Key Responsibilities 

Education & Clinical Practice Support 

  • Assess educational needs, develop content, and deliver learning programs for clinical teams. 
  • Provide mentorship, orientation, and ongoing training for team members. 
  • Support performance improvement and continuing education. 
  • Lead organization-wide clinical initiatives and ensure alignment with best practices and regulatory expectations. 

Quality & Risk Management 

  • Analyze data and implement continuous quality improvement strategies.

Requirements 

Education & Certification 

  • Registered Nurse (BCCNM) or degree in a recognized health science/professional program. 
  • Certified in gerontology (preferred). 
  • Formal training or experience in adult education or clinical instruction. 

Experience 

  • Minimum 5 years in healthcare with at least 2 years in a clinical role (long-term care preferred). 
  • 5+ years in education or training roles with experience in quality improvement. 
Skills & Competencies 
  • Demonstrated alignment with Baptist Housing’s values and the Wellness Model of Care. 
  • Strong understanding of seniors’ needs, adult learning principles, and change management. 
  • Experience with RAI-MDS, PointClickCare, SharePoint, and Microsoft Dynamics.

    Overview:
    This member of the Canadian Mental Health Association works alongside a team of professionals whose focus is safety, stability and support for individuals in a mental health crisis who are in immediate contact with police but not suitable for incarcerations or admission to the hospital.     Melfort Family Physicians Group/ Melfort Medical Collective is seeking a Temporary Full-Time Registered Nurse. Reporting to the Supervision Physician and Clinical Manager, the Registered Nurse will be responsible for providing evidence-based nursing care within a family medical clinic supporting both acute and ongoing clinical needs, particularly in the area of prenatal and postpartum care. This role involves close collaboration with physicians and allied health professionals to manage patient care plans, contributing to holistic patient care.

    If you have a Bachelor of Science in Nursing (BSN) or equivalent and experience in a clinical setting, obstetrics care or public health, then this job is for you!

    This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support, that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31, 2027, with the possibility of an extension.

    Responsibilities:

    • Deliver patient care in accordance with current clinical guidelines and best practices.
    • Review and update records in the EMR, including allergies, lab results, diagnostic tests and health profiles.
    • Support and coordinate medication coverage applications, such as EDS, special support programs, NIHB, pharmaceutical assistance, and other relevant sources.
    • Facilitate referrals to members of the Primary Health Care team, including diabetic nurse educators, chronic disease nurses, dietitians, mental health and addictions counsellors, lactation consultants, public health nurses, and exercise therapists.
    • Case management of vulnerable populations (e.g., Multiwork).
    • Provide holistic postpartum care to mothers and newborns, monitoring newborn health and development, assisting with breastfeeding and feeding techniques and educating parents on newborn care
    • Offer emotional and physical to mothers throughout the postpartum period
    • Administer medications and treatments as prescribed
    • Provide follow up on test results as needed
    • Coordinate overall case management for prenatal patients ensuring prenatal care components (vaccinations, laboratory investigations, ultrasound appointments) are completed
    • Maintain clear communication throughout the care journey
    • Coordinate with other healthcare professionals to ensure comprehensive care on return to home community and resources available in their community
    • Communicate and document any concerns with the prescribing physician.
    • Provide care for chronic disease management as needed
    • Administer intramuscular (IM) and subcutaneous (SC) injections.
    • Perform dressing changes, wound management, and suture/staple removal.
    • Prepare and clean exam rooms, call patients from the waiting room, measure vital signs, and conduct urine tests.
    • Attend and participate in clinic huddles
    • Advocate for the rights and needs of parents and infants within the healthcare system

    Qualifications:

    • Current registration and good standing with the College of Registered Nurses of Saskatchewan (CRNS)
    • Completion of an accredited Bachelor of Science in Nursing (BSN) or equivalent.
    • Demonstrated knowledge of primary care and chronic disease management (e.g., INR management, STI treatment) as per CRNS guidelines and employer-approved protocols.
    • Certified or eligible for RN Specialty Practices (e.g., INR Management, STI treatment) as per CRNS guidelines and employer-approved protocols.

