The agency currently requires a: 1 - 12 Month Contract - Peer Support Worker.
We are currently seeking a Peer Support Worker who identifies as 2SLGBTQ+ to join our Youth Wellness Hub team. - Facilitate community engagement and outreach through connecting with organizations, schools, and service providers to increase awareness and access to the Youth Wellness Hub.
Qualifications:
- Degree or Diploma in Chid and Youth Care, Social Work, Psychology, Addiction and Mental Health.
- Lived experience as a young person with mental health and/or substance use challenges and lived experience as a 2SLGBTQ+ Young Adult. Your personal journey through mental health systems, identity development, or systemic barriers brings unique value to this role.
- Experience providing peer support or/or group facilitation in a formal or informal capacity, with strength in building rapport with youth.
- Strong self-awareness, self-care practices and can establish and maintain professional relationships with youth
- Strong communication and relational skills with the ability to build trust, listen deeply, and support youth with empathy and respect.
- Team-Oriented with ability to take initiative and work independently
- Possess a valid driver’s license with access to a vehicle.

We are seeking a bi-lingual individual to join our team - English, Spanish, Arabic and/or Farsi In partnership with Niagara Region and Youth Wellness Hubs Ontario (YWHO), Bridges Niagara is seeking a passionate, responsible, and creative professional to serve as a Care Coordinator and Peer Support Worker for newcomer youth (ages 12–25). As part of the Youth Wellness Team, this role combines case management with lived/living experience of mental health and/or substance use to provide both practical support and peer-informed guidance. The Care Coordinator/Peer Support works collaboratively with youth and a multidisciplinary team to help identify goals, navigate services at the Niagara YWHO site and in the broader community, and build skills, connections, and continuity of care. This includes one-on-one and group-based support, as well as engagement with families, caregivers, and other service providers to ensure holistic, youth-centered care.
About YWHO:
YWHO supports local service providers to work together to provide young people in Ontario aged 12 to 25 with low barrier access to a full range of services, including mental health and substance use supports, primary care, education, employment, housing, and other social services, all through youth-friendly access points.

Qualifications / Required Experience / Skills:

- Undergraduate degree/diploma preferred (psychology, social work, or relevant health or social
- science field).
- Registration with a professional body is considered an asset (College of Nurses of Ontario,
- Ontario College of Social Workers and Social Service Workers).
- 3-5 years of experience in healthcare or the social services sectors.
- 3-5 experience working with children, youth, families, and caregivers.
- Comprehensive knowledge of issues related to substance use, concurrent disorders, mental health, trauma, and recovery.
- Knowledge of the impact of social/cultural/economic factors on the determinants of health.
- Knowledge of community resources and services is considered an asset.

The Peer Support Worker will work as a member of a diverse interdisciplinary Youth Wellness Team to support youth experiencing mental health, substance use or other challenges.
As a Peer Support Worker, you will utilize your lived/living experience of mental health and/or substance use to support youth accessing the Hub to determine their unique goals and service needs. In this role, you will provide education about navigating mental health and substance use challenges as well as where to find resources in the community to address those concerns.
As part of the Youth Wellness Team, the Peer Support Worker will contribute to program development and outreach activities related to YWHOs Clinical Services, Skills & Well-being Activities and Community Support Services. The Peer Support Worker will be responsible for co-creating a welcoming space for youth who access Hub services, attending drop-in activities, as well as one-on-one mentorship.
POSITION DUTIES:
- Engage youth and create a welcoming environment for youth accessing Hub services.
- Connect with clients on an individual basis and also as a group facilitator.
- Support youth with the completion of standardized measures to help improve youth outcomes (focus on measurement-based care).
- Facilitate social and recreational activities, both within the Hub and (when relevant) community-based outings (e.g. gyms, community centres, land based programming).
- Provide individual mentorship and coaching to youth to navigate mental health system.
- Assist in the planning, outreach and co-facilitation of skills activities and drop-in programming.
- Complete documentation of client interactions and timely completion of data entry, case notes and other electronic records as required.

TERMS AND CONDITIONS:

About the Role:
The Permanent Supportive Housing (PSH) Program provides a long-term, affordable housing solution with flexible and voluntary support services offered to support high-acuity individuals who are homeless with housing retention and independent living.
This program is provided through multi-agency collaborations to support individuals living in the PSH building who are dealing with significant substance use and mental health issues (as well as other complexities) to gain housing stability and enhance their quality of life by providing housing, practical support, counselling, advocacy, landlord mediation, and eviction prevention.
The Peer Support Worker will provide important peer support services to tenants, and will play a vital role as a member of our multi-disciplinary teams by offering a range of practical and emotional support from the perspective of their lived experience, embodying a sense of hope and possibility for the future.

Direct Peer Support:

- Able to mediate and negotiate situations and possesses de-escalation skills
- Self-motivated, comfortable working with minimal oversight
- Proven computer proficiency to facilitate the use of automated assessment tools and Microsoft products
- Understanding of data collection for the purposes of statistical reporting and quality improvement

About the role
As our Community Support Worker, you will work as part of a vibrant team to provide care and support to people living in staffed homes within their community. You’ll encourage and empower the people we support to be independent and self-determining and to live the life they envision while maintaining excellent standards of care, health, and safety. Core Responsibilities
- Ensure the supported persons’ rights and choices are respected and advocate as needed
- Interact effectively with people we support
- Empower the people we support to be full citizens of their community
- Participate in the development, implementation, and documentation of Personal Service Plans
- Foster positive relationships with persons’ family members, professionals, co-workers, and members of the community
- Assist with activities of daily living and medication administration
- Excellent understanding of behaviour management principles and strategies including mental health needs
- Record the daily activity and personal health information (PHI) of the people we support on our case management database
- To work in conjunction with the people we support to ensure their home is clean and well maintained at all times, including daily food preparation and light housekeeping duties
You will have
- Grade 12 education
- Community Support Worker Diploma or equivalent
- Minimum 2 years of experience in community social services/health care
- Current Emergency First Aid/CPR and Food Safe certificates
- Previous behaviour management course/training
- Ability to effectively communicate (verbal & written) with team members, professionals, families, and community members
- Non-verbal communication skills
- Restricted class 4 driver’s licence (for Full-Time positions)
- Satisfactory Driver’s Abstract
- Criminal record clearance for children and vulnerable adults
- TB Test
- Moderate computer literacy
- Eligibility to work in Canada

- Valid Class 5 Driver's Licence at minimum
- Satisfactory Driver’s Abstract
- Criminal record clearance for children and vulnerable adults
- TB Test
- Moderate computer literacy
- Eligibility to work in Canada
- Skill in Music, swimming and basketball will be an asset
