**JOB OVERVIEW**
The LPN Medication Care Manager is responsible for providing the highest degree of quality care and services by administering medication and treatments in a safe organized manner. Responsibilities include but are not limited to administration of medications, documentation of medication administration, conducting SHUs, and providing resident care while demonstrating the Mission for Sunrise Senior Living, "to champion the quality of life for all seniors" in accordance with federal, state/provincial, and local laws, standards and regulations and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents.
**RESPONSIBILITIES & QUALIFICATIONS**
Temporary Position for one year.
**Essential Duties**

**Medication Administration**
+ Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
+ Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents.
+ Receive medication updates from Resident Care Director (RCD) or Wellness Nurse.
+ Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
+ Ensure that medications are passed according to times utilizing a mobile medication cart.
+ Document and initial as medications are given and ensure that appropriate documentation is completed for refusal or missed doses.
+ Maintain confidentiality of all resident information including resident medication among other residents.
+ Report all resident concerns and unavailable medication while administering the medication to the RCD or Wellness Nurse.
+ Restock medication cart after all medication passes.
+ Assist in checking medication regardless of packaging system.
+ Assess the residents to determine need for "as needed medication" and appropriately document and report to supervisor.
+ Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
+ Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
+ Follow re-fill process for medications.
+ Help residents maintain independence and promote dignity and physical safety of each resident adhering to the Sunrise Principles of Service.
+ Strive to understand and respond to each resident with empathy, always remaining mindful of the resident's unique communication patterns, history, and basic human needs.
+ Practice routinely good standard care precautions of cleanliness, hygiene, and health.
+ Audit medication carts.
**Resident Care**
+ Notify RCD of any resident and/or family concerns.
+ Attend and actively participates in daily Cross Over meetings facilitated by the LCM.
+ Conduct Service and Health Updates as directed by RCD.
+ Participate in the development of the Individualized Service Plans (ISP).
+ Transcribe orders.
**Risk Management and General Safety**
+ Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements, promoting Risk Management programs and policies, and adhering to safety rules and regulations.
+ Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
+ Report all accidents/incidents immediately.
+ Report all unsafe and hazardous conditions/equipment immediately.
+ Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
+ Comply with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
+ Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.
+ Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately.
+ Ensure oxygen tanks are stored safely, exchange guest/resident's tanks when empty, and monitor to make sure liters of oxygen are at prescribed levels.
**Training and Contributing to Team Success**
+ Participate as a member of a team and commit to working toward team goals.
+ Demonstrate in daily interactions with others, our Team Member Credo.
+ Commit to serving our residents and guests through our Principles of Service.
+ Contribute to the overall engagement programs and processes (customer and team member engagement) including participating in the team member engagement survey and engagement improvement planning workshops.
+ Attend regular meetings; Town Hall, Department Team, Cross Over, Medication Technician and others as directed by the Supervisor/Department Coordinator.
+ Attend regular training by RCD and neighborhood coordinators.
+ May be designated as shift supervisor.
+ May supervise other medication care managers.
+ Maintain compliance in assigned required training and all training required by state/province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.
+ Perform other duties as assigned.

**Coordination of Health Needs**
+ Identify on-going needs and services of residents in promotion of the highest quality resident services to be delivered.

+ Assist Resident Care Director (RCD) in conducting resident evaluations and data gathers information at time of move in, 30 days post move in, semi-annually, and with any change in status.
+ Conduct monthly wellness visits for all residents.
+ Monitor and follow up with residents who have experienced any fall, accident, or incident and document and report as required and directed.
+ Document all pertinent information in the resident wellness file.
+ Maintain communications with resident's family and or responsible party to ensure medical needs for the resident are being met.
+ Contact resident's attending physician when necessary and/or upon family request.
+ Ensure weights and vital signs are obtained monthly for each resident.
+ Assist in maintaining wellness files according to Sunrise policies and federal and state/provincial regulations.
+ Maintain medical supplies and emergency kits for the community.
+ Provide clinical support and assistance to community team members as needed.
+ Understand and follow infection control practices.
+ Partner with the RCD and community team to promote infection control standards including any resident testing, training, and skills checks.
+ Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials in compliance with universal care precautions.
**Medication Programs**
+ Monitor each resident's medication and treatment profile regularly to ensure each medication and treatment is administered as ordered and documented accurately.
+ Transcribe physician orders Medication Administration Record Treatment Administration Record (MAR/TAR) to include initials, date and time 'noted', and date faxed to pharmacy accurately and promptly.
+ Complete MAR/TAR audit each month.
+ Demonstrate and is knowledgeable of the Six (6) Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
+ Act as liaison for pharmacy services to ensure effective services for residents.
+ Monitor the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
+ Complete Medication Observations and skills checks for medication care managers at the direction of the RCD.
**Quality Assurance and Regulatory Compliance**
+ Demonstrate and is knowledgeable in the following key quality improvement areas:

+ Actively participates in Sunrise Quality Assurance Meetings and Clinical Meetings

+ Resident Assessment/Evaluation process in accordance with state/provincial regulatory requirements

