Position Summary:

  • The Nurse Practitioner (NP) is responsible for leading the provision of comprehensive geriatric services as part of a collaborating member of SRR’s inter-professional Care Team.
  • The NP lead clinical services would include ongoing assessments that will be documented in the plan of care as well as a review and an update of the plan of care for each resident at least twice per year.
  • The NP will complete a comprehensive geriatric assessment (CGA) for the management of care for frail seniors.
  • The Nurse Practitioner will coordinate and work in collaboration with inter-professionals such as the other members of the care team, a social worker, an occupational therapist, a pharmacist, and a BSO Clinician to offer geriatric clinical integrated care to support frail older adults and seniors who live at SRR.
  • The NP will provide direct patient care focusing on promoting health, maximizing patient safety, and function that will support frail seniors living in the retirement home community.
  • The Nurse Practitioner functions within their expanded scope in a highly autonomous manner to use their expert skill to formulate clinical decisions and appropriate diagnosis and manage acute/chronic illness and wellness promotion for all residents. Care is conducted in the patient’s suite and SRRs in-clinic.


Education:

  • Master's Degree in Nursing, and successful completion of the Ontario Primary HealthCare or Adult Nurse Practitioner Certificate Program.
  • Membership in good standing with a regulatory body in Ontario (College of Nurses of Ontario).
  • Thorough understanding of the RN(EC) scope of practice and core competency framework.
  • Current CPR and First Aid Certificates.
  • Canadian Certificate in Gerontology an asset, with three-five (3- 5) experience working with older adults and frail seniors.
  • Desired: Training in P.I.E.C.E.S.TM, Gentle Persuasive Approach, U-First!, Montessori, an asset


Experience and Skills:

  • Preferred 1-3 years of experience working with older adults and frail seniors.

Qualifications:

The qualifications needed to join our family are as follows:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Comprehensive knowledge of evidence based clinical practices
  • Comprehensive knowledge of long-term care concepts and philosophies
  • Excellent interpersonal and communication skills
  • Post-graduate education and experience in gerontology (recommended)
  • Specialty certification in gerontological nursing (recommended)
  • Experience working as a Nurse Practitioner,

  • Experience providing leadership and supervision in any healthcare environment.
  • Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
  • To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction.
  • To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one

Autonomously diagnose, order and interpret diagnostic tests and prescribe pharmaceuticals within scope of practice as it relates to the Bariatric Program; includes micronutrient supplementation
Update policies, procedures and Standards of Care for the Bariatric Program
Ability to support patients by providing clinical expertise, guidance and education
Assist in the coordination of activity between the surgical and medical clinics
Work collaboratively with the interdisciplinary team and with the physicians
Communicate abnormal findings with the team
Conducts health assessment and focused physical exam on high-risk patients; identifies normal and abnormal findings through investigation and liaises with PCP as needed and provides appropriate referrals
Facilitate the transition of patients post-operatively over to their Primary Care Providers
Facilitate team rounds and assist in the provision of comprehensive medical care for the bariatric patient
Identifies researchable questions and participates in research initiatives
Providing education on matters related to bariatric surgery, weight loss and health promotion
Formulates differential diagnoses, orders and/or performs screening and diagnostic investigation based on presenting health concern and treating using evidence based best practices and triaging post-operative for complications warranting ED visit and liaising with surgeons and internist as needed. This includes observing for post-bariatric surgery complications and treating appropriately
Prevents unnecessary ED visits
Ability to incorporate knowledge of diversity, cultural safety and the determinants of health in the assessment, diagnosis and therapeutic management of patients and the evaluation of outcomes
Identifies educational programs for the care team.
Ability to build and maintain effective relationships and partnerships with a diverse range of stakeholders as well as work effectively within multidisciplinary teams
Ability to work days, evenings, and weekends as required

Qualifications:
Current registration with the College of Nurses of Ontario as a Registered Nurse in the extended Class (Primary Health Care or Adult) and entitled to practice with no restrictions.
Member of the Registered Nurses Association of Ontario preferred
Minimum 2 years experience as an NP in primary care, long-term care, acute care or continuing care.
CDE and CBE certification preferred
Knowledge of Bariatric, community resources, crisis work and a working knowledge of the Regulated Health Profession Act, Mental Health Act, Health Care Consent Act and Substitute Decisions Act.
Demonstrated knowledge of the principles of adult education
Experienced in Program development and evaluation and analysis of statistics
Demonstrated experience and proficiency in the provision of effective clinical consultation and assessment.
Strong interpersonal communication skills and ability to establish positive relationships with all staff, clients, families, and community agencies
Excellent verbal and written presentation skills
Excellent problem solving ability and critical thinking skills and able to work independently and in collaboration with interdisciplinary team
Excellent computer skills using Office 365
Knowledge of Quality Improvement process, data analysis and program development
Demonstrated commitment to excellence in patient/family centred care
Able to work independently with minimal support.

The Humber River Health Oncology program provides comprehensive cancer care services in our outpatient clinic made of 28 treatment spaces and care for over 15,500 patients annually, including lab, systemic therapy, medical oncology, and supportive care visits. Our treatment and services include:
Chemotherapy, Immunotherapy, and Targeted therapies
Symptom management related to systemic treatments
Assessment and follow up from inter-professional cancer care team
Supportive care related to complex cancer pain and cancer-related symptom management
We are currently seeking an Oncology Nurse Practitioner (NP) to partner with another Oncology NP and work collaboratively with the interdisciplinary team, oncologists, palliative/supportive care physician and program leaders to provide direct and comprehensive care and services to our patients.
You will use advanced practice skills to provide assessment, coordination and provision of services to patients and families including crises intervention and short-term management while integrating best practice guidelines and current evidence-based research into clinical practice for in-patient and ambulatory clinic for oncology and oncology palliative population.
You will have the opportunity to participate and lead quality improvement initiatives and make recommendations for change in consultation with the Program Director, Manager and Physician Leader.
Your leadership skills will be utilized to provide training, coaching and education related to evidence-based Oncology practice and knowledge among team members.
Qualifications:
NP - Adult, RN(EC), Registration in good standing with the College of Nurses of Ontario
Minimum 3 years experience in acute oncology and systemic therapy experience
Masters degree in Nursing
Member of the Registered Nurses Association of Ontario, preferred
Oncology Nursing Certification (CNA), preferred
