Term Contract - 12 month Maternity Leave
South-East Ottawa Community Health Centre (SEOCHC) is a non-profit, community-governed organization that provides a range of multi-disciplinary services to clients, including primary care, health promotion, social services, community and resource development, home support, and advocacy to address the social determinants of health. SEOCHC particularly responds to persons who face barriers to accessing health and social services and who live in Southeast Ottawa. Our services and programs are designed to meet the needs of our community.
Our Primary Care Outreach to Seniors program is seeking an experienced Registered Nurse to deliver high-quality services to frail seniors in their homes. The successful candidate will help to support client medical management and work across the Champlain Region as a member of an interprofessional team using a palliative care approach.

Position Responsibilities
- Able to conduct comprehensive geriatric assessments to design individualized plans of care
- Recent clinical experience working with seniors with mental health and substance use concerns
- Strong skills in clinical assessment, screening, illness prevention, and disease management to improve health outcomes and facilitate access to health services
- Experience with Advanced Care planning
- Able to have serious illness conversations
- Aptitude and interest in client education and health promotion with seniors
- Knowledge of community resources for seniors
- Able to work independently as well as part of a team
- Recent experience working in a community-based setting and/or ability to work independently in the community
Minimum Qualifications
- Registered with the College of Nurses of Ontario, with no restrictions to practice
- Certificate of Basic Life Support (BLS) for Healthcare Provider or equivalent such as Cardio-pulmonary Resuscitation/Automated External Defibrillator (CPR/AED) Level C
- Valid drivers license, insurance, and use of a personally owned vehicle
- Fluency in English and French
- Proficiency in the use of computers, software applications, and electronic documentation
- Vulnerable sector screening completed within the last 12 months
- Gerontological nursing certification and/or training is an asset

Qualifications
- Current Registration with the College of Nurses of Ontario
- Current Certification in BCLS and demonstrated ongoing competence through annual re-certification
- Certification in Blood Glucose Monitoring and demonstrated continuing competence in BGM through annual recertification
- Certification in IV Therapy with demonstrated ongoing competence
- Current certification in NRP (was NALS) and demonstrated continuing competence through bi-annual recertification
- Current course in fetal monitoring
- Current course in breastfeeding techniques
- Maternity experience in the past 12 months
- Ability to function independently in labour/delivery/postpartum/nursery setting
- Knowledge of First Nations culture an asset
- Reliable attendance record

Overview:
This member of the Canadian Mental Health Association works alongside a team of professionals whose focus is safety, stability and support for individuals in a mental health crisis who are in immediate contact with police but not suitable for incarcerations or admission to the hospital. The team would consist of a multidisciplinary team either within the Canadian Mental Health Association, Kenora Branch or another community agency. This member is responsible for assessing clients physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the a practitioner, and providing a range of treatment, rehabilitation, and support services both for a variety of programs offered by the Canadian Mental Health Association, Kenora Branch. The Registered
Nurse/Registered Practical Nurse will coordinate shift-management with other registered nurse/registered practical nurses within the Canadian Mental Health Association, Kenora Branch. This position reports directly to the Safe Bed Program Team Lead and Chief Executive Officer.
Qualifications:
Registration or eligible for registration with the College of Nurses of Ontario
Knowledge of mental health/illness and mental health difficulties and treatment modalities Assessment/ Evaluation skills
Working knowledge of community resources and agencies
Ability to work independently and in a team environment
Ability to manage conflict and crisis situations
Computer literacy and recording skills
Cultural Competence for diverse populations
Valid Drivers Licence
Criminal Records Check
Certification in CPR and First Aid or willingness to obtain same
Duties:
Adherence to all policies of the CMHA Kenora Branch
Assurance of confidentiality of all client, personnel and organizational records and information
Maintenance of client files and data files including updating of client information consistent with policies and
procedures of the CMHA Kenora Branch
Maintenance of professional qualifications and registration
Participation in ongoing training and education and application of new skills to practice
Coordinate shift management as per established policies and procedures
On call from Monday to Friday from 4:30 p.m-9:00 p.m. and on weekends and statutory holidays. When on
vacation coverage, sick etc. coverage is coordinated through internal programs
Provide crisis intervention, Advocacy
Take the lead role or participate in providing medication administration and medical services
Develop, revise, and maintain client psychopharmacological and medical treatment including transcribing,
administering, evaluating and recording psychotropic medications prescribed by the treating practitioner
Under the direction of the Safe Bed Program Team Lead & Chief Executive Officer and treating practitioner
manage pharmaceuticals and medical supplies
Provide case management for an assigned group of clients and their families
Function as a fully integrated member of the team for the client residing within the residences or other
identified agency programs
Assessment of client needs and mental status and the development of appropriate recovery plans
Facilitate and monitor the implementation of client recovery plans
Participate in staff meetings and recovery plan meetings
Participate on specific Committees or task or work groups as assigned by the Safe Bed Program Team
Lead and Chief Executive Officer
Provide a safe environment by ensuring adherences to Workplace Health and Safety Regulations and
Policies, Infection Control Guidelines, WHIMIS and Safe Work Procedures; including but not limited to Fire,
Disaster and Evacuation Plans
Ensure client safety and client safety activities are emphasized throughout orientation of new staff
members
Support an environment which avoids, prevents and corrects all activities or actions which may result in an
adverse outcome in the delivery of mental health services
Participation in administrative and office functions
Responsible for self care