+ Provide clinical training and education, as needed, to nurses, medication care managers, care managers, and others who provide resident care. Perform skill evaluation to assure clinical capability of care team members.
**Quality Assurance and Regulatory Compliance**
+ Track, trend, and report clinical quality data to identify risk.
+ Participate actively as a member of the community Quality Assurance and Performance Improvement committee.
+ Lead clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes.
+ Recruit, hire, and train clinical team members and is responsible for performance management, evaluations, and engagement.
+ Complete direct report team member staffing and scheduling according to operational and budgetary guidelines.
+ Partner with the community leadership team to promote resident safety and compliance with Risk Management and national/provincial regulations pertaining to occupational health and safety.
+ Serve as the ICC and CLIA Director as applicable for the community.
+ Responsible for infection control programming, including delegation of infection control preventionist, as per provincial requirements.
+ Assure compliance with all Federal, State/Provincial, board of nursing, and other applicable regulations.
**Financial Management**
+ Manage the department budget to include labor/labour and other expenses and understand it's impact on the community's bottom line.
+ Processes and submit monthly expenses and budget data in a timely manner, per Sunrise policies and internal business controls.
+ Understand the internal costs associated with all Sunrise resident care programs.
**Training, Leadership and Team Member Development**
+ Partner with the Regional Director of Resident Care and/or Executive Director in the delivery and participation in Sunrise University training and self-study programs.
+ On-boards new RCD leaders and other department coordinators as needed.
+ Develop a working knowledge of state/provincial regulations and ensure compliance through supervising and coaching team members.
+ Complete clinical team member staffing and scheduling according to operational and budgetary guidelines when assigned to a community.
+ Hold clinical team accountable, correct actions when necessary and document.
+ Attend regular meetings; Stand Up, Cross Over, Department Head, Town Hall, QAPI, and others as directed by the Executive Director.
+ Keep abreast of professional developments in the field by reading and attending conferences and training sessions.
+ Maintain compliance in assigned required training and all training required by state/ province or other regulating authorities as applicable to this role to ensure that Sunrise standards are always met.

+ Provide the highest degree of quality care and services by administering medication and treatments in a safe organized manner.
+ Review, read, notate, and initial the electronic health record to document and learn about pertinent information about residents.
+ Receive medication updates from Resident Care Director (RCD) or Wellness Nurse.
+ Administer, assist with, and observe medications and treatments for each resident using the medication administration record and the Six Rights of Medication Pass ("Right" resident, medication, dosage, time, route, right to refuse).
+ Ensure that medications are passed according to times utilizing a mobile medication cart.
+ Document and initial as medications are given and ensure that appropriate documentation is completed for refusal or missed doses.

+ Count all narcotics with another Medication Care Manager (MCM) or Lead Care Manager (LCM) each shift and report discrepancies to the RCD or Wellness Nurse.
+ Maintain and clean the Medication Room, medication carts, and treatment carts for neatness, cleanliness, availability of medications, and expired medications.
+ Follow re-fill process for medications.
+ Help residents maintain independence and promote dignity and physical safety of each resident adhering to the Sunrise Principles of Service.

+ Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS), and Lockout Tagout procedures.
+ Report all accidents/incidents immediately.
+ Report all unsafe and hazardous conditions/equipment immediately.
+ Ensure any cords, carts, equipment, and other hazards are always kept out of the way, not blocking exits, and in compliance with fire codes.
+ Comply with all infection control techniques, placement of bio-hazard containers, removal techniques, procedures, and policies.
+ Understand and practice the proper method of attending to and disposing of, and the possibility of exposure to, blood borne pathogens, bodily fluids, infectious waste, sharp sticks, and hazardous materials.

+ Complete MAR/TAR audit each month.

+ Act as liaison for pharmacy services to ensure effective services for residents.

Notre personnel infirmier autorisé a l'occasion unique d'utiliser son expertise en soins infirmiers cliniques d'une nouvelle manière en dehors des soins aux patient(e)s, tout en continuant à collaborer avec les professionnel(le)s de la santé et à défendre le bien-être des patient(e)s grâce à la technologie des dispositifs médicaux. Vous serez responsable d'aider l'équipe de service à collecter les dispositifs et à faciliter les échanges de dispositifs d'une pièce à l'autre.
+ Naviguer dans les unités et coordonner avec le personnel infirmier de l'unité pour aider au retrait des pompes si nécessaire.
+ Aider avec les questions relatives à la mise à niveau du logiciel.
+ Soutenir le personnel de gestion dans la configuration de la pompe à perfusion.
+ Engagement à temps plein nécessitant des déplacements et une flexibilité pour travailler avec votre horaire actuel.
+ Horaire basé sur des missions avec un engagement de plus de 40 heures par semaine. Voyager à l'échelle nationale pour répondre aux besoins de la clientèle.

