Job DescriptionThe Georgetown Hospital Emergency Department serves the communities of Halton, Halton Hills, and the surrounding areas. Our ED is a 15-bed unit with a separate Rapid Assessment Fast Track (RAFT) zone. We serve patients of all ages and treat patients of every medical and mental health illness. Last year we served over 39,000 patient visits.
Our 24-hour staff are comprised of nurses, physicians, physician assistants and many other team members of a robust multidisciplinary team. Our team includes a dedicated mental health and internal medicine resources to ensure comprehensive assessments of patients for timely referrals and/or admission.
Qualifications- Current Registration in good standing with the College of Nurses of Ontario
- Current Heart & Stroke Approved BLS Provider Card upon hire.
- Previous Emergency Department experience required
- Experience in triage and in-charge responsibilities required
- ACLS certificate required at time of hire and ongoing.
- Completion of an on-site sponsored course is required within one year of hire date
- Successful completion of Canadian Emergency Nursing Certification Exam would be an asset
- Successful completion of post-diploma Emergency courses, e.g. TNCC, ENPC, PALS, would be an asset
- Basic ECG interpretation and 12 lead ECG application and interpretation preferred
- Must be able to meet the physical demands of the position
- Satisfactory employment and attendance record required

Hours: Monday to Sunday - Various rotating 12 hour shifts (hours subject to change based on operational needs)

The program also collaborates with Home and Community Care Support Services to ensure continuity of care and smooth transitions back into the community. ABOUT THE ROLE
Join our dynamic interprofessional rehabilitation team delivering high-intensity, time-limited care to clients with musculoskeletal conditions (e.g., hip/knee replacements, hip fractures, amputations) and/or spinal cord injuries. Collaborate with a multidisciplinary team including Physicians, Occupational/Physiotherapists, Nurses, Social Workers, Dietitians, Speech Language Pathologists, and Spiritual Care Associates, with access to Psychologist referrals as needed.
The Registered Nurse (RN) is responsible for assessing clients, developing a plan of care, implementing treatments, and evaluating the outcomes, in consultation with the client and/or other health professionals, in accordance with the Standards of Practice for the College of Nurses of Ontario (CNO). The RN works as part of an interdisciplinary team.
Staff will be oriented to both 3 North and 3 South and will be scheduled to work on either unit based on operational needs.

Education/Experience:

ABOUT THE JOB:
The Akausivik Inuit Family Health Team (AIFHT) is currently looking forRegistered Nurses to work conjointly with the interdisciplinary team of physicians, case managers, interpreters and other service providers to serve as medical and cultural interpreters for AIFHT patients. The Registered Nurse supports the activities of the staff physicians, residents and students. The position provides a high level of patient care in a team of health professionals and is responsible for co-ordination of patients' care with other health care professionals and outside resources, patient follow up, triage, developing an ongoing awareness of the various programs and services available in the community, acting as a health resource to community members, and providing for patient care in the office, including assisting at minor procedures and general nursing care. The registered nurse participates in the education of patients and their own professional development.
MAIN DUTIES AND RESPONSIBILITIES:
- Support to patients and families
- Interprets reports to patients as requested by physician
- Provides emotional support in difficult situations
- Assists individual patients in setting objectives and goals
- Works in liaison with community resources to meet the needs of the Centres patients
- Patient education; provide individual and group sessions to patients in the understanding of physical care
- Arranging consultations, referrals and tests
- Team support and communication
- Maintenance of Electronic Medical Record

SKILLS/QUALIFICATIONS REQUIREMENTS:
- Two years clinical experience in a Primary Care setting as a Registered Nurse
- Diploma or Baccalaureate Degree in Nursing
- Current membership with the College of Nurses of Ontario
- Current membership in professional associations, RNAO, NPAO
- Demonstrated competence in team building, conflict management, and interpersonal effectiveness
- Teaching experience will be considered an asset
- Excellent oral / written skills
- Strong clinical skills
- Demonstrated knowledge and understanding of Inuit culture
- In depth knowledge of chronic illness care, including both clinical and supportive care
- Able to be innovative and creative in order to develop strategies that successfully meet the needs of diverse patient, family, and provider population
- Flexible
- Ability to work effectively with all members of the health care team

The North Lambton Community Health Centre is a community governed health centre serving identified priority populations in a defined catchment area. The Centre operates multiple sites in Ontario: Forest, Kettle & Stony Point, Watford, and Sarnia. Our interdisciplinary team provides primary health care, health promotion and community development services.
The North Lambton Community Health Centre is currently seeking applicants for the following position:
Registered Nurse - Cardiac Rehab/Heart Failure Team
18-Month Contract, up to 37.5 hours/week
We are seeking an experienced Registered Nurse with specialized training in cardiac care. The RN will work within a team to implement evidence-based care to clients in our Cardiac Rehabilitation and Heart Failure Program.
Key Responsibilities:
- Assess, monitor, and support guideline-based therapies for clients with heart disease/heart failure
- Support exercise stress testing with nursing team and Specialists
- Provide client education and counselling aligned with best practices
- Collaborate with interdisciplinary team members and participate in care planning
Qualifications:
- Registered Nurse and in good standing with the College of Nurses of Ontario (CNO)
- Advance Life Saving Certification, an asset
- 35 years of cardiac care experience; ECG proficiency an asset
- Excellent communication, problem-solving, and organizational skills
- Strong commitment to patient- and family-centered care
This is a contract position, working up to 37.5 hours per week, commencing in December. The rate of pay for this position is $ hourly (based on a six-step grid) plus 10% in lieu of benefits. The incumbent work primarily at our Cardio-Pulmonary Rehab location but must be able travel at all four sites and any of our outreach locations, as required. Flexible hours of work are required.
