**Location**

**Responsibilities**
+ Ensure compliance with regulatory requirements, and responsible for securing and developing all records, equipment and programs related to occupational health services.
+ Provide first aid at site for RFR personnel, evaluate and treat injuries and illnesses in an efficient and professional manner within the scope of practice and consistent with clinical nursing guidelines.
+ Provide post injury follow up with personnel
+ Maintain treatment log
+ Maintain first aid cabinet and first aid room supplies
+ Monitor and maintain documentation on any modified or restricted duty cases
+ Develop and support early and safe return to normal duty plan
+ Provide wellness information to RFR personnel
+ Liaise with internal claims management staff, WSIB and medical personnel regarding any claims /case related issues
+ Provide basic vision test for personnel as required
**Qualifications**
+ Must be a registered nurse in the province of Ontario
+ Basic understanding of regulations applicable to occupational and environmental health and safety
+ Detail oriented and have the ability to multitask
+ Excellent interpersonal skills
+ Occupational nursing accreditation or direct construction site experience preferred
+ Ability to manage cases with a view to providing site-based treatment whenever possible
+ Available to work night shifts

+ Document all pertinent information in the resident wellness file.

+ Ensure weights and vital signs are obtained monthly for each resident.
+ Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.
+ Maintain medical supplies and emergency kits for the community.
+ Provide clinical support and assistance to community team members as needed.
+ Understand and follow infection control practices.
+ Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.
+ Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
**Medication Programs**
+ Monitor each resident's medication and treatment profile regularly to ensure each medication and treatment is administered as ordered and documented accurately.
+ Transcribe physician orders Medication Administration Record Treatment Administration Record (MAR/TAR) to include initials, date and time 'noted', and date faxed to pharmacy accurately and promptly.
+ Complete MAR/TAR audit each month.
+ Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
+ Act as liaison for pharmacy services to ensure effective services for residents.
+ Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
+ Complete Medication Observations and skills checks for medication care managers at the direction of the RCD.
**Quality Assurance and Regulatory Compliance**
+ Demonstrate and is knowledgeable in the following key quality improvement areas:

+ Quality Care Indicators and Outcomes
+ Actively participates in Sunrise Quality Assurance Meetings and Clinical Meetings

Working as a part of the interprofessional care team, the Registered Practical Nurse (HART Hub) will deliver comprehensive care to DCHC clients, utilizing an understanding of the social determinants of health. The RPN will prioritize the health needs of DCHC’s priority populations, with a primary focus on delivering client-centered care and addiction-focused health services to individuals experiencing homelessness and/or substance use challenges in the community. The RPN will engage both internally and externally to assess population health needs and enhance well-being through targeted assessment, treatment, and prevention strategies aimed at reducing health inequities.

Key Responsibilities

  • Provide client-centered primary care in a non-judgmental manner, involving clients as partners in their care, recognizing the social determinants of health and associated health risks, and taking the stance of a health advocate.
  • Actively participate, seek, and provide consultation within an inter-professional model of care.
  • Perform clinical assessments and health information gathering for clients with complex needs.
  • Provide nursing services that address the biopsychosocial needs of clients, including those with multiple comorbid health conditions, mental health diagnoses, and/or concurrent disorders involving substance use/abuse.
  • Prioritize client-centered care for DCHC’s priority populations, including the homeless population and individuals recovering from addiction.
  • Perform intake assessment activities for new clients and provide direct access to primary care.
  • Contribute to providing more accessible, targeted services to DCHC clients through direct service, referrals, and navigation.
  • Complete timely and accurate medical records of client encounters, documentation, and thirdparty correspondence using DCHC’s Electronic Medical Record (EMR) system.
  • Support client safety through evidence-based practices and adherence to policies and standards, including communication and teamwork, infection prevention and control, safe medication practices, safe use of equipment and facilities, and risk reporting.
  • Contribute to activities related to the development, implementation, and evaluation of medical directives, policies and procedures, best practices, and protocols.
  • Adhere to and promote evidence-based practices and standards of best practice in keeping with DCHC clinic operational guidelines.
  • Actively participate in case conferences when required.
  • Participate in chart audits, development, and implementation of Quality Improvement Plans (QIP), and accreditation activities with the Canadian Centre for Accreditation (CCA).
  • Demonstrate active participation in the CNO Quality Assurance (QA) Program to ensure the maintenance of continuing competence to practice and with DCHC’s performance management requirements.
  • Participate in agency training activities and meetings as requested. • Provide clinical supervision and training for nursing students when applicable.
  • Ensure client care and caseload are managed to achieve the goals of DCHC’s Multi-Sector Service Accountability Agreement (MSAA).

