unit Clerk program plus one year’s recent, related experience or an equivalent combination of education, training and experience.

Ability to deal with others effectively.

What you will be doing:
Conduct comprehensive health assessments of patients presenting at the clinic.
Diagnose and treat a variety of acute and chronic medical conditions.
Develop and implement personalized treatment plans, including prescribing medications and therapies.
Administer vaccinations and provide preventive healthcare services.
Offer health promotion and disease prevention guidance to patients.

What You Need to Have:

Active, unrestricted provincial license(s) with prescribing privileges.
CPR certification.
