Position Summary and Scope of Service:

Reporting to the Supervisor, Acquired Brain Injury, or to the Service Area Manager, the incumbent is responsible for the delivery of services and supports to adults & children admitted to this service. The PSW-Stabilization provides personal support services in accordance with the Mission and Values of the organization and applicable provincial legislation and regulations. The PSW-Stabilization ensures that services are provided in a manner that:

  • Optimizes dignity, self-determination and attainment of personal outcomes;
  • Maximizes environmental and lifestyle features required to maintain safety and achieve a good quality of life;
  • Incorporates high quality, consistent support and intervention as prescribed by the clinical team
  • Supports successful transitioning back to a community home and/or successfully remaining in a community home.

Qualifications:

Educational:

  • Health Care Aide certificate from an approved educational institution required. Suitable combinations of education and experience may be considered.
  • Completed Diploma, Certificate or Degree in a related field (Disability & Community Support, Psychology, Counselling, ABI, etc.) preferred. Suitable combinations of education and experience may be considered.
  • Grade XII Diploma or GED required
  • Current certification in CPR and Emergency First Aid required in ABI homes
  • Current Certification in Nonviolent Crisis Intervention (NCI) required.
  • Successful completion of St.Amant training requirements within prescribed time frames.
  • Valid Class 5F Manitoba Driver's License and Abstract in good standing required, Class 4 preferred.

Professional:

  • Minimum of 3-years' experience providing residential and/or community support services to people with Intellectual / Developmental Disabilities, Autism, Acquired Brain Injuries or other neurodevelopmental disorders.
  • Minimum of 1-year previous experience working with individuals presenting with aggressive and/or disruptive behaviours, and, a demonstrated ability to assess safety risks and respond appropriately in challenging situations.
  • Knowledge of current trends in services for individuals with neurodevelopmental disabilities.
  • Familiar with the principles of a Person-Centred & Family-Centred approach to support & care planning.
  • Demonstrated experience working effectively as a member of a team.
  • Demonstrated experience in supporting / completing routine household management tasks (cooking, cleaning, shopping, transportation coordination, etc.).
  • Basic computer skills (Microsoft Office).
  • Proficient in English (and French for designated bilingual positions).
  • Strong oral and written communication skills.

Personal:

  • Positive, energetic and able to effectively manage time and prioritize workload.
  • Possess the physical and emotional capacity to meet the demands of the job, including the ability to utilize physical interventions when required to maintain safety of individual supported, self and team.
  • Ability to engage in self-reflection and be accountable for decisions and actions.
  • Acceptable Criminal Record, Child Abuse Registry and Adult Abuse Registry Checks, submission of a satisfactory Driver's Abstract.

Duties and Responsibilities:

Personal Care / Support Duties and Responsibilities:

Consistent with the Mission and Values of the organization, the PSW-S is responsible for the provision of personal support services (within assigned home or service area) that maximize the development of personal independence and choice-making while attending to safety / management of risks. In providing these services, the PSW-S is guided by the policies & procedures of the organization and the requirements of external accrediting or regulatory bodies.

Duties Include:

  • Provide individualized support as required, to assist individuals with the activities of daily living (e.g. instruction / supervision / prompting and/or hands-on assistance with personal care routines, engagement in social / recreational / educational / vocational activities, etc.).
  • Participate in the Individualized daily or discharge planning process as assigned.
  • Provide care and support in a manner which is holistic, person- and family centred and community-focused.
  • Provide a structured, consistent and predictable daily routine as outlined in the clinical plan or in the daily action plan.
  • Complete documentation and reporting as required, being careful to protect the individual's privacy.
  • Promote and facilitate connection to family and friends. Facilitate celebrations and cultural traditions that are important to the individual and his/her family or community.
  • Explore recreational resources and assist people to plan and safely engage in a variety of activities addressing physical, emotional, intellectual, cultural and spiritual needs / interests.
  • Transport individuals to activities and/or appointments using agency vehicles, or, support the individual to learn and safely use the public transportation system when this is identified as a goal.
  • Employ NCI principles and SBMT as well as low arousal strategies in responding to situations wherein individuals are not coping and are engaging in escalated or unsafe behaviour. Debrief with individuals and team members once the situation has been managed and ensure that required documentation is completed and distributed within identified timeframes. Notify immediate supervisor and/or on- call personnel regarding any circumstance that creates an immediate safety risk for individuals and/or staff.
  • Assist individuals with management of their personal finances as outlined within the individual plan and organizational policy.
  • Monitor the individual's health status (physical, mental, emotional) and facilitate access to appropriate health services, as required. Assist people to attend scheduled medical or other health related appointments and document all relevant information for follow up.
  • Perform any delegated health care tasks in accordance with nurse-delivered training and delegations (medication administration, seizure management, enteral tube feeding, etc.) in ABI homes; in Stabilization on outings, vacations, etc.