Key Qualifications

  • Practical Nursing Diploma
  • Current registration with the College of Nurses of Ontario (CNO).
  • Membership in the Registered Practical Nurses Association of Ontario is an asset.
  • Experience providing care to individuals facing homelessness, substance use, or complex health and social challenges.
  • Phlebotomy certification is an asset.
  • Minimum of two years of experience working as a primary care RPN with clients with complex needs preferred.
  • Experience in a Community Health Centre (CHC) setting or other community health practice, preferred.
  • Experience working with communities and individuals facing barriers to accessing healthcare services preferred.
  • Excellent problem-solving, organizational, and interpersonal skills, with the ability to work effectively and respectfully in a collaborative, inter-professional team environment.
  • Ability to communicate with clients in an easily understandable manner, practicing active listening to understand client needs and coaching clients as collaborative partners in their healthcare.
  • Excellent written communication skills, including report writing for a clinical setting.
  • Basic CPR certification required. • Experience using Electronic Medical Records (EMRs) and the Ontario Telemedicine Network (OTN).
  • Proficiency with Microsoft Office applications (e.g., Word) is an asset.
  • Competency in facilitating client group activities.
  • Valid current vulnerable sector police check required upon hire.
  • Valid Ontario Driver's License and access to a vehicle.
  • Flexibility to work or drive to meetings at any of DCHC’s clinical locations in our service area, including mobile clinic sites, as required.
  • Ability to work a flexible schedule with at least one evening per week.

Responsibilities:

  • In collaboration with the Medical Director and Attending Physicians, provides comprehensive primary care to residents
  • Collaborates and consults with physicians, the Director of Care, nursing staff, interdisciplinary team members and external resources regarding residents’ plan of care.
  • Assists, supports, guides, and provides consultation to the Director of Care, Nurse Managers, nursing staff and interdisciplinary team members regarding challenging clinical situations.
  • Makes referrals to specialized consultants, services, and other health providers.
  • Performs a person-centered health assessment of residents on admission, annually and as needed (including medication reconciliation and quarterly medication reviews).
  • Conducts person-centered comprehensive health histories and participates in regular care conferences.
  • Orders and/or performs appropriate screening and diagnostic investigations, interpreting results and assuming responsibility for follow-up
  • Identifies and implements research-based innovations for improving resident care and collaborates with members of the interdisciplinary team and/or community to identify research opportunities and to conduct and/or support research
  • Other duties as assigned and or indicated in Job Task Inventory.

Qualifications:

The qualifications needed to join our family are as follows:

  • Registered Nurse Practitioner in good standing with the College of Nurses of Ontario, or a Registered Nurse Practitioner registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Comprehensive knowledge of evidence based clinical practices
  • Comprehensive knowledge of long-term care concepts and philosophies
  • Excellent interpersonal and communication skills
  • Post-graduate education and experience in gerontology (recommended)
  • Specialty certification in gerontological nursing (recommended)
  • Experience working as a Nurse Practitioner,
  • Experience in long-term care, perferred,
  • Experience providing leadership and supervision in any healthcare environment.
  • Ability to embrace various skills from the team to maximize the potential to enhance the well being of all individuals
  • To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into positive social interaction.
  • To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
