POSITION SUMMARY:
St. Joseph’s Home Care (SJHC) is dedicated to providing quality, compassionate, and client centered care to individuals in their homes and/or alternative settings through visiting, shift or the Integrated Comprehensive Care programs. Registered Practical Nurses enhance the life of their clients by providing a broad range of care needs within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Practical Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community.
CORE DUTIES AND RESPONSIBILITIES
- Utilizes the nursing process effectively, in order to provide high quality Wound and Ostomy Care for clients and/or caregivers.
- Collaborates with the interdisciplinary health team to provide cost-effective, high-quality Wound Care, using Best Practices.
- Applies current knowledge and research of Wound Care Protocol to practice.
- Creates, in consultation with the physician, CCAC Case Manager, collaborate with ICCP coordinators and Primary Nurse, a plan of care for patients requiring wound care in the community.
- Initiates plan of nursing care in consultation with the client and/or caregiver and modifies plan of care on an ongoing basis.
- Promotes the independence of clients and caregivers considering their needs, strengths and weaknesses.
- Utilizes available resources, facilitates clients/caregivers' problem solving, participation in decision making, and access to appropriate resources.
- Participate in the evaluation and refinement of the Wound Care Protocol and overall program.
- Involved in Education of other members of the SJHC team on the protocol and usage of products, providing guidance to best treatment choices
- Maintain regular communication with supervisor to include review of problems or unique issues and exchange feedback
- Participates in Bi-Weekly nursing case rounds

KEY EDUCATION AND EXPERIENCE
- Diploma in Practical Nursing from an accredited College, required
- Current and unrestricted Registered Practical Nurse license, required
- Member in good standing with Ontario College of Nurses, required
- A current Ontario driver’s license in good standing, required
- Proven ability in utilizing MS office software including excel spreadsheets, required
- Minimum 1-3 years of experience in home & community, acute and/or long-term care nursing, required
- Familiar with Procura, Procura Mobile or other scheduling software, as asset
- Certification as a wound care nurse, or equivalent courses; IIWCC or SWAN course recommended
- Previous experience in complex wound care, required
- Previous experience in ostomy and/or continence care, preferred
- Previous experience with palliative patientives, preferred
- Demonstrated excellence in direct client care
- Excellent verbal, written communication and interpersonal skills
- Ability to engage in daily travel in a defined geographical area, required

Community Wound Care Clinic Nurse Coordinator
To provide wound care services and coordinate with other wound care service providers within WNHAC and the interprofessional team and other wound care service provider organizations to provide clients a range of treatment services representing both Indigenous and contemporary models of care.
The Community Wound Care Clinic Nurse Coordinator (CWCCNC) will play a pivotal role in creating an urban Community Wound Care Clinic (CWCC), which will provide accessible wound care services emphasizing client-coordinated wound care plans across all sectors within the professional scope of practice established by the College of Nurses of Ontario
This position is part of the Ontario Nurses Association Bargaining unit.
NOTE: Indigenous traditional practices involve cultural ceremonies and burning sacred medicines including tobacco, sweetgrass, sage and cedar within the work setting.
QUALIFICATIONS:
current registration as an RPN with the College of Nurses of Ontario, and willing to work to a full scope of practice including performing delegated acts through medical directives;
Registered Practical Nurse specialized in Wound, Ostomy and Continence care certification is preferred;

The MDS/RAI Coordinator & Wound Care Back Up reports to the Director of Care and is responsible for managing the information technology and compliance related to Resident Care through coordinating the RAI-MDS process and monitoring of Quality outcomes and CIHI reports), the quality of resident care and nursing restorative philosophy and furthermore ensures compliance through coaching, mentoring, and discipline as required. Also this position will be responsible for coordinating and overseeing all aspects of wound care management.
Responsibilities:
- Responsible for an overview of Informatics on Quality Improvement and RAI-MDS 2.0 process of new staff or students.
- Responsible for education and software training to all appropriate staff members on RAI-MDS 2.0, RAPS, subscale outcome measures, quality indicators, and reports.
- Coordinates the AIS certification of all newly hired registered staff and annually thereafter
- Informatics and RAI-MDS 2.0 assessment process
- Schedules and monitors all informatics on residents assessments
- Working in consultation with management and interdisciplinary team members to develop processes that ensure data integrity, efficient data collection process, assessment completion and data utilization
- Coordinates the MDS Analytics process and trends coding deficiencies
- Establishing data quality processes to evaluate MDS coding accuracy and its timely completion
- Generates and evaluate RAI MDS 2.0/CIHI reports. Discusses findings with department heads and use them in clinical and operational decision making.
- Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
- Required to participate in the improvement of the indicators and achieve satisfactory results.
- Co-ordinates the data submission process to Canadian Institute for Health Information (CIHI)
- Takes the lead of the Nursing Restorative program in the home
- Mentors and coaches the Nursing Restorative Therapists
- Closely monitors ADL scores, outcome scores, and Special Rehab RUG in relation to nursing rehab.
- Interprets legislative requirements to the nursing team and monitors compliance to standards through continuous quality improvement activities.
- As a member of the in-home management team, participate in the recruitment, selection, training, and performance management of in-home staff, analysis of related management information, and in the implementation of related measures
- In collaboration with the Director of Care, monitors RAI MDS expenditures monthly to ensure that expenses are within budget.
- Assess wounds as per the Skin Care and Wound Management Protocol
- Assist the Registered Staff with the dressing of complex wounds as per MD orders
- Analyze data collected of wound prevalence in WoundRounds
- Other duties as assigned and or indicated in Job Task Inventory
Qualifications:
- Registered health professional licensed under the Health Disciplines Act
- Well-developed written and oral communication skills
- Knowledge and experience with adult education strategies/facilitation
- Proficient computer skills and strong problem solving abilities
- Ability to work with and relate to interdisciplinary team members and other internal stakeholders
- Knowledge of RAI-MDS – AIS certified as RAI Educator, if not must be completed successfully as soon as possible, i.e. within one month
- Skills and knowledge in trending and analyzing data
- Knowledge of Quality Improvement
- Minimum of two (2) years’ experience in a long term care setting.
- Minimum of 2 years supervisory experience desired
- Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
- As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements

- Attends and participates in resident care conferences as required.

- Assesses changes in health status and reports changes to Substitute Decision Maker (SDM) and interdisciplinary team
- Completes assessments (RAI-MDS and other assessments) develops and updates plan of care for each resident in collaboration with the resident and/or SDM and the interdisciplinary team

Qualifications :
The qualifications needed to join our family are as follows:
- Current Certificate of Competence with the College of Nurses of Ontario as Registered Practical Nurse or a Registered Practical Nurse registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
