The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.
The North Lambton Community Health Centre is currently seeking applicants for the following position:
0.6 FTE PHYSICIAN (Sarnia)
22.5 Hours/Week (3 days/Week) Permanent
The Physician will practice evidence-based medicine and utilize a holistic approach to client care, including acute curative care, promotion of healthy lifestyles and prevention of illness by involving the client in his/her diagnosis and treatment, and through sensitivity to non-medical problems and appropriate referrals both inside and outside the Centre. The Physician will also participate in case conferences and information sharing with other members of the multidisciplinary clinical team.
Qualifications :
- Current CCFP certification, registration with CPSO and a member in good standing. Current CMPA coverage.
- Ability to work as a team member with nurse practitioners, nurses, social workers, health promotion staff and support staff.
- Strong health assessment and organization skills.
- Demonstrated ability to function independently and interdependently within a collaborative inter-professional team setting.
- Experience in addressing social determinants of health and client-centred care.
- Experience working in a rural or aboriginal community an asset.
- Proficiency in the use of computers and various software applications, including electronic medical records.
This is a permanent position with flexible hours, working 22.5 hours per week Monday to Friday. The rate of pay for this position is 126.24 - 149.93 per hour (on a six-step grid), with benefits, including the Healthcare of Ontario Pension Plan. The rate will be based on experience and education. The incumbent will primarily be required to work at our Sarnia site.

Teladoc Health is growing rapidly across the country, we are expanding our services to include Urgent and Primary Care Centres (vUPCC) in British Columbia. We are looking to expand our current team of primary care physicians for our virtual programs as we scale our clinical impact further.
Urgent and primary care centres (UPCCs) provide access to same-day, urgent, non-emergency health care. UPCCs are an alternative to visiting the hospital for issues that don’t require emergency care. Working virtually at an UPCC offers a unique opportunity to practice in a collaborative team-based setting providing high quality care to communities across British Columbia without leaving your home.
Role and Responsibilities:
- Provide comprehensive care services to a diverse patient population
- Diagnose and treat patients triaged to the Teladoc Health team
- Provide care as an integrated member of the team of physicians, nurses, and nurse practitioners to support the UPCC
- Complete documentation, referrals, and other work in a timely manner to support positive patient experience
- Participate in huddles and handover sessions at start and end of shifts, as needed
- Collaborate closely with medical, nursing and management teams internally and externally
Qualifications:
- Ideally +3 years of direct clinical experience as a Primary Care Physician (will consider +1 year of clinical experience in a similar setting)

Teladoc Health Canada is looking for Primary Care Physicians (CCFP) to join our clinical team. This is an opportunity to support patients in their own communities with quality healthcare and to add capacity to existing systems.
Teladoc Health Canada is offering virtual primary care services through a partnership with the provincial health authority to all patients in Newfoundland and Labrador who are not currently attached to a family doctor. Patients can access care through Teladoc Health’s virtual platforms and receive comprehensive, publicly funded services. In this engaging and dynamic environment, you will play a key role in managing primary care consults and supporting patients across the continuum of care.
All healthcare providers on the Teladoc Health team have a shared responsibility for matters pertaining to clinical quality and patient safety, including cultural safety, clinician wellness and health equity bringing attention to issues faced by rural Canadians and underserved populations, particularly Indigenous communities.
Currently, Teladoc partners provincially to offer publicly funded services in an integrated manner, such as through our recent work supporting primary and emergency care in Newfoundland and Labrador. In such settings, we are intent on delivering a first point of contact to comprehensive, coordinated, and continuous patient care, helping provinces integrate and expand their virtual primary care & acute care visions. Role and Responsibilities:
- Assess, diagnose, and treat patients triaged to Teladoc Health Canada’s virtual care platforms
- Lead both scheduled and on demand consultations
- Maintain up to date and accurate records
- Participate in huddles and handover sessions at start and end of shifts, as needed
- Collaborate closely with medical, nursing and management teams internally and externally
- Support ongoing needs of patients via coordination of necessary care with virtual and local in person teams
- Advance relationship-based culture within our comprehensive care teams, physicians, nurses, allied health professionals and managers
- Escalate cases as required to designated shift leader and/or medical leadership team
- Support team members across programs via other duties, as reasonably requested
Qualifications:
- Minimum of +3 years of experience in a practice setting providing comprehensive care to a wide range of ages and conditions
- Current and unrestricted registration as a Physician in good standing, and must be actively registered with your applicable provincial college
- Board certification under CFPC (CCFP)
- Experience and demonstrated success working with diverse primary care clinical team members
- Experience working in the digital health sector preferred
- History of strong clinical performance
- A passion for high-quality and safe patient care
- Tech savvy with the ability to manage multiple applications simultaneously and comfort working on multiple monitors
- Proficiency with Electronic Medical Records
- Proficiency with Microsoft Office applications such as MS Teams and Outlook
