Overview:
This member of the Canadian Mental Health Association works alongside a team of professionals whose focus is safety, stability and support for individuals in a mental health crisis who are in immediate contact with police but not suitable for incarcerations or admission to the hospital. The team would consist of a multidisciplinary team either within the Canadian Mental Health Association, Kenora Branch or another community agency. This member is responsible for assessing clients physical health needs, making appropriate referrals to community physicians, providing management and administration of medication in conjunction with the a practitioner, and providing a range of treatment, rehabilitation, and support services both for a variety of programs offered by the Canadian Mental Health Association, Kenora Branch. The Registered
Nurse/Registered Practical Nurse will coordinate shift-management with other registered nurse/registered practical nurses within the Canadian Mental Health Association, Kenora Branch. This position reports directly to the Safe Bed Program Team Lead and Chief Executive Officer.
Qualifications:
Registration or eligible for registration with the College of Nurses of Ontario

Duties:

On call from Monday to Friday from 4:30 p.m-9:00 p.m. and on weekends and statutory holidays. When on
vacation coverage, sick etc. coverage is coordinated through internal programs

Develop, revise, and maintain client psychopharmacological and medical treatment including transcribing,
administering, evaluating and recording psychotropic medications prescribed by the treating practitioner

Function as a fully integrated member of the team for the client residing within the residences or other

Facilitate and monitor the implementation of client recovery plans
Participate in staff meetings and recovery plan meetings
Participate on specific Committees or task or work groups as assigned by the Safe Bed Program Team

Reporting to the Supervision Physician and Clinical Manager, the Registered Nurse will be responsible for providing evidence-based nursing care within a family medical clinic supporting both acute and ongoing clinical needs, particularly in the area of prenatal and postpartum care. This role involves close collaboration with physicians and allied health professionals to manage patient care plans, contributing to holistic patient care.

This position is part of a newly developed Innovation Fund between the Saskatchewan Government and medical clinics across Saskatchewan with the aim of improving patient access and quality care through team-based support, that will deliver better access to primary care in the province of Saskatchewan. This position is temporary until March 31, 2027, with the possibility of an extension.
Responsibilities:
- Deliver patient care in accordance with current clinical guidelines and best practices.
- Review and update records in the EMR, including allergies, lab results, diagnostic tests and health profiles.
- Support and coordinate medication coverage applications, such as EDS, special support programs, NIHB, pharmaceutical assistance, and other relevant sources.

Overview
SEOCHCs Preventative Care & Wellness Clinics for Priority Populations is an innovative program designed to provide accessible, equitable and high-quality care to our community. At the heart of our mission is a commitment to servicing vulnerable populations, including newcomers, members of the African, Caribbean and Black (ACB) community, LGBTQIA+ individuals and other underserved groups. Our Walk-in Clinic will provide locally informed, culturally responsive interventions and clinical care to advance access to healthcare services with a focus on health promotion, preventative care and culturally responsive prevention models including proven cancer and chronic disease screening models. Our clinic offers walk-in and virtual care services 5 days/week, including evenings and weekends.
The Registered Nurse will work collaboratively with an interprofessional team, including NPs, RMs, LCs, social workers and administrative assistants, to provide education, screening, chronic disease prevention and wellness support. Given that this is a new program, the RN is expected to actively contribute to its development, including refining workflows, identifying service gaps, and ensuring continuous improvement in service delivery.

Education/Experience:
- Certificate of Registration with the Ontario College of Nurses (CNO)
- Knowledge, skills, and experience in: community assessment, psychosocial treatment and rehabilitation crisis management and recovery

The unit is supported by a Clinical Resource Nurse, a Professional Practice Clinician and a Patient Care Manager.