**Responsabilités :**
+ Ramasser les dispositifs en milieu hospitalier en collaboration avec le personnel infirmier local.
+ Utiliser l'expérience hospitalière antérieure et les compétences en matière de service à la clientèle pour garantir que le processus de collecte des dispositifs n'interfère pas avec les activités de l'emplacement.
+ Utiliser l'expertise des soins infirmiers cliniques pour répondre aux questions et aux préoccupations concernant le dispositif médical.
+ Fournir des mises à jour, des rapports et des commentaires précieux à la clientèle pendant et après les missions en service.
+ Travailler avec une équipe de technicien(ne)s pour gérer efficacement le flux de dispositifs pendant toute la durée du projet.
**Exigences :**
+ Diplôme d'infirmier(ère) requis (ADN ou BSN)
+ Expérience hospitalière ou industrielle requise
+ **Expérience en salle d'urgence ou en soins intensifs préférée**
+ Une expérience en leadership (gestionnaire, éducateur(trice), précepteur(trice)) peut être utile, mais n'est pas requise
+ Sens des affaires professionnel, y compris en communication et compétences générales requises
+ Excellentes compétences en documentation
+ Capacité à rester debout pendant de longues périodes, de se pencher, de s'agenouiller et de soulever de l'équipement jusqu'à 50 livres peut être requise
+ Vous devrez fournir 3 semaines de disponibilité par mois.
+ Doit avoir un véhicule personnel et être prêt à l'utiliser pour des missions liées au travail jusqu'à 3 heures de son domicile
+ Doit habiter à moins d'une heure d'un aéroport majeur
+ Capacité à travailler de manière autonome dans les locaux de la clientèle avec un minimum de supervision
+ Capacité à satisfaire aux exigences d'accréditation des fournisseurs hospitaliers, y compris la fourniture d'une preuve de statut vaccinal à jour, est requise

Our Registered Nurse experience a unique opportunity to employ their clinical nursing expertise in a new way outside patient care, while continuing to collaborate with healthcare professionals and advocate for the well-being of patients through medical device technology. You will be responsible for assisting the service team in device collection and facilitating room to room device swap outs.
+ Navigate units and coordinate with unit nurses to assist with pulling pumps as needed
+ Assist with questions pertaining to the software upgrade.
+ Support management staff with infusion pump configuration.
+ Full-time commitment requiring travel and flexibility to work with your current schedule.
+ Assignment-based schedule with 40+ hour commitment per week. Travel nationally to support customer needs.
You will have an opportunity to travel, grow, and expand into new opportunities while earning supplemental income and learning the medical device industry.
**Responsibilities:**
+ Collecting devices in a hospital setting while collaborating with the local nursing staff.
+ Utilizing previous hospital experience and customer service skills to ensure the device collections process doesn't interfere with the location's activities.
+ Utilize clinical nursing expertise to answer questions and address concerns with the medical device
+ Provide updates, reports, and valuable feedback to client during and after in-service assignments
+ Working with a team of technicians to effectively manage the flow of devices during the duration of the project.
**Job Requirements:**
+ Nursing Degree required (ADN or BSN)
+ Hospital or Industry Experience required
+ **Emergency Room or Critical Care experience preferred**
+ Leadership experience (manager, educator, preceptor) can be helpful but is not required
+ Professional business acumen including communication and soft skills required
+ Excellent documentation skills
+ Ability to stand for long periods of time, bend, kneeling and some lifting of equipment may be required of up to 50 pounds
+ You will be required to provide 3 weeks of availability per month
+ Must have a personal vehicle and be willing to use it for job related assignments up to 3 hours from home
+ Must be located within 1 hour of a major airport
+ Ability to work independently in customer settings with minimal supervision required
+ Ability to clear hospital vendor credentialing requirements, including providing proof of up-to-date vaccination status, required
**Duties may require compliance with client requirements that all those performing services on-site be fully vaccinated**

We are seeking an experienced registered nurse to join our team as a Night Shift Occupational Nurse, supporting the health and safety of personnel on job sites. In this role, you will provide medical support and lead occupational health and preventative initiatives on Kiewit Nuclear Canada projects.
**District Overview**

**Responsibilities**
+ Ensure compliance with regulatory requirements, and responsible for securing and developing all records, equipment and programs related to occupational health services.
+ Provide first aid at site for RFR personnel, evaluate and treat injuries and illnesses in an efficient and professional manner within the scope of practice and consistent with clinical nursing guidelines.
+ Provide post injury follow up with personnel
+ Maintain treatment log
+ Maintain first aid cabinet and first aid room supplies
+ Monitor and maintain documentation on any modified or restricted duty cases
+ Develop and support early and safe return to normal duty plan
+ Provide wellness information to RFR personnel
+ Liaise with internal claims management staff, WSIB and medical personnel regarding any claims /case related issues
+ Provide basic vision test for personnel as required
**Qualifications**
+ Must be a registered nurse in the province of Ontario
+ Basic understanding of regulations applicable to occupational and environmental health and safety
+ Detail oriented and have the ability to multitask
+ Excellent interpersonal skills
+ Occupational nursing accreditation or direct construction site experience preferred
+ Ability to manage cases with a view to providing site-based treatment whenever possible
+ Available to work night shifts

+ Regular, reliable attendance
+ Work productively and meet deadlines timely
+ Communicate and interact effectively and professionally with supervisors, employees, and others individually or in a team environment
+ Perform work safely and effectively. Understand and follow oral and written instructions, including warning signs, equipment use, and other policies.
