The Licensed Practical Nurse (LPN) provides essential nursing care to clients in various clinical and community settings, including rural and remote First Nations communities . Key Responsibilities:Direct Patient Care:
- Assist clients with personal care activities (bathing, dressing, grooming, mobility)

**Job Summary**
The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. Provides disposition and discharge planning, risk assessment, case management, and coordination of services to provide continuity of care.

**Job Summary**
The Clinical Social Worker coordinates the development and implementation of a patient care/treatment plan through use of diagnosis and clinical assessment of psychiatric, chemical dependency and psychosocial needs as they relate to medical and behavioral care needs. + Performs assigned tasks within the scope of a Certified Nursing Assistant and in accordance with PeaceHealth and general nursing policies and procedures.