Team work & Safety:

The PSW-S contributes to a cohesive and collaborative team environment and safe workplace.

Duties include:

  • Share support and encouragement and required information with all team members. Ensure required documentation is completed prior to leaving shift.
  • Participate in the orientation and training of new team members.
  • Participate in required training.
  • Maintain a safe work environment by working in compliance with legislation and established workplace practices and procedures.
  • Participate in/conduct emergency codes and drills as required.
  • Maintain property and equipment in clean, safe working order. Report damage to property or equipment to Supervisor and inform the on call administrator immediately if the damage creates a safety concern.
  • Attend & participate in regular team meetings.

Resource Utilization:

The PSW-S ensures that materials, supplies and financial resources within his/her care and control (whether belonging to an individual or provided by St.Amant) are safely stored, accurately accounted for and efficiently utilized.

Duties include:

  • Ensure the safe storage, accurate accounting and documentation of money/credit card/purchase cards kept in the home/unit.
  • Ensure that an adequate supply of required materials/resources is maintained, facilitate the ordering/purchasing of materials / supplies, when required.
  • Ensure efficient use of all materials, supplies, and equipment.
  • Document all financial transactions in accordance with St.Amant policies & procedures.

Quality Assurance and Process Improvement:

The PSW-S supports continuous quality improvement within his/her service area and the larger organization.

Duties include:

  • Participate actively in establishing and supporting achievement of the personal goals that individuals set for themselves during the Individual or Discharge Planning Process.
  • Document and report Incidents/Occurrences in accordance with St.Amant policy and procedure and applicable legislation/regulation. Ensure that suspected abuse, neglect or undignified treatment is immediately reported and that the individual is protected from potential further victimization.
  • Recommend ways and means of improving the quality of services.
  • Participate in process improvement activities and organizational committees, as assigned.
  • Identify inefficiencies and/or opportunities for improved efficiency in service.
  • Collect indicator data regarding programs and activities, as required.
  • Identify educational opportunities and service opportunities that would contribute to personal and/or organizational development.

Direct Support Worker: We seek compassionate individuals to provide essential care and assistance to individuals with intellectual and developmental disabilities. Responsibilities include:

  • Providing personal care and support (e.g., hygiene assistance, dressing, toileting, mobility support)
  • Attending and advocating at medical appointments
  • Managing medication regimens (following proper protocols and documentation)
  • Grocery shopping and meal planning
  • Meal preparation & cooking (considering dietary needs and preferences)
  • Cleaning & maintaining the home (household upkeep, laundry, organizing)
  • Providing emotional support and companionship
  • Supporting individuals in recreational and community engagement activities
  • Building strong, professional relationships with those you support
  • Crisis prevention, intervention, and de-escalation when needed
  • Maintaining clear and accurate documentation (incident reports, communication logs, daily notes)
  • Promoting independence while ensuring safety and well-being
  • Encouraging life skills development (e.g., cooking, cleaning, self-care, social skills)
  • Maintaining professional boundaries while fostering trust and support


Qualifications:

  • High School Diploma.
  • 1-2 years of full-time experience working with adults with an intellectual disability.
  • Some post-secondary courses in the social services fields (Community Support Work, Mental Health courses)
  • Valid Driver's License is an asset
  • Specific training in the support work field (NVCI,ASIST, Mental Health First Aid)
  • Excellent workplace communication skills (written & verbal)
