Job Summary:
We are seeking a Registered Practical Nurse (RPN) with a heart for community nursing to join our team in the GTA area . This part-time, flexible role involves providing in-home nursing care, creating individualized care plans, and ensuring clients’ well-being. The ideal candidate is kind, patient, detail-oriented, and passionate about delivering exceptional care.
Responsibilities:
Assess clients’ health needs and develop personalized care plans for the PSW's to follow.
Provide holistic nursing care in clients' homes, ensuring comfort and safety.
Monitor and evaluate clients' conditions, adjusting care plans as needed.
Maintain accurate and timely documentation of client care and progress.
Communicate effectively with clients, families, and other healthcare professionals.
Advocate for clients by promoting their rights and well-being.
Participate in community outreach as needed to build trust and align with our value of giving back to the community.
Ensure compliance with health and safety standards.
Qualifications:
Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario (CNO).
Minimum of one (1) year of recent experience in community nursing.
Strong problem-solving and time-management skills.
Excellent interpersonal and communication skills, both verbal and written.
Proficiency in documenting and reporting client care.
Valid driver’s license and reliable vehicle (preferred but not required).
Second language is considered an asset.

- You are currently certified as a Registered Nurse (RN) or Registered Practical Nurse (RPN) with the CNO.
- Currently certified with Standard First Aid & CPR/ AED (Level C).
- Can possess current Mask Fit test.
- You are able to commit to weekends and on-call