    Job Description

    You will be providing specialized patient care in an active complex continuing care unit with a focus on providing care to our patient population; who require medically complex care, slow stream rehabilitative care, reactivation, or palliative care. You will also provide supportive care to their families.   You will assume responsibility for the coordination of care focusing on the patient’s transition through the continuum of care, patient and family education, and to support factors that impact customer satisfaction. You will be supported to provide a full range of nursing care within your scope of practice; which will include, administering of medications and procedures as per established policies and guidelines, completing health assessments and documenting observations and changes in your patient’s condition,  and collaborating with health team members to update individualized care plans to facilitate positive patient care outcomes. As well, as the RN, you may be expected to fulfill the charge role and provide support to your colleagues.

    Qualifications

    • Current Registration in good standing with the College of Nurses of Ontario
    • Current Heart & Stroke Approved BLS Provider Card upon hire
    • Proven ability to care and manage patients with tracheostomies, enteral/parenteral feeds, CADD pumps, PICC lines, IVs, Wound Management Systems, Fecal Management Systems 
    • Completion of a physical health assessment course
    • One year nursing experience in Long Term Care setting
    • Exceptional communications skills, verbal, written, and electronic
    • Demonstrated compassion, patience and empathy
    • Demonstrated leadership ability
    • Ability to multi-task and prioritize depending of changing needs of patients
    • Ability to work collaboratively in a team environment but also work independently as needed
    • Computer skills/CCRS data entry an asset
    • Evidence of dedication to continuing education

    • Must be able to meet the physical demands of the position

    Overview

    SEOCHCs Preventative Care & Wellness Clinics for Priority Populations is an innovative program designed to provide accessible, equitable and high-quality care to our community. At the heart of our mission is a commitment to servicing vulnerable populations, including newcomers, members of the African, Caribbean and Black (ACB) community, LGBTQIA+ individuals and other underserved groups. Our Walk-in Clinic will provide locally informed, culturally responsive interventions and clinical care to advance access to healthcare services with a focus on health promotion, preventative care and culturally responsive prevention models including proven cancer and chronic disease screening models. Our clinic offers walk-in and virtual care services 5 days/week, including evenings and weekends.

    The Registered Nurse will work collaboratively with an interprofessional team, including NPs, RMs, LCs, social workers and administrative assistants, to provide education, screening, chronic disease prevention and wellness support. Given that this is a new program, the RN is expected to actively contribute to its development, including refining workflows, identifying service gaps, and ensuring continuous improvement in service delivery.

    ** Please Note: This is a new program that is still in development and may go through new changes from time to time. As such, the job description of this role will be updated accordingly.

    Key Responsibilities

    Program Development & Implementation

    • Contribute to shaping clinic structure, processes, and service delivery.
    • Identify and implement best practices in preventative care for priority populations.
    • Collaborate with leadership to assess community needs and adapt clinic programs accordingly.

    Clinical Care & Health Promotion

    • Perform comprehensive health assessments, triage clients, and provide treatments within the RN scope, including medications, immunizations, wound care, and POCT.
    • Deliver culturally responsive, trauma-informed, and harm-reduction care that promotes prevention, wellness, and chronic disease management.
    • Provide health education, screenings, and client support to improve self-management and access to healthcare and community resources.
    • Maintain timely and accurate documentation, uphold PPE and infection-control standards, and participate in community pop-up clinics and health education sessions.

    Collaboration & Teamwork

    • Work collaboratively within an interprofessional team to deliver holistic, client-centered, and integrated care.
    • Support the NP with referrals, forms, and coordination with specialists and allied health professionals.
    • Engage community partners to strengthen services and client supports.
    • Take a lead role in ensuring clinic spaces and equipment are organized, well-maintained, and properly stocked.

    Data Collection & Quality Improvement

    • Identify service gaps and contribute to continuous quality improvement initiatives.
    • Support the development, implementation, and evaluation of clinic protocols, procedures, and performance measures.

    Qualifications

    • Bachelors degree in nursing
    • Registration in good standing with the CNO as an RN
    • At least 3 years of experience working as in primary care, serving equity-deserving members of the community
    • Strong critical thinking, problem-solving skills, organizational and interpersonal skills with an ability to work effectively within a multidisciplinary team environment
    • Ability to thrive in a busy and fast-paced environment
    • Strong understanding of social determinants of health and their impact on health outcomes
    • Familiarity with trauma-informed, culturally responsive and anti-oppressive care practices
    • Strong communication, interpersonal and conflict resolution skills
    • Proficiency with EMR systems
    • Vulnerable sector police check completed with the last 12 months
    • Proof of COVID-19 vaccination status will be required for employment
    • Languages spoken other than English and French
