ICU COVERAGE
- Seven (7) day shifts of ICU Most Responsible Physician duty in 8 bed unit
- Weekend shifts may also be available
- Must be competent in airway management including non-invasive ventilation, intubation and invasive ventilation, and invasive monitoring including central line and arterial line insertion
ON-CALL
- The GBGH Internists will arrange their schedule to ensure coverage 24 x 7 x 365 for response to Emergency Department consultations, Inpatient consultations and ICU Most Responsible Physician Duty
- Read ECG, PFTs when on call
IN-PATIENT
- Consultations for GBGH credentialed staff and Emergency Department Follow-up in collaboration with Family Physicians and Hospitalists
OUT-PATIENT
- Provision of out-patient consultations in Medicine Consult Clinic Outpatient procedures such as paracentesis and thoracentesis Cardiac stress testing
QUALIFICATIONS & EXPERIENCE
The ideal candidate will possess the following key competencies, record of experience and academic preparation:
- College of Physicians and Surgeons of Ontario (CPSO) unrestricted license that allows the physician to fulfil the requirements of this position
- Fellowship in Internal Medicine recognized by the Royal College of Physicians and Surgeons of Canada (RCPSC)
- Previous General Internal Medicine and Critical Care experience in a community hospital setting preferred

Position Summary:
As a salaried employee, the Physician provides and manages primary care for marginalized clients with varying degrees of social and medical complexities in accordance with the current College of Physicians and Surgeons of Ontario Standards of Practice. The physician's holistic approach to client care utilizes current client education and therapeutic techniques. The Physician collaborates, consults, and participates as an equal multidisciplinary team member (internally and externally). Experience:
- A minimum of 3 years’ experience practicing in a clinical setting or community setting working with marginalized patients preferred.
- A minimum of 3 years of experience in providing care to socially and medically complex patients (e.g., multiple chronic conditions, substance misuse, homelessness, newcomers, individuals with mental health, etc.)
- Demonstrated experience in collaboration and working within an interdisciplinary team.
- Demonstrated experience and high proficiency in Electronic Medical Records and the use of virtual care tools, including AI scribing.
Skills and Abilities:
- Ability to practice utilizing a client-centered approach with a strong understanding of the impact of the social determinants of health on patient care management.
- Excellent time management skills with the demonstrated ability to effectively set priorities and complete the required administrative tasks.
- Excellent computer proficiency (i.e. MS Office – Word, Excel, and Outlook; Electronic Medical Records, AI Scribing)
- Superior verbal and written communication skills, including effectively communicating with internal teams and external partners.

Job Summary:
The Physician works to their full scope of practice as a member of the interdisciplinary team that supports CHCKW outreach teams that serve individuals who are homeless and precariously housed. Often, these individuals are more complex due to mental health and addiction issues. This position works closely with outreach clients and provides support to mobile primary care teams & will participate in the HART hub locations. In addition, our outreach teams work with other partners of the Inner City Health Alliance that serve the same clients.
Responsibilities:
- Perform assessment, diagnosis, treatment, health education/counselling and other medical functions as required according to the College of Physicians and Surgeons of Ontario standard of practice.
- Work from a Social Determinants of Health perspective to improve health outcomes for individual clients and/or for the development of community health and well-being, and to reduce health inequities.
- Use a Harm Reduction approach with clients who may exhibit behaviours that place their health at risk.
- Provide addiction and mental health care imbedded in the primary care model for people with complex physical and mental health needs.
- Triage client needs either over phone, by video visit or in person.
- Provide consultation services to Nurse Practitioners upon request.
- Contribute to chart documentation and participate in chart reviews and case conferences.
- Contribute to the development, implementation, monitoring and evaluation of medical protocols and directives.
- Participate in organizational and clinical specific training.
- Work with members of the Inner City Health Alliance (or other social service agencies) to contribute to larger initiatives such as vaccination clinics, isolation spaces, covid response, etc.
- As required, participate in the development, implementation, monitoring and evaluation of health promotion/disease prevention programs for individuals and client populations as determined by the Health Centre.
- Provide input/explore Outreach locations and connect with other agencies to create new partnerships regarding outreach client care
- As required, participate in team and staff meetings and other meetings and committee work as appropriate to support both service delivery and organizational goals.
- Work during both regular and extended hours (evenings/weekends) and in locations within Kitchener as determined by the Health Centre. Participate in the Centres on-call service as required.
- Work individually and collaboratively in the development and implementation of quality improvement initiatives and activities.
- Work in a manner that preserves privacy and confidentiality.
- Work in a manner that ensures client safety and minimizes risk to clients, volunteers and the Health Centre.

- Adhere to appropriate infection control practices.
- Perform any other duties as assigned by your supervisor.

Qualifications
- Medical degree from a recognized university.
- CFPC Certification or equivalent practical experience
- General license to practice from the College of Physicians and Surgeons of Ontario (CPSO)
- Membership in the Ontario Medical Association (OMA)
- Membership in the Canadian Medical Protective Association (CMPA)
- Demonstrated experience with vulnerable, complex populations
- Minimum of three years of community-based experience. Some combination of community and hospital experience may be considered.
- Demonstrated experience working in an inter-disciplinary teamwork environment
- Demonstrated experience working effectively with culturally and economically diverse populations.
- Demonstrated ability to communicate effectively in English, both written and verbally
- Competency in MS Office Suite programs and electronic medical records systems
- Ability to maintain strict confidentiality.
- Skilled in setting and maintaining strong boundaries with clients.
- Ability to obtain a Police Vulnerable Sector Check satisfactory to the Health Centre.
- 2-Factor Authentication for Outlook is App based (Microsoft Authenticator Application) and used to ensure the security of our online accounts. 2-FA requires the use of a personal mobile device with a recent operating system.
